SCOM 2012 RTM CU3 upgrade error

I tried to install CU3 on a 2012 RTM server and after installation the Health Service will not start. It was installed by Windows Update as I figured Microsoft would not push it there unless tested. 
I get the following errors in the OpsMgr Log: 
Event ID: 20056: Failed to initialize the IPC channels for other processes to communicate. The HealthService might fail to start
Event ID:2000: The Management Group MG failed to start.  The error message is 0x80131501(0x80131501).  A previous message with more detail may have been logged.
Data Access Service and Configuration Service are running fine. Anyone else seen this or have an idea on how to solve it?

To resolve this issue use either of the following methods:
1. Use Gacutil /u with the dllname that was added then restart the computer. Gacutil is available from http://msdn.microsoft.com/en-us/library/ex0ss12c(VS.80).aspx
or
2. Navigate to C:\windows\assembly and look for the dll affected, right click on this DLL, and select Uninstall.
Once you do either of these, the management group should start and the events should no longer be logged.

  • SCOM 2012 R2 agent upgrade fails crippling agents

    Running into a large amount of SCOM agents that are failing the upgrade from 2012 SP1 to R2 and would appreciate any feedback from my SCOM community colleagues.  Warning this issue is not for the faint of heart. 
    Plan:
    I am in the process of upgrading 1900 manually installed SCOM 2012 SP1 agents to R2.  I am using SCCM to deliver the upgrade using the standard sanctioned upgrade parameters. 
    Momagent.msi /qn /l*v %SystemDrive%\SCOM2012AgentUpgrade.log AcceptEndUserLicenseAgreement=1
    Problem:
    I have run into a problem where on a larger group of systems, 165 servers, where the upgrade fails and leaves the agent in a crippled state.  At this point the agent cannot be removed cleanly via add remove, nor can a straight install of the agent
    be done.  What is required is a manual removal of registry keys and then a clean install of the agent can be performed to remediate.  This is no problem and I am able to do this.  (Note: all other methods of agent removal did not work ie:
    Cleanmom.exe utility or add remove)
    Task:
    Seeing that we need to do agent upgrades in the future it would be great to know why this happened so we can plan for this in the future.
    Notes:
    Seems like a random sample of servers (2003/2008) with different applications running on them so that doesn't help in narrowing things down. 
    Looking at the MSI log, I see a common issue among systems that had this problem.

    Ok so the obvious is that the installer is looking for original source installation files and not able to find them. What is surprising to me however is that the product references a 32 bit scom agent guid
    387306D9-78CE-4E0E-B952-28A50CC8B3EE, however this is a 64 bit machine.  Our build process dictates that a 64 bit machine only receive a 64 bit SCOM agent. 
    Doing a search on this product guid I realized I skipped some other references at the top of the MSI log that might offer some more explanation...

    Well that is interesting, seems to me that in the first line the MSI installer reads the OM_OM12_SP1_AGENT_FOUND property and identifies this with a  64 bit agent guid (8B21425D-02F3-4B80-88CE-8F79B320D330), but then appends
    the 32 bit guid at the end (387306D9-78CE-4E0E-B952-28A50CC8B3EE).  This is the point of my confusion and my suspicion as the cause of the problem I am having. 
    Concluding thoughts:
    Why does the installer seems to first recognizes a 64 bit agent, but then later changes it's property to include a 32 bit agent guid.  Could a 32 bit agent have got on this 64 bit server based on the details of this log?  Would this
    cause my agents to fail the upgrade?  I think so, but looking to bounce this over to another fellow SCOM colleague who may have wrestled with this before.

