If you don't know your password, don't know your security questions and don't have a rescue address or don't receive a reset email, you should contact AppleCare who will initially try to assist you with a reset email or if unsuccessful will pass you to the security team to reset your security questions for you.
If you are in a region that doesn't have international telephone support try contacting Apple through iTunes Store Support.

I am a lady and need your help. I upgraded by Ipad with the new ios 8.0.1 or 8.0.0 and after that i was unable to use my passcode. I then tried many time now i am ask to produce an icloud password, I know my icloud ID but cannot remember the password because the IT person in my office was the one who helped me set it up.
Now my iPad is useless and i cannot use. It keeps asking me the icloud password and when i follow the instructions i am told an email is sent to me for the reset password but i dont get any email.
Please Help.
I can provide you with all details on my ipad ranging from EMEI number and S/N if that can help. I also have an Apple ID which works fine.
PLEASE PLEASE HELP ME.

Thanks for the question. Based on your question, it seems like you havent received a password reset email. Are you using different Apple ID's for iCloud and other features? I would recommend that you read this article, it may be able to help you resolve or isolate the issue.

That is the activation lock. Your iPad will be useless unless you can remove it.

If you cannot remember the passcode, you will need to Restore your device...
Connect to iTunes on the computer you usually Sync with and Restore...

I have done a lot of restores to the Iphone, and resets also.
I;ve done holding both buttons as you sugested, but there is no signal appearing.
I am in Uruguay, South America and the original company is Movistar ( Telefonica) and the new sim card is from ANTEL ( the officail and biggest company in my country).
I don;t knoe what todo.
I have been trying for more than one mont.

Contact Apple through iTunes Store Support, and explain that you have forgotten your 3 security questions, that you can reset your password, but doing so doesn't reset your security questions.
Remember, support will receive hundreds, if not thousands of requests per day, some from people trying it on, others with little explanation and others that are written extremely poorly. Take the time to explain your situation properly, be precise and concise, brief but comprehensive.

You cannot return to previous state. The update re-locked the iPhone
to the original carrier. You must contact the original carrier to see if
they offer unlocking and if you qualify.
What does it say when you look at Settings=>General=>Carrier?
Be advised that if your iPhone had previously been hacked/modified/
jailbroken to work with other than original carrier, it may be impossible
for the original carrier to unlock it properly.

No. Apple has no part in the decision ot lock or unlock at all. Lock/unlock is 100% the decision of the carrier. Apple has nothing to do with this decision.
