Several Issues including kernal panics, can't reinstall OS X, icons gone
I've read what seems like a hundred threads on this forum the last 3 days with regard to similar issues to mine but not finding any help that seems to work with my situation, I'm posting this all in a new thread and welcome any suggestions/feedback.
I'm not new to Mac, but it's been quite a few years since I had one and as such many things have changed. Overall, I've been very happy with my Week 43 MacBook Pro. Everything was running like a top - until just after Christmas.
Then, my Adobe Illustrator program began "unexpectedly quitting." At first, there was no other problems with any other programs. Then, about 5 days ago, it started happening with ALL programs. And usually within just seconds of the program starting.
I've tried deleting the preferences files, .plist files, and other such recommended steps all to no avail.
Next, about 3 days ago, I started getting kernal panics, randomly. So far, there's probably been about 40 altogether.
I have also tried reinstalling OS X but it tells me there's an error and can't complete. I've run Disk Utility from the OS install disc and it repairs preferences but when I try to repair disk, it says it there are errors on the disk and quits.
I did try starting in Safe Mode, but the computer will not even start up. It quits immediately after the apple and the spinning gear start. I can start up regularly, though.
Another issue I am now having which just started yesterday, is that folder icons are all missing. The words show up and program icons, machd icon and a few others are there, but no folder icons.
I have had this a little over a month and no issues until right about the 30 day mark (I purchased it Thanksgiving weekend).
I have an appointment later today at the Genius bar but would like any suggestions you all might have in the meantime.

From your description you're simply having disk errors. At this point I would try DiskWarrior 4 to repair your drive and go from there. You have a couple of options here…
1) Go out and buy DW 4 and try it yourself. It's a good purchase anyway.
2) Wait for your appointment with the Genius and most likely he'll use DW 4.
Either way I would suggest you don't use your MBP in the meantime. Often with disk errors your problems get worse the longer you keep it in operation. While it might not be the case for you there's no reason to be increasing your risk of data loss.

Reset the SMC exactly as described in the article:

It could be that the cord or surge protector was damaged, try using a different power cord and a different surge protector.

After Kernal Panic can't see Time Machine Drive
I have a usb iSub speaker.
Apparently it is NOT compatible with Leopard but I have been following certain threads with interest as I hate to sell it just yet.
Yesterday there was a post that said if you go into Audio MIDI Setup and change some settings, then change them back- your iSub will work!
Well when I was in the Audio MIDI Setup utility I experienced a kernal panic (first since using Leopard)
After that I was not able to boot past the Grey Apple Screen.
I even left it overnight and it would not start.
I finally reverted to my old Tiger backup external drive- I booted to that, then started my Tiger DVD,
I ran Disk Utility, then Repaired permissions and I am back up and running.
My Time Machine hard drive is not showing on my desktop though! (I have 2 external drives- one is my old Tiger cloned back up with SuperDuper- the other is my TimeMacine backup)
I tried looking for it with DiskUtility but it is not seeing it either.
Any advice would be helpful at this point.
One other thing...it seems to be writing....the case the hard drive is in has lights on it and it is blinking..and I can hear it slightly.Hello:
If I were you (and I am not), I would bundle up the repaired computer and your Time Machine drive and take it in and ask the Apple person to bring it back to life. IMHO, that should have been done the first time.

Kernal Panic/Erasing and reinstalling
Hi everyone. Thanks for taking the time on my post.
Basically. My iBook froze up and I restarted it and it came up with the infamous flashing folder with a ? mark and the smiling Mac face. Basically, I've done everything to get into the system to change anything or reinstall whatever, but even from CD and Safe Boot WILL NOT START. I just hang out at the page. I've tried StartUp Manager. I've tried resetting the PRAM. Basically. I've let go of the few pictures I didn't put on my Flickr, and I'd like to start clean. How would I go about doing this without actually getting into OS X? I bought a new copy of OS X (Power PC Compatible), and put it in to no avail. I'm wondering if there's anything I can do before I have to send it in.
Also. While I'm posting. The silver bar looking thing that holds my display to the keyboard of my iBook snapped a while back. Basically, about a half inch of the left corner just broke off. Is there any way to repair that (if I happen to be sending it in) without replacing the display?