  • SCCM 2012 R2 CU3 UPGRADE BROKE REMOTE ADMIN CONSOLE CONNECTIVITY

    I installed SCCM 2012 R2 CU3 on my site server but it broke a couple things. I've never had problems with SCCM upgrades before. The admin console works if you log into the site server and run it from there but not when you try it from anywhere else (another
    server or a workstation). When you open the admin console on the site server it does have the new version of 5.00.7958.1401. Of the five suggestions that appear when your console won't connect, here's what I know:
    1) the remote machines can all ping the site server (call it server 01)
    2) we know it's not my account that's a problem as I can open the console on the site server itself
    3) This problem happens on server 02, which I put the new admin console on. It also happens with other machines which still have the CU2 console
    4) Yes, I'm an admin role, shown by logging into the console on the site server
    5) I have another network where everything works. I have not put CU3 on it yet, of course. The WMI permissions on Root, Root\SMS, and Root\SMS\site_ABC are identical on both site servers yet it works on non-upgraded one and not on the upgraded one.
    WMI info-I can WMI connect from the site server to these other machines but not WMI connect from the other machines to the site server: not root, SMS, or SMS\site_ABC, nor cimv2, even though the permissions match what is on the working network.
    When I launch the console from the site server entries about loading the console loading ok are in SMSAdminUI.log, but when I try to launch the console from other machines no entries are made, even though I changed the SmsAdminSNapIN value to VERBOSE.
    I checked DCOM permissions and they are the same on both networks.
    Another issue that appeared with this upgrade: I can install the client upgrade to CU3 on other machines but on the site server it fails with "Internal Error 2503" and when you click OK it you get "Internal Error 2502".

    I'd suspected this WMI problem was only affecting our Server 2012 boxes. I compared RSOPs and noticed the ones having this problem were all under the same OU and getting few GPOs. Our GPO person found a GPO that has somehow become unlinked.  He relinked
    it. After a gpo update AND a reboot these machines start working again.  So CU3 did not cause this WMI after all.
    And yes, the CU2 consoles won't connect to a CU3 site server, you have to run the console upgrade.
    I fixed the "internal error 2503/2502" by doing this: open task manager, click details, end the explorer task (screen will look blank at first). Click File and select "start new task", enter "explorer.exe' and click the run as admin
    button. The msp for CU3 client installed and the screen looks normal again. I only saw this error on the site server and have never seen it before.

  • Thin Client reporting issues since SCCM 2012 R2 CU3 upgrade.

    Please forgive me, I'm a SCCM newbie. I'm having an issue with my Thin Clients reporting back to SCCM since I upgraded to 2012 R2 CU3.
    The only pertinent thing I find in any log is "RegTask: Failed to get registration state. Error: 0x8009000b" "GetRegistrationState failed (0x8009000b)" in the ClientIDManagerStartup.log
    If anybody has any tips, I would be more than appreciative.

    I have not run into this before, but have you validated that your management point is healthy?

    Could you please verify if the Web Service Activation (WAS) feature is installed or not? If not, could you please add the “Application Server” Role and adding the activation settings “HTTP Activation” and “TCP Activation”.

  • SCOM 2012 R2 Agent Issue: Error 25211.Failed to install performance counters.. Error Code: -2147024809

    Hi All,
    I'm trying to deploy SCOM 2012 R2 agent onto a domain controller and I get the following error "Product: Microsoft Monitoring Agent -- Error 25211.Failed to install performance counters.. Error Code: -2147024809 (The parameter is incorrect.)."
    I've installed the agent successfully onto 4 other domain controllers with out any issues.</p><p>The domain controllers are all VM's running on VMWare and are Windows Server 2012 R2.
    I've rebuilt the Perfmon Counters based on this article:&nbsp;https://support.microsoft.com/en-us/kb/2554336?a=wsignin1.0
    I've also enabled verbose logging on the msi installation:

    I've successfully installed the agent. The server needed a reboot after fixing the corrupt perfmon counters.
    I know have a issue with the agent on the domain controller. It kkeeps on greying out and have used hslockdown to allow the local system access by using the following command.
    HSLockdown.exe "ManagementGroupName" /A "NT AUTHORITY\Authenticated Users"
    Further digging into the issue I see in the SCOM Management Server the following error "The entity servername is not heartbeating"
    Written a SQL query to gather more information. SQL query I used is:

  • SCOM 2012 R2 Gateway installation error and no System Center Management server after install

    I have installed SCOM 2012 R2 Gateway and I got an error 25372 error at the start of the install. It still installs though. However I have no system center management service running in services but I can see healthservice.exe is running.
    Why am I not seeing the system center management service?

