Please help! How to manage "edit your details" form for customers

Question

Please help!
I don't know what exactly is happening but after clicking Submit, appears a windows saying: "Form submission is in progress"
But I don't know what changes and where goes this "form".

your javascript looks like its broken
Uncaught TypeError: Cannot set property 'value' of null line 425
These two elements in your submission javascript don't exisit on the form Im guessing thats the problem.
