If you have restarted (turned completely off, and back on), and restored to factory settings, and the problem still exists, it means you likely have a hardware error.  If you are still covered by the limited warranty (1 year), or if you purchased the Apple Care plan (2 years from date of purchase), it should be covered assuming no accidental damage.  If your warranty is expired, or there is accidential damage, the next option is an out of warranty repair.  $199.00 USD for an iPhone 4s (I believe).
