Quires regarding re-connecting with AT&T after an year.

Hi,
I was At&t post-paid customer from 2011 till 2014. In 2014 I travelled back to my home country and since I was not in USA, I cancelled the service. The number which held with At&t during that time was ****.
Now I am planning to come back to USA in another 3-4 weeks for long term stay. I do have SNN and an excellent credit history. I have below questions regarding getting a new post-paid connection with At&t.
1. Event though I cancelled my previous service, is it possible to retain the old post-paid number which I had i.e. [edited for privacy-please do not post personal or unique information such as but not limited to full names, employee ID numbers, email addresses, phone numbers, account numbers, etc.]?
2. What all documents are required for getting a new post-paid connection?
3. Are there any specific constrains for getting iPhone 6 or 6+?
Kindly please clarify.

1. Chances of getting your old phone number back is almost impossible. It may have already been reassigned to someone else.
2. Doucment's proving who you are should be all that is needed. If you have the paperwork from your old account, it may help prevent a deposit on your account.
3. No constraints on getting the iPhone 6 or 6+ as long as you can pay for it.