Last things first, it IS possible to replace just the display housing, although if you send it in, I'm not certain Apple would do that. You'd have to ask. It may take finding a local repair facility that would be willing to replace just the housing.
Now, on to the software problems. What you are describing doesn't sound like a kernel panic. Check out the Apple Knowledge Base article on dealing with a flashing question mark folder and see if anything there helps.
When starting up from an OS Install disc, hold down the c key as the startup chime is ending. Hold it down until the progress indicator (which looks like a spinning gear) appears.

Imac g5 - kernal panic, can't restart, can't start from CD- HELP
My iMac G5 went into a kernel panic last night, so I just shut it down. This morning tried to restart and it keeps going to the gray screen and telling me I need to restart my computer. I also got some code on the screen which mentioned kernel panic, so I'm pretty sure that's what happened (and I have had kernel panic problems in the past, but not recently). I put the system DVD in to try to start up from it, but it won't boot from the DVD. Does anyone know what this indicates?
I had previously had the power supply fail (replaced under warranty about 2 1/2 years ago) and last year the hard drive failed and I replaced that myself. When the power supply failed, it wouldn't do anything at all, so I don't think it's that. Could it be the logic board? Anyway, I'm pretty much resigned to buying a new computer, but just thought I'd see if maybe I could keep it going for a few more months with an inexpensive solution. Any help appreciated! (Fortunately, I did learn my lesson when the hard drive failed and have a backup drive, so I won't lose my data.)Take a look at Dr Smoke's Resolving Kernel Panics and see if anything there helps you.
It could well be the logic board, but it also could be corrupted directories. See if you can get hold of DiskWarrior or TechToolPro, they might be able to fix things. -
HT2500 I have several issues ...I can see my mail but can't open it,
My computer is doing several things all at once.
My mail will not open say nothing selected.
My Face time doesn't open
My Apple Icon where I go to the App store will not go .
I have time machine backed up but I don't know how to go retrieve it.Are you running 10.9 Mavericks?

Uninstalled Facebook due to issues...How can I reinstall on a 3rd Gen iPod
I uninstalled the Faceobbk app because it was stalling...now I cannot reinstall because it needs to update the operating system. I am currently at version 4.2.1 and says this is current.
Your only way is if you have the version that was compatible on your computer such as in the recycle bin/trash or in a computer file backup. In that case delete the current version that is in your iTunes App library and replace it with the older version. Then sync.

Kernal Panics. Again!
a couple months ago my computer all of a sudden started having these kernal panics and absolutely nothing i did, including erasing the hd and installing Leopard, helped at all. finally it just went away on its own. now, since yesterday it gets these kernal panics constantly, just like before! when i took it to the genius bar before, it ran for half an hour without crashing but when i brought it back home it acted crazy again or maybe its just a coincidence. can anyone shed some light?
I've been using Macs for a long time so please be assured that I have tried to troubleshoot this already and nothing is working. but i need help from someone who knows more than i do before i just run to the genius bar, again. its a 17" flat panel G4, 1.25Ghz processor, 160Gb hd, 768MB RAM, nothing has been recently installed or updated.I believe you have some bad memory. I've experienced this same problem on my G5. Random crashes and occasional kernal panics. I reinstalled the OS, ran disk utility, you name it, nothing worked.
Then on a hunch, I ran Tech Tool Pro. In the hardware test section I chose to loop the memory test 100 times. I did this a couple of times and the memory passed 199 times out of 200.
My particular model does not require paired memory, so I took one of my two memory cards out. The problem continued. So I put that one back in and took the other one out. The problem went away.
I bought replacement memory, installed it, and haven't had a problem since.
If I had run a hardware test only once, I would not have discovered that my memory was faulty. -
Kernal Panic upon waking from sleep
Has anyone had an issue with kernal panics upon opening the macbook pro after it has been sleeping for a bit. It seems to happen when the computer is closed and I open it and it has been unplugged and then plugged in right when the computer is turned on it kernal panics, below is the entire error log, not sure if this will help anyone help me find a solution to this very annoying problem! Any help would be appreciated and I am curious if others have this issue. I am hoping that it is something that I can fix though..

