No pages in the Index

Question

Just downloaded AMA Guides Causation 2E E-book.  The Index has no page numbers.  How do I find the indexed item in the text?

Been online for about a year. Will check to see if Googlebot
has visited
in the past. Will add a robots.txt file if that helps too.
Will take a look at how to create the text file sitemap. Saw
the
generator but that';s for Windows.
