Set your apple pages to UK via the small flag in the bottom left hand corner.
This is Apple's guidance ... Though you last few words suggest you may already have tried this route.

Hi,
I have noticed that generating an invoice from a delivery that relates to an order that has been paid by credit card, seems to initiate a payment window, where you have to put the payment in to generate the invoice.
Is there any documentation available covering what is supposed to happen here?, or if not, would it be possible for someone to let me know what is supposed to happen and if anything else needs to be setup.

You must be talking about the flow in business one when fulfilling a webtools CC order, correct?
The easiest way is to get a certified third party credit card add-on for business one, this will do two things when creating a delivery.
1) It will change the "authorize" on the credit card gateway into a "charge"
2) It will create an invoice so that G/L accounts reflect the payment properly.
To do this manually, you will need the reference number of the autorization (this is located on the order) to look up the authorization on your credit card gateway and change it into a charge. And you will also need to record the payment in business one so that the accounts reflect the credit card payment.
At least that is my best understanding.

I am changing to a new laptop and as part of that action I am thinking to up from OS X 10.6 to 10.9. I have an Apple developer account since long time, also a working Apple ID. However I have never used the App Store (and frankly don't intend to do it).
So I opened the App Store for the first time. I have to sign in to download Mavericks. If I use my existing Apple ID, it says that the account needs to be associated with App Store, and then proceeds to query my credit card information.
I have no intention to transmit my credit card information to the Apple / the App Store. I have read various conflicting posts on the net saying that there is a way to specify "None" as a payment type (others claim that this option doesn't exist or require that I create a new account).
On my screen I can only choose between VISA, MasterCard, American Express, ClickAndBuy, and Maestro.
How can I, as of 19 Feb 2014, download and install OS X 10.9 without providing credit card information to Apple?

I spoke with Apple Tech Support and they guided me through this. I was trying to get the App Store working on a new iPad mini using a new, but existing Apple ID. To get it to work, I had to go onto iTunes on my main computer (not on the iPad). I had been trying to use the Sign In button, and I was having the issue of being "forced" to enter a credit card, which I did not want to do. Here was the solution I was given, which worked for me.
Log out of iTunes, if you are not already. (Or hit Cancel if you're on the Review Payment screen.)
Instead of going into Sign In, go to Redeem under the Quick Links, on the right-hand side, down a bit.
Go through the screens, accepting the EULA, etc. You will get to a Review Payment screen, similar to the one you saw before, but it will have a None button.
You can enter an iTunes card here if you want, but I was able to leave that field blank and still successfully get the account set up, ready to use on the iPad.

Unfortunately, this is a problem that is driven by the DRM Dictatorship. Despite the proliferation of mobile devices and the fact that there are many of us who do a lot of international travel, content providers don't want you to have access to their products outside of your homeland. The Balkanized mentality of the DRM Dictatorship is way out of touch with the modern world.
At least Apple, to its credit, allows you to use your accounts outside of your homeland as long as you have the proper credentials for them. Most other services use the more Draconian geolocation filtering which does require you to be physically present in your homeland. For the most part, you are not allowed to leave home if you want access to your favorite entertainment!

I live in italy, so my credit card address is italian, even if it is an international master card. I cannot download filsm in english, as I am based italian for my account. there is no way how I could change this so far.

Due to the demands of the content owner, you can purchase only from the iTunes Store in the country in which you reside, and as proof of that residence Apple can accept only credit cards issued in and billed to that country. You can call this "crazy" if you wish, but it's not by Apple's choice.
As to the language of films available, that again is by choice of the content owners. If they won't license movies with English audio tracks or English subtitles, there's nothing the iTunes Store can do.

Go to Apple Menu > App Store > View my account > sign in and then edit the payment details with the new card details.
I am assuming that your card and supporting bank are in the same country as your residential (billing) address and that you are using the App Store of that country.

The only people who can possibly assist you with this is Apple Customer Relations, call your local Apple contact number and ask for Customer Relations then explain your situation clearly and politely (be firm but don't rant).
You might want to investiage what the local laws are regarding defective goods and 'fit for use' definitions on warranties etc. Consumer Protection can be a useful tool to use or bargain with if needed ...

I'm in exactly the same situation -- I just got a big gift card on my iTunes account, and then American Express sent the 5 free songs offer, which expires in a few weeks. I found the following instructions from someone on another website. I haven't tried them myself but they look like they'll work.
According to Apple's page, it uses the following order for song purchases:
Song credits.
Gift certificate, prepaid card, or allowance credits.
Credit cards.
Thus, it would appear that you cannot charge a credit card without using up your credits or gift certificates first.However, similar to what @Nick said, you could just open up another account, add the credit card to it, and then gift the items you want to your original account.Update: Based on my own tests of self-gifting, these are my findings:
Gifts will deduct money from the credit card on your account; they will not use up song credits nor gift card credit. This means that you do not need to open another account, just add it as a credit card and gift it to yourself.
You cannot send a gift to the same email address as your iTunes account. This can easily be circumvented by sending it to one of your other email accounts.
The easiest method of gifting to yourself is to select the print option. It will show you a code which you then use to redeem the song. It will allow you to redeem the code on thesame account.
