SATA disk missing after reboot - Solaris 11 express

Using solaris 11 express, after full boot one SATA disk is unknown in cfgadm:
sata0/0::dsk/c8t0d0     disk     connected     configured     ok
sata0/1::dsk/c8t1d0     disk     connected     configured     ok
sata0/2::dsk/c8t2d0     disk     connected     configured     ok
sata0/3::dsk/c8t3d0     disk     connected     configured     ok
sata0/4     unkown     connected     unconfigured     unkown
sata0/5::dsk/c8t5d0     disk     connected     configured     ok
I was able to get it configured sing these two commands:
cfgadm -x sata_port_activate sata0/4
cfgadm -c configure sata0/4
The disk seems to work find now but after a reboot it loses the disk again and is unkown. How can I get the system to "remember" the disk and configure at boot automatically?

    Replying to this question to answer it with a working solution. I'm not sure what the other replies marked as answers to this post are trying to answer, but they are completely unrelated to this topic. It appears they are describing a fix for a server acting
    on the initiator side connecting to a target, not a server acting as an iSCSI target hosting LUNs. Very frustrating to say the least.
    I ran into this problem over a weekend maintenance project where I rebooted a Server 2012 R2 File Server to enable some updates. When I logged back in, the iSCSI section was blank and my two existing iSCSI virtual disks had vanish. Now with that said the
    underlying file system that the VHDX files were located on was intact. I tried to import them and got the same error that the path was already in use. 
    I first tried the fix mentioned lower in this thread where you move the VHDX file into another folder to change the path. That worked, but I wanted to figure out exactly why it broke and get my old file to load as it should. Now I don't know the answer to
    "why" but I found a way to clear the existing information stored that is preventing you from re-mounting the existing iSCSI files without changing the path.
    From this page, there are PowerShell scripts outlined to change iSCSI parameters from the command line.

    This command in particular deletes the record of unassigned virtual disks that aren't assigned to a target:
    $AllDisks | where {$AssignedDisks -notcontains $_} | 
    foreach {Remove-IscsiVirtualDisk $_ }
    After running that command that broken link that had existed prior was removed and I was able to re-mount my iSCSI virtual disks without that error coming up.
    As always, make sure you have a backup before you try any odd powershell commands. That one worked for me.

    I am having a nearly identical problem as listed in the "RAID 5 missing after reboot; RA Utilities non effective" thread, with one or two small differences.
    We shut down our Xserve RAID normally on friday, started it back up on Monday to find that one of the arrays was no longer there. RAID 1 is still working perfectly, but RAID 2 now has all the drives listed as spare. Tried all the tools in RAID Admin, but nothing has worked. At the advice of Apple tech support i updated the firmware to 1.5, swapped out controllers and cables, and tried using the Recognize RAID command in utilities. Nothing has worked. Apple recomended bring the drives to Drive Savers to get the data recovered. Does anyone have any other ideas?
    Oh, also this raid was reporting a failed drive on friday before it was shut off, when powered back on the drive now claims to be fine.

    Guess I got lucky.... After a reboot of the Xserve RAID, RAID #2 mounted again. Did my backups, shut it back down, and updated firmware to 1.5 with no problems. All seems good now.
    Still wondering what happened.
    It appears that drive #8 failed, and after the rebuild with new replacement drive, it seems as though the controller needed to be restarted too. The odd thing is, before the restart, #2 was acting like it had three new drives and no array configured at all. After the reboot, there is the RAID 5 array, no rebuild or anything - ready to go.
    So here is my question:
    How often does everyone reboot their Xserve RAID? should it be done periodically? Every 3 months? Every month?
    Or is this just a hiccup with the parity rebuild, and nothing else to worry about, or even a firmware issue for the controller(s) that 1.5 may resolve?

  • RAID 5 missing after reboot; RA Utilities non effective

    I had 7 drives formated in RAID 5, after reboot, on my Xserve I can not mount RAID.
    If I check with RAID admin, it seems everything to be o.k, but one event is strange:
    RAID Controller 1 Fibre LIP; A Fibre Channel Loop Initialization Primitive (LIP) was detected. This often occurs when a new device is connected to the Fibre Channel network.
    I did not connect any new devices... Same is with controller 2.
    Every one of drives is described as "spare" but they were in RAID 5 all of 7 drives. If I check the arrays tab, no arrays are present.
    If I try to Verify the array the "verify array" button is dimmed so I can not proceed.
    Same is with Rebuild parity data or recognize array on both controlers.
    Is there any possibility that I can establish RAID with all data?
    I did not found any terminal commands for that...
    Any idea? Any hint?