    Using gateways with certificates is always a bit complicated because there are several things that needs to be configured correctly.
    DNS: The MS and the GW server need to be able to resolve each others FQDN. you can adjust the hosts files if needed.
    Traffic is only TCP 5723 from the gateway to the MS. You can test this with the telnet client.

    The gateway server should NOT be in pending management. Remove it from here before running the approval.
    You should start by making sure DNS and the 5723 port are functioning because that is probably where the problem is.

  • External Web Console Access SCOM 2012 R2 - An unexpected error has occured.

    I have an issue when accessing the Web Console for our SCOM 2012 R2 environment - I have installed the web console on a standalone machine with mixed authentication - i have windows authentication enabled and NTLM is at the top of the providers list. I have
    gone through the process of registering the SPN's and configuring constraint delegation. The SSL settings are ticked on the Default Web Site, the Monitoring View and the Operations Manager.

    I can verify that this SCOM Web Console issue (System.ServiceModel.CommunicationException: [HttpWebRequest_WebException_RemoteServer]Arguments:
    NotFound) was resolved in multiple instances simply by dropping and re-adding the IIS bindings for 80 & 443 (using Default Web Site for SCOM console) and restarting IIS.

  • SCOM 2012 R2 UR4 upgrade question - where exactly is the UNIX/Linux agent?

    I am practicing the UR4 on a test |SCOM environment (my first upgrade) and our UNIX Admins have requested the "UNIX/Linux agent" so they can create packages to upgrade the UNIX/Linux agents when the time comes. 
    I have downloaded all the UR4 updates/software. Where exactly is the agent for UNIX/Linux machines?
    I opened the "Update Rollup 4 for Microsoft System Center 2012 R2 - Operations Manager Agent (KB2992020)" folder and I saw the following agents:
    amd64-agent.msp
    i386-agent.msp
    Are one of these the UNIX/Linux agents?
    I know I'm overlooking something here since this is my first time. Any assistance is greatly appreciated.

    Install the management pack update package to extract the management pack files.

  • SCOM 2012 R2 Post upgrade issues

    Hey all,
    yesterday we upgraded our SCOM environment from 2012 SP1 to R2.  All of the installers ran fine without any issues.  After a couple reboots it was back up however we have run into some issues.
    Issue 1:  The ACS seems to be broken.  I can usually run a query on the ACS database manually and get back audit entries.  However, now I am getting nothing back at all.  Here are the errors that are repeated every few minutes on both
    management servers.  I did run the ACS upgrade on them both yesterday.
    Error occured on database connection:
     Status: 0x00040000
     ODBC Error: 14
     ODBC State: 01000
     Message: [Microsoft][ODBC SQL Server Driver][DBNETLIB]ConnectionOpen (ParseConnectParams()).
     Database: Register
     Connection: ComplianceTest

     ODBC State: 08001

    Issue 2:  Our UNIX agents are all greyed out now and when I try to upgrade them I get the following error.

    I am going to update this post.  The service is still crashing nightly.  The views dvAll, dvAll5 and dvHeader are not being created.  I tried the fix provided by the Microsoft rep and it did nothing.  I have uninstalled and reinstalled
    the ACS server multiple times and the views are never created.  When I start the ACS services I get the same errors.
    AdtServer encountered the following problem during startup:
     Task: Load Certificate
     Failure: Certificate for SSL based authentication could not be found. SSL authentication will be disabled
     Error: 0x00000002
     Error Message:
    The system cannot find the file specified.
    Error occured on database connection:
     Status: 0x00040000
     ODBC Error: 4624
     ODBC State: 01000
     Message: [Microsoft][ODBC SQL Server Driver][SQL Server]Cannot grant, deny, or revoke permissions to sa, dbo, entity owner, information_schema, sys, or yourself.

