My Macbook Pro keeps restarting itself with a Panic message each time!

Hi there,
I have had a few problems with my Macbook since I have bought it, namely the replacing of the hard drive twice. I am completely useless when it comes to tech, so my question may have already been answered but I might of just not understood the answers provided!
Was just on my Macbook and it decided to completely freeze, the Skype call I was in went silent and whilst I was still able to move the cursor, everything else was utterly frozen, so after a while I decided to just press the switch and try to restart it ! However, when I typed my password it, a black progress bar began to fill and at about 1/5th full, a bunch of lines with black backgrounds came up, and the computer shut itself back down to restart! The message says;

And that is what the message says, hopefully someone out there could point me towards a solution as every time it keeps coming up after I enter my password to log in, so I cant do anything! I have attempted to turn it on in safe mode but I believe it isn't responding to it! Any help is truly appreciated!

See Mac OS X- How to log a kernel panic. Post the entire log (not all of them, only the most current one) in your next reply. This assumes you can startup the computer. If you cannot startup the computer, then you will need to do this instead:
Reinstalling OS X Without Erasing the Drive
Boot to the Recovery HD: Restart the computer and after the chime press and hold down the COMMAND and R keys until the menu screen appears. Alternatively, restart the computer and after the chime press and hold down the OPTION key until the boot manager screen appears. Select the Recovery HD and click on the downward pointing arrow button.
Repair the Hard Drive and Permissions: Upon startup select Disk Utility from the main menu. Repair the Hard Drive and Permissions as follows.
When the recovery menu appears select Disk Utility and press the Continue button. After Disk Utility loads select the Macintosh HD entry from the the left side list.  Click on the First Aid tab, then click on the Repair Disk button. If Disk Utility reports any errors that have been fixed, then re-run Repair Disk until no errors are reported. If no errors are reported click on the Repair Permissions button. Wait until the operation completes, then quit Disk Utility and return to the main menu.
Reinstall OS X: Select Reinstall OS X and click on the Continue button.
Note: You will need an active Internet connection. I suggest using Ethernet if possible because it is three times faster than wireless.
