According to Apple, if you have forgotten the code, you can unlock the iPod by doing the following:
Connect your iPod to the primary computer you use it with (the first one iPod synced with), and open iTunes. When you disconnect iPod from the computer, it will no longer be locked.
If you can't unlock iPod using either [the code or connecting it to the computer you use to manage its songs]. . . you can restore your iPod to factory settings using iPod Updater. Please note that this will erase all data on your iPod. Afterwards, you can add your music and data back to your iPod.
If you want to change the primary computer that your iPod is associated with, do this:
1. Restore your iPod. Warning: The restore process cannot be undone. All of your songs and files will be deleted. Always make a backup of your important data.
2. Connect your iPod to the computer that you want to be the primary computer.
3. Open iTunes and synchronize.
If you can't do that you can restore the ipod entering in thr DFU mode
All you have to do is:
1- Plug in tour ipod to your computer
2- turn off the ipod
3- press the power for 3 seconds
4- continue pressing the power button but press the home button at the same time for 10 seconds
5- release the power button bu continue pressing the home button for 15 seconds
And thats all
Itunes will detect an ipod in recovery mode

My ipod has a black screen. I have tried to hold the sleep/off button and menu button at the same time with no luck. I connected to my computer and tried restore on itunes - still with a black screen a voice said "screen locked' code 542

Forgotten Restrictions Passcode Help
You will need to restore your device as New to remove a Restrictions passcode. Go through the normal process to restore your device, but when you see the options to restore as New or from a backup, be sure to choose New.

What iPod model does she own? If it's a Nano, simply connect it to iTunes and restore it.
If it's an iPod touch, you will need to connect to iTunes in Recovery Mode and restore it using the instructions in this Apple support document.

I have tried rebooting it. nothing seems to help. it always ends up with the same screen a picture prompting me to hook up via usb to itunes.Then without fail i get an error message. itunes connpt connect to the ipod touch because it is locked with a passcode. you need to enter the passcode on the ipod touch first. unfortunately the touch will not show any other screen but the connect to itunes.

Try to get it to recognize it by using DFU Mode. Then restore.

my daughter has a brand new ipod touch - latest one - she started to input a passcode then didn't finish the process then turned off ipod. she turned it back on and it is asking for a passcode - she doesn't remember what she typed.