    I was in the same situation few months ago. The same problems you reporting there, except of that, that my array dissapeared after successfull firmware update (and I had RAID3 on 6 disks). After unsuccessful technical support here I managed two trips of my disks to Apple engeneer in USA (I am from Czech Rep.) with no success. For the first time, they gave me back RAID 3 on 5 disks (not on 6 as I had before) and after second trip, thay gave me back 5 empty disks and the 6th one was damaged. So, my opinion as a man from Central Europe is, that we have no chance to have our datas back!
    Problem was, that first 64 kB were zeroed (metadata with RAID schema are placed there). They tried (in Apple) to restore these metadatas on disks back, but as I said before, thay probably had a bad days.
    Only one thing you can try is to find someone with XRAID at your place and try to put your disks there. May be your array will appear there, so you will be able to copy your data to some other disk. But if the first 64 kB on disks are zeroed ....
    Anyway, please let me know if you solve your problem. I still have my disks untouched waiting for some miracle...

  • Cookies missing after reboot

    I'm running windows XP and lately all cookies were deleted every time I reboot my computer. All cookies, and default settings are gone after reboot, I even switched back to IE8 and same issue noted on both IE8 and Firefox 3.6.12

    Again and again!!!I reformatted my usb drive and created a brand new safepoint.
    everyting was ok but two days after, the system has rebooted (no warning and I don't know why) and my Safepoints are missing in the dashboard.
    Obviosly in the WDMyCloud desktop app the SP are visible... Frustating! :-(

  • Hard disk problem after installing Solaris 10 u5 on a Sun Ultra 24

    Hi.
    After receiving a new Sun Ultra 24, shipped with Solaris 10 u4, the first thing I did was installing Solaris 10 u5. X wouldn't start but that was not a big problem and installed it in text mode. Installation went fine but, when the system starts, a single beep is heard and after some seconds some warning message appear on the console, before the "Hostname: ..." message, informing that some read operation on my hard disk had timed out and were aborted. The system then starts up normally. The only other anomaly is that the diagnostic partition in unbootable.
    I ran the hard disk tests from the tools and drivers CDs and all of the tests were passed. Solaris 10 u4 was working normally so I'm wondering if this may be a bug.

  • Safepoint missing after reboot

    Hello,after every reboot (balckout or similar) the MyCloud doesn't find anymore the previous Safepoint.
    The WD usb drive attached directly to the MyCloud is properly "seen" in the dashboard (USB icon on the top right and listed on the Shares) but in the Safepoint tab there is anything. 
    I can only create a brand new one on the same USB drive. With the MyCloud application, I can see that the Safepoint still exists in the share folder created for the SF. If I need to recreate SF, it's not so sure!!! I have tried to unmount the USB drive and remount but without success... Thanks

  • OSX 10.5.6 Arbitrarily reasigns logical disk id after reboot

    Mac Pro 2007 w/ 3 disk drives.
    Since I have been using this Mac Pro, OSX has always assigned logical disk ids according to their physical locations. Bay 1 <-> disk0, Bay 2 <-> disk1, Bay 3 <-> disk2, etc.
    Now after upgrading to 10.5.6, OSX arbitrarily assigns disk ids and seemingly random orders after each shutdown/reboot.
    This behavior has particular affect on VMware Fusion which uses logical disk names for virtual machines on an entire drive. e.g. /dev/disk2

    I've just noticed this behavior myself on my Mac Pro. No answer for you, but maybe replying to the thread will bump it up to attention of others who are more knowledgeable.
    My drives used to get assigned sequential order from Bay 1 to Bay 4, assigned disk ids based on that order, and would appear in About This Mac in that order, now they're just random.

  • Time machine all recent backups missing after reboot

    I accidentally deleted an important file on my mac, and when i opened time machine, i discovered that my it couldn't be found wirelessly. The green light was still on the front of the device but my mac couldn't locate it.
    I'm not sure exactly how long it has been since timecapsule made a backup, but i have a clear memory of it working about 3 months ago.
    I rebooted it and now its running, and has backed up.
    My problem is when i go into the "star wars screen" to view past backups, the only backups are from today and then next most recent are from July 2012 when i first bought the time capsule, everything else in between has disappeared.  It definitely had 2013 backups.
    Are the backups from 2013 still there and can't be seen, or have they been deleted permanently?

    I presume then you upgraded to Mavericks?
    It has a nasty habit of doing this.
    If not on Mavericks please tell me what OS you are on.
    Did you search by Finder.. after mounting the sparsebundle.. ie do not use Time Machine directly.
    I know this is silly question .. but if you deleted the file is it in Trash??
    If you deleted from Trash an immediate recovery is possible at times.. you will need to purchase a recovery software unless your are good with the CLI.

    If it is deleted weeks ago then recovery is unlikely as the area of the drive will have been written over.

