Try:
- Reset the iOS device. Nothing will be lost
Reset iOS device: Hold down the On/Off button and the Home button at the same time for at
least ten seconds, until the Apple logo appears.
- Restore from backup. See:
iOS: How to back up
- Restore to factory settings/new iOS device.
If still problem, make an appointment at the Genius Bar of an Apple store since it appears you have a hardware problem.

For what it's worth, you posted this in 2011, and here in 2014 I am still having this same issue. Over the last two days, I have had to unlock my apple account 8 times. I didn't get any new devices. I haven't initiated a password reset. I didn't forget my password. I set up two factor authentication and have been able to do the unlocking with the key and using a code sent to one of my devices.
That all works.
It's this having to unlock my account every time I go to use any of my devices. And I have many: iMac, iPad, iPad2, iPad mini, iPhone 5s, iPod touch (daughter), and my old iPhone 4 being used as an ipod touch now. They are all synced, and all was working just fine.
I have initiated an incident with Apple (again) but I know they are just going to suggest I change my Apple ID. It's a simple one, and one that I am sure others think is theirs. I don't want to change it. I shouldn't have to. Apple should be able to tell me who is trying to use it, or at least from where.

I downloaded the OSX and now my iphoto will not open and has the spinning icon going around for the last TWO and a half hours!! i cannot access my photos or quit the program. help

I was able to turn it on but when I turned on WiFI the apps would not open and the phone crashed again and turned off.

The 2G iPod can only go as high as iOS 4.2.1. If you want apps that require an iOS higher than 4.2.1 you need a newer model iPod.

Because all apps are FOREVER tied to the Apple ID used to obtain them, & they can only be updated using that ID.

That depends on what you mean. If you mean two iTunes Store accounts, no. Only one Apple ID can be logged into the iTunes Store at a time and if automatic download, iTunes Match or re-downloading of content is used the ID can not be changed for 90 days.

If you previously had been using the same Apple ID for iTunes Store purchases for both of you there is no reason to change what you've been doing. If this is your wife's first iPhone, and you want to share the iTunes Store account just don't use the new Apple ID you created. As long as there are no purchases on it there's nothing to worry about.

Most likely a hardware failure, & if so, you won't be able to restore/activte your phone. If you've tried at least two times, & are still getting this error, make an appointment at any Apple store, or call AppleCare.

History menu, if it was destroyed in the last session
then use "Restore Previous session:" and then restore all windows also from the history menu.
If done in this session try the restore all windows under the history menu.

Songs have been wiped out 3 times from new ipod touch

Just bought 2 new ipod touches. One seems to be working fine. I had to restore the other because my songs were wiped out twice. It now has done it a third time. My son was listening to it and it suddenly stopped and the songs are no longer on there. Any suggestions?

That second iPod Touch goes back to the store for replacement.

When I try to update my ipod 3d gen to ios 5.11 and check for the update, itunes says it is on it's most recent software already. my ipod touch is running on ios 4.2.1.

I suspect you have a 2G iPod. Those can only go to iOS 4.2.1.
