They can either share an Apple ID, or, if they create two seperate accounts, they can authorize the same computer with two different apple IDs so purchases across both accounts are accessible on the same machine.

The music is stored on the iTunes library. This library isn't deleted when you uninstall or reinstall iTunes, so you will keep everything. Anyway, it's better to make a backup of the library. Apart from that, you can download the purchased music as many times as you want -

Any of the above will work, depending how you would like to set it up.
Given that kids tend to eventually grow up and be independent, the best long term answer for most people is to let her have her own account and her own library as soon as she can handle it. Keep in mind that content purchased from the iTunes Store is permanently tied to the account from which it was originally purchased, so separating later is a challenge.
If you want to sync multiple devices to the same library, that will work. Or if you want separate libraries (as I would recommend), they can be either on separate computers or on separate Windows user accounts on the same PC.
For the name change: Connect the device. When the name appears in the left sidebar of iTunes, highlight it and change it. -

I set up FaceTime and Messages on four different devices with only one Apple ID. I used the exact method that is described in the video that AnaMusic has provided to you in the link in her response above. That method worked fine for my family, but you can create new Apple ID's just to use for FaceTime and Messages if you like. Many users prefer doing that as opposed to adding new email addresses.
IMO - using unique email addresses (like in the video) makes sense, because we (my family) all have different email addresses anyway, and it was very simple to add each family members email address to both apps and remove my Apple ID email address as the contact address on each device.
BTW .... if you are using your Apple ID on both devices - your phone number should be the number that is showing in the settings - not you daughter's. Did you set up the Apps with your Apple ID? -

From your backups. By doing a clean install you erased your startup volume. You may have installed OS X, but your personal software was erased. You can restore from backups if your have them. If not, then you've lost it all, permanently.

I have iTunes installed on a laptop that I use for work, but it's owned by my employer. A new policy my have them deleting iTunes from the machine. If that happens, will I lose all my music? I am an iCloud user and pay for iTunes match.
If they just delete iTunes, the music files should still be there. I suggest making a backup of them though. Locate your iTunes folder (The one that contains your library file) and just copy the whole thing to your Documents folder or wherever you like (Just not in the same directory of the original folder otherwise the computer will get confused and could confuse your iTunes library). Then you will have a backup of all your music, plus if you purchased it from iTunes, you will still have it linked to your Apple ID so you can re-download them to any computer with iTunes on it when you feel like it.

My itunes match subscription will be expiring soon. Will I lose all my music if I choose not to subscribe again?

If all your music is in your itunes library on your computer, you will not lose that music.
If you do not have the music on your computer and stream the music you will lose all your music except for previous itunes purchases. Likewise you will no longer have access to match music.

Once you are in Recovery Mode, it's too late to do any backup. I am afraid you are going to lose your data.