    I see where the forwarders are connecting.  No other errors or messages.  The service is set to start up as the Network Services account.  The DOMAIN\SERVER$ has a logon and is mapped to the SQL user AdtServer on the OperationsManagerAC database.
     We have almost a terabyte of data that we cannot get to after the R2 upgrade.

    I have seen this behavior on multiple servers. I receive this error when upgrading from a 2007 Agent to SCOM 2012.

    Event ID: 1008
    The Open Procedure for service "HealthService" in DLL "C:\Program Files\System Center Operations Manager 2007\HealthServicePerformance.dll" failed. Performance data for this service will not be available. The Status code returned is the
    first DWORD in the attached data.
    Any idea why I receive this error?

  • Issue upgrading SCOM 2012 SP1 to SCOM 2012 R2

    I have been having a really fun time trying to get my test SCOM 2012 SP1 environment upgraded to SCOM 2012 R2.  I initially had an issue with the DB server not accepting connections to SMB which was causing the upgrade to fail stating database permission
    issues (that was a fun one to troubleshoot).  Once I figured out that problem I am now getting an error in the OpsMgrSetupWizard log that is stating the following:

    :LaunchMsiSetup: Failed, return code: 1612
    [14:46:47]: Error:

    In the application log I am getting the following error:
    "Product: System Center Operations Manager 2012 Server -- The Operations Manager management server cannot be installed on a computer on which the Operations Manager agent, Operations Manager gateway, System Center Essentials, or System Center Service
    Manager is already installed. These must be uninstalled to proceed."
    I have verified that I don't have any of these installed and have even gone so far as to remove my Config man agent and System Center Endpoint Protection just in case it was getting confused with System Center Essentials.
    I am at a loss as to what to try next.  Any suggestions?

  • SCOM 2012 Ver 7.0.8560.0 + SP1 Upgrade

    I am running SCOM 2012 RTM 7.0.8560.0 and would like to install SP1. 
    My environment:
    1 VM - MS, OC
    1 VM - MS, OC
    1 VM - MS, WC, OC
    1 Ph - Reporting server and DB
    1 Ph SQL Cluster - OM DB and OM DW DB
    I have the following questions if someone could please help out.
    1. I read that you need to install Rollup 3 before installing SP1. Is this true or can i just go straight to Sp1. We have had this version for atleast 5 months running.
    2. What server order should SP1 be upgraded. MS 1 then 2 then 3  then Reporting? I dont need to install it on the cluster SQL Server where OM DB and OM DW DB are right?
    I appreciate any help as Google is giving me multiple answers to the questions above.

  • Dashboards Problem after SCOM 2012 Inplace Upgrade- Missing Stored Procedures

    Dears,<o:p></o:p>
    After finishing SCOM 2012 in place Upgrade I face a problem on Performance and Service level dashboards, the Upgrade has been finished successfully as shown below
    But after I start Dashboards configuration I receive below errors
    After troubleshooting I see an error in Event viewer beside the errors on Snapshots above said that
    Fault Detail is equal to could not find stored procedure
    so I compared Stored Procedures under SCOM data warehouse database with another SCOM 2012 installation (different customer) and i see a lot of Missing Stored Procedures
    (all Stored Procedures  start with 'SDK.Microsoft_SystemCenter_Visualization are missing)
    Here i am stuck, I don’t know what I can do, I have some ideas but i don't know if its will work.<o:p></o:p>
    My ideas are:<o:p></o:p>
    1. Repair SCOM 2012 Installation (does the repair option will repair the databases)
    2. Re import Default SCOM Management packs like (Microsoft.SystemCenter.Visualization.Library.mpb) (does MP contains Stored procedures in the Database)<o:p></o:p>
    3. Install Rollup Update (may during installation it will create missing Stored Procedures)
    4. Move Stored Procedures from other SCOM server installation
    Please advise its strange issue