  • Attached disk missing after VM upgrade

    Tonight I upgraded my Linux VM from A6 to D13. I then restarted the machine. Afterwards the attached disk (which happened to hold my entire web directory and document storage folders) was no longer attached.
    Unfortunately I'm not certain of the name of the vhd - so it may or may not still be visible from the management dashboard. I do know for sure that the disk was 285gb. I used CloudXplorer to resize that disk from 127gb to 285gb and then starting using it
    for my webroot. There are no longer any 285gb vhds present in my account.
    As far as I can figure, manually resizing the disk with CloudXplorer and then upgrading to D13 either deleted my disk entirely or reset the size to 127gb.
    The is a big big issue for me. I'm hoping someone may have some ideas to pint me in the right direction. Is there a way to recover this disk if it got removed? Should I resize it back to 285gb with CloudXplorer and then re-attach it?
    Any help would be very much appreciated.

    Hi Prasant. Thanks for the reply. Yes it is a Linux VM. I opened a ticket over the weekend, and after some research we determined that the disk I had all my data on was actually the sdb temporary disk. When I upgraded my VM it was switched to a different
    node and the sdb disk was wiped. I was told that the data was un-recoverable. If there is actually any slight chance of recovering it, please let me know. We lost a lot of critical data, and it would be a lifesaver to have it back.

  • Address Book missing after reboot

    I done a complete reboot sometime ago of my iMac with transferring all of my archived files back to the computer.
    Some reason my Address Book didn't transferred. Not sure if I've lost it complete through the archive process or if I'm not able to transfer it properly.
    Anyone experienced this before? Any advise will be helpful. I've got so many contacts that I don't want to lose.

    Did you use the Archive and Install method with the option to preserve User and internet settings? Or did you do a Clean and Install which would lose all of your personal files.
    TIP: For insurance against the iPhoto database corruption that many users have experienced I recommend making a backup copy of the Library6.iPhoto (iPhoto.Library for iPhoto 5 and earlier versions) database file and keep it current. If problems crop up where iPhoto suddenly can't see any photos or thinks there are no photos in the library, replacing the working Library6.iPhoto file with the backup will often get the library back. By keeping it current I mean backup after each import and/or any serious editing or work on books, slideshows, calendars, cards, etc. That insures that if a problem pops up and you do need to replace the database file, you'll retain all those efforts. It doesn't take long to make the backup and it's good insurance.
    I've created an Automator workflow application (requires Tiger or later), iPhoto dB File Backup, that will copy the selected Library6.iPhoto file from your iPhoto Library folder to the Pictures folder, replacing any previous version of it. There are versions that are compatible with iPhoto 5, 6, 7 and 8 libraries and Tiger and Leopard. Just put the application in the Dock and click on it whenever you want to backup the dB file. iPhoto does not have to be closed to run the application, just idle. You can download it at Toad's Cellar. Be sure to read the Read Me pdf file.
    NOTE: iPhoto 8's new option in the library rebuild pane, "Rebuild the iPhoto Library Database from automatic backup" may make this tip obsolete. We'll know when users have occasion to use it and see if that's the case.

  • Open Message Windows Missing After Reboot

    I have experienced this issue after Apple Mail has to reboot for a variety of reasons, i.e. quit and reopen, reboot, basically any time Mail has to reopen.  When it does, often many of the open message windows which I have open at the time are not reopened upon reopening Mail.  It seems to occur whenever there is something which "interrupts" the reopenning process, as when I attempt to open another program, Safari for example, before Mail has fully opened.
    While I do not want to keep too many message windows open at the same time, keeping the ones which I do have open works for me as an organizational tool, as in I need to address the open windows.  But, if they do not reopen the next time, they get lost in my overly massive inbox. 
    Anyone have a solution, other than completely reworking my use of open message windows as an organizational tool?

    Anyway: To avoid the procmon thing, just think about "what" could delete
    a service?
    Group Policy Preferences comes to mind first, then computer startup
    scripts and scheduled tasks. Grab a RSoP report on the affected server
    and check what's applied at all.

  • Soundcard suddenly missing after reboot.

    Last night I installed and Arch and got pulseaudio configured with ALSA and everything was working perfectly. However, upon starting my computer today one of my sound cardsappears to have disappeared.
    When I run aplay- l only card 0 appears. Card 0 is only the HDA Intel HDMI outputs. Previously Intel PCH was also visible.
    Any ideas on how to bring back the second card?

  • Dynamic online disk array without (reboot -- -r) on Solaris 10.

    I am seeking a way to online a new disk array after I have added the array into the fabric (FC switch) with Solaris OS 10 without reboot and reprobe HW command.
    This used to work for Solaris OS 9 or lower using command "devfsadm" or "devfsadm -c disk" (class specific).
    Leadville command "cfgadm -al" did not see the new path as I was hoping for with tag of "unconfigure" (so that I can configure it in).
    Any suggestion on how can I online the disk array using certain OS (Solaris 10) command would be very much appriciated.

    Interesting info as I tried with SPARC base runing Solaris 10 OS. The daemon is auto imported the unit without I have to issue the OS scan device "devfsadm".
