PO: Error number 3 - Failed Activity: Does Approver Have Authority?

Hi,
I know, this is old known error, but I cannot resolve the problem. Many solution I have read a applied, but some workflows still shows: error number 3.
1) user was notified by this:
Document Manager Failed With Error Number while processing Standard purchase order 50025512
2) error shows this error:
Workflow Errors: XXK_POAP, 116384-269277, 50025512
Failed Activity XXKDoes Approver Have Authority?
Activity Type Function
Error Name 1
Error Message User-Defined Exception
Error Stack Wf_Engine_Util.Function_Call(XXK_PO_WFS3Z.XXK_VERIFY_AUTHORITY, XXK_POAP, 116384-269277, 204044, RUN)
Yes, there are customizations.
3) for errored workflows the debug table shows this:
for example:
FUNCTION = fnd_trn_pipe.get_manager
ACTION = 1|No manager defined
RESPONSIBILITY_ID = 20420
RESP_APPL_ID = 0
eBS 11.5.10.2 runs on Win 2003 server.

The error that appears is a generic error.
But the activity that is erroring out is "XXKDoes Approver Have Authority" is an customized one..
Can you ask your technical engineer to reivew the events associated with the activity, to troubleshoot further.
If you want to confirm, whether the issue is arising out of oracle code - you can forceload the workflow and then retest the issue.
If it still errors - you can ask "oracle Support" to assist.