It seems as if I have the computer closed for an extended period of time, I would get an immediate kernel panic when I opened up the computer. So far, according to my panic.log, i have had 9 such crashes.
I did a reset of the PMU, disabled hibernatemode and none of that solved the problem. I'll try reinstalling everything (next week after I get my new external HD) and see if that solves the problem. Maybe this is caused by a hardware flaw? It just recently started to happen, and when I first got my MBP, this type of crash didn't happen. Other than that, my computer has been perfectly stable. Hi, I have some issue with the reports,
Can I reinstall the SCCM Reporting Services Point role without delete or change the SSRS-DB (just reset the SCCM SSRS function)?
I have saved/backup the custom made reports.
/SaiTechYes you can remove and reinstall the Role. I had to do this recently on our SCCM 2012 R2 due to an issue with the reports not loading. Found out that the problem with reports was from the upgrade to R2 from 2012 SP1 and was able to run
a command to re-register the reports with WMI. I actually removed the role, uninstalled reporting services feature from SQL, deleted the report databases, reinstalled the SQL feature, configured the reporting databases again, added the role back, and
ran the following command.
mofcomp "C:\Program Files (x86)\Microsoft SQL Server\100\Shared\sqlmgmproviderxpsp2up.mof"
This allowed all my reports to register as they should have and run without errors. If you are having similar problems, you may be able to just run that command to get reports running without all the extra work I did. For us we were getting an
invalid Class error in the srsrp.log.

IMac experiencing kernal panic. What hardware could be at fault?
My 21 inch iMac running Snow leopard with a 1.83 Ghz processor is not functioning properly. I purchased the computer used from a local university and after a few months of running Leopard it displayed the gray kernal panic screen that says "you need to restart your computer". I wiped the hard drive and did a fresh instal of Leopard and upgraded to Snow Leopard. Computer ran fine for a month or so and is now malfunctioning again.
Symptoms are:
1. Programs crashing - spontaneous crashing during use or occasionally complete failure to open a program before it quits. Restarting the computer usually fixes this problem
2. Hardware (iSight camera and airport card) not recognized. Restarting the computer sometimes fixes this problem.
3. Boot failure - either sits at the screen with the apple logo and spinning wheel or gets stuck at the first blue screen.
4. Computer has not yet displayed a kernal panic screen since reinstalling the OS
Any ideas for what hardware/software might be at fault or a course of action to further diagnose the problem? Local IT department just gave me the run-around.Thanks everyone. Here is the most recent entry in the DiagnosticReports folder:

Kernal panics, graphical issues
I installed OS X 10.4.3. I began to experience kernal panics one right after the other. The only way the computer would boot was in safe mode. I Zapped PRAM, performed fsck, reset open firmware. Checked all RAM disabled all non-shipped perifs. I even erased all hard disks, checked them out and reinstalled OS X.
What would happen in reg boot mode, the screen would get all distorted. This would usually happen when a graphic action was taking place. Like a sub-menu coming down from a window (asking for a ok or cancel action). In safe mode this never happens, but the menus are not smooth when the action is taking place in safe mode). When it crashes, a kernal panic screen occurs or i get a frozen blue screen with cursor in upper left of screen. I even installed panther on another hard drive but it did it there too. I have been running OS X since version 10.0.1 and it never did it untill tiger was installed.
I was running in safe mode almost exclusively. Then perchance I booted in reg. mode and the computer seems fine...however, when I run some graphic programs, I get a window that says OpenGL not installed or cannot access.
I did notice that preview will not display any graphic files. when I double click a jpeg file or tiff file...Preview launches but the screen is grey and no file displayed. I can open them in QuickTime or the graphic program (photoshop).
the odd thing is that when I check About this Mac, the graphics card and settings say that QuartzExtreme and core Image is not supported.
UPDATE: I downloaded and installed 10.4.4. Now I can not boot in regular mode and computer will only operate in safe mode now. I have run TechTool Pro, Norton (from the Norton Boot disk, I will not install it on my computer). Now, all of a sudden, Preview WILL display jpeg and other graphic files...Jeez! What the heck is going on???? I have tried several times to reboot to see if computer will boot normally, but to no avail.
I was wondering if I reinstalled the 10.4.4 combo update after deleting some preferences files. Any help you can give me or if you want to see any Console Logs I would be happy to forward them to you.

