Service-based login screen

Question

I created a custom service which looks like:

I was able to view the service in AMAdmin's 'Service Configuration' tab and was able to add this custom service to my default organization. I've also put the JSP and XML files in <WEB-APP>/service/config/default_en/html/PMSService.
However I can't use <URL>/amserver/UI/Login?service=PMSService to access my custom service's login screen. Could someone please give me some hints? Thank you.

Log into amconsole as amadmin.
at the top level org, select services
register new service
select authentication configuration
create a new template
in the service instance list, click new
fill in instance name (i.e if instance name is myservice, the service=myservice parameter in the login URL will redirect to this config) and success url
After saving, edit it and fill in the authentication configuration (module chaining)
