Quicktime crashing all over the place

This happens in both firefox and IE. In some twisted fashion, if I try to load a video in Firefox and it buffers (say 20%) and it crashes, if I try to load it in IE, it will pop up at the exact same spot, like they're sharing a cache. If I clear the cache, and try to load the trailer again, same thing. Its like it was never cleared. This happens a good 80% of the time.
I've reinstalled quicktime twice, (ran a registry cleaner before the last one) no difference.
Event viewer gives me this:
Faulting application iexplore.exe, version 7.0.6000.16473, faulting module quicktimeh264.qtx, version 7.1.6.200, fault address 0x0005145a.
Also get: Fault bucket 446840317.

Green and purple stuff ... Do you still get the same effect if you try the "Disable Direct3D acceleration" section from the following document? (iTunes uses QuickTime for video playback, so the same basic troubleshooting procedures apply.)