Hi there...thanks for the tips...These are things I tried countless times before. The only extension, per se I had as a third party item was the Intego NetBarrier extension. I took that out of the startup folder but to no avail. As I thought that my graphics card is going, I shut down the computer, pulled card, RAM etc out, left case open to dissapate any heat. Came back this morning, reinstalled everything...Wow what a difference. I could not see anything on the screen. Total distortion, vertical color lines 4 on left side of screen, pixel break-ups, etc. I rebooted, single user, fsck, open firmware reset, ZAP PRAM, etc. etc. etc. I even took out the RAM and reinstalled one by one, in each slot to eliminate that (it all seems OK on the RAM question). I must have restarted this machine at least 50 times. I have held down the shift key. Sometimes I would get to the point of seeing the log in dispite the garbled screen image. I would log in but with each mouse movement, the screen got progressively worse till I could not read a thing. On a lark I checked the seating of the RAM, my SCSI card, my extra USB ports card AND the video card....when I rebooted without holding down the shift key, it was garbled and froze. I then rebooted and held down the shift key. There were maybe 5 or so horizontal lines scattered through out the screen distorted. When It booted successfully into safe mode. When I moved a window around the screen the pixels corrected themselves. All is clear at this point, however, while in safe mode I can not print, etc. It will not allow me save a printer to save. And also when I am connected to the internet, no NetBarrier protection.
The continual break up of the screen and the correction on screen of a few stray pixels is leading me to think that the card is going. I do appreciate your help. I have owned Mac's since a MacPlus and NEVER had any hardware issue that I could not resolve myself. It is most frustrating to someone who prides himelf in knowing how to resolve a problem.
In addition, I have a program called StillLife. It assembles digital pictures into a movie with Ken Burns effects. When I installed 10.4.3, for some reason, some graphical features were disabled. I get a dialog on launch of this program I get a dialog that says: "Can't Initialize OpenGL". The strange thing is that this program ran fine BEFORE I ran a few software updates after the initial 10.4 install. Now, no go! When this all started happening, when I would open a tiff or jpeg file in preview, the screen would just be grey no image...and low and behold a few days ago...preview displays graphic and pdf files fine....It is driving me nuts...any response or further insights would be appreciated.

Startup Issues, Kernal Panics, and much much more
I'm having a myriad of issues with my G4. They started a month or 2 ago with a bunch of kernal panic restart-screen fun, at different times. Upon restart, the Question Mark folder comes on screen. Most of the time, it finds my backup system on my external drive. Occasionally it doesn't, and just sits there (so I restart).
When it does find the backup system, everything starts up fine, except my internal drive is nowhere to be found- not the desktop, in disk utility or techtool.
And sometimes, everything goes back to normal. disk utility doesnt find anything wrong, but techtool is very prone to crashing/freezing.
A few weeks ago, I backed up evreything and reformatted my internal HD (which is only a few months old). The problem seemed to stop for a while, but now its back in the past few days.
I've zapped PRAM/NVRAM, reset the PMU... And occasionally, when my intHD decides to show itself and I run disk utility on it, it freezes while "Gathering Disk Information."
It sounds like a disk problem to me, but I just reformatted the disk... could it be back so soon? Could something else be to blame?

If it were me I'd suspect a hard disk problem as well. They can and do fail, and whilst I guess it can happen at any time, they do seem to fail either very early on (in the first few months) or very late on (after several years). When the problem occurs, if the drive doesn't show up in the disk utility, it would seem to be something fairly fundamental that a reformat probably wouldn't solve anyway. I guess you could just check the power and ribbon cables are seated securely, but failing that, trying another hard disk would be one option. That would prove whether the problem lies with the drive or the computer. Most external drives are just normal internal ATA drives mounted in an external FireWire or USB enclosure. It may be possible to remove your external drive from the enclosure and install it internally just to see whether that fixes the problem. -
After install of Mavericks, it looks like my system kernal panics and just sits there. I tried waiting over two nights. Tried reinstalling twice, booting ro the repair partition and repairs permissions/drive/verify , with no problems. Tried reseting PVRAM(or whateverit called). Recovery partition now has Mavericks as OS. Im running a iMac i3 27 inch circa 2011. Any ideas? Can still bootcamp into windows.

The file seems to create the CD/DVD/Image and USB just fine. So I used this only to create a CD image which I then wrote on a blank CD which seems to work fine. Also, I played around a bit and had some partial success in booting the partition. I've updated my original opening post with the new findings.
Whew!! what a waste of time! Never want to do all of this again .

I installed Mackeeper to help with these issues but unfortunately, it seems that its part of the problem.
You don't have LS installed. The immediate culprits are the third party installations that installed these kext's.
