My iphone screen is cracked. if i take it to the apple store will ot be able to be used at any carrier?

I have an iPhone 4 and heard that you can take it to the apple store and tell them what happen and they will give out a free repair one time only. Do they give you a completely new phone or do they just fix the screen ? And if they give you a new phone do you have to take it to your carrier to and reprogram it to your network?

Here is an excerpt of the 1 year warranty for an iPhone 4  / US:
This Warranty does not apply: (a) to consumable parts, such as batteries or protective coatings that are designed to diminish over time, unless failure has occurred due to a defect in materials or workmanship; (b) to cosmetic damage, including but not limited to scratches, dents and broken plastic on ports; (c) to damage caused by use with another product; (d) to damage caused by accident, abuse, misuse, liquid contact, fire, earthquake or other external cause; (e) to damage caused by operating the Apple Product outside Apple’s published guidelines; (f) to damage caused by service (including upgrades and expansions) performed by anyone who is not a representative of Apple or an Apple Authorized Service Provider (“AASP”); (g) to an Apple Product that has been modified to alter functionality or capability without the written permission of Apple; (h) to defects caused by normal wear and tear or otherwise due to the normal aging of the Apple Product, or if any serial number has been removed or defaced from the Apple Product.
And an excerpt from the Applecare for iPhone plan:
4.1 Hardware Service. Apple may restrict service to the Covered Equipment’s original Country of Purchase. The Plan does not apply to:
Installation, removal or disposal of the Covered Equipment, or provision of equipment while the Covered Equipment is being serviced;
(ii) Damage caused by (a) a product that is not the Covered Equipment, (b) accident, abuse, misuse, liquid contact, fire, earthquake or other external causes, (c) operating the Covered Equipment outside the permitted or intended uses described by the manufacturer, or (d) service (including upgrades and expansions) performed by anyone who is not a representative of Apple or an Apple Authorized Service Provider (“AASP”);
(iii) Covered Equipment with a serial number that has been altered, defaced or removed, or has been modified to alter its functionality or capability without the written permission of the manufacturer;
(iv) Covered Equipment that has been lost or stolen. This Plan only covers Covered Equipment that is returned to Apple in its entirety;
(v) Cosmetic damage to the Covered Equipment including but not limited to scratches, dents and broken plastic on ports;
(vi) Preventative maintenance on the Covered Equipment; or
(vii) Defects caused by normal wear and tear or otherwise due to normal aging of the product.
I'd take it in and see if they will cover the damage or not.
