  • Disk Utility internal error message

I am receiving an error message every time I try to repair permissions on my MB running OS X 10.4:
    error:
    Disk Utility Internal Error
Disk Utility has lost its connection with the Disk Management Tool and cannot continue. Please quit and re launch Disk Utility.
Tried that several times to no avail. After quittting Disk Utility, system becomes very sluggish and Finder stops responding.
I then booted from the installation CD and ran a Hard Disk integrity check. It checked Ok. Then I tried a permission repair and got the exact same error message quoted above.
    Any help?

There was a problem with this a couple of years ago involving iTunes Library Receipt files. See [Mac OS X 10.3: Repair Disk Permissions 'Disk Utility has lost its connection with the Disk Management Tool.|http://docs.info.apple.com/article.html?artnum=303132].
It seems unlikely that would surface now, but the suggested fix is worth trying. The article also mentions looking at Console log. Console is in Applications/Utilities.

  • Disk Management Tool error in Disk Utility

Update: (first, reviewing problem that I and others have been having, including on 10.4.x systems):
When I attempt to repair permissions on my HD with Disk Utility, I get this specific error message: "Disk Utility internal error -- Disk Utility has lost its connection with the Disk Management Tool and cannot continue. Please quit and relaunch Disk Utility" and the repair-permissions will not proceed. It fails to get the proper "correct" permissions info for that drive to allow the repair. The error log points to a file in the Library called "DiskManagementToll" The path is ~/System/Library/PrivateFrameworks/DiskManagement.framework/Resources/DiskManag ementTool)
This problem is appearing in many forums here and elsewhere, including 10.4.x systems (like mine) and persists in a few after deleting suggested files. Is there a sophisticated solution that addresses the problem through the DiskManagementTool file by replacing some values or replacing the file from a backup vis SUDO or Root?
Previous posts have focused on Macs running 10.3.x and with iTunes 6.0x installed. Not here, so the problem is more general. None of the "delete iTunes / plists / Chess.app" suggested fixes have worked for me. Also tried running DU from my Tiger install disk; no luck! -- still fails when it tries to determine the correct permissions.
The "DiskManagementTool" file apparently either stores the correct file permissions or handles reading them from the receipts in the Library. If I understand things right, Disk Utility is actually a (user)-permissioned graphic front end, while DiskManagementTool is a Root-user-level tool that actually does the heavy lifting. Can I replace -- and any benefit to that -- or rewrite it with proper access?
There is a complete crash log report in Crash Reporter, by the way. Anyone want the text, and would it help figure out what's happening?
I've tried the suggested deletion of iTunes 6.0.2 or Chess.app and their plists (these and other steps were suggested on MacFixIt at http://www.macfixit.com/article.php?story=20060111090035797 the advice is, basically, to delete iTunes 6.0.2 and/or Chess.app and related plist files.) As I said, no luck!
Everyone that I talk to says it MUST be the fault of some other 3rd-party disk utility software.
For what it's worth, I HAVE run Disk Warrior -- and, the drive that's having the problem is a recent reinstall from a backup using Carbon Copy Cloner. The original disk is NOT having such a problem, only the hard drive on my iBook after the reinstall. Don't know if that gives any clues (or to how the link gets somehow disengaged between Disk Utility and the Disk Management Tool file / software).
Any thoughts on that? What about replacing the Disk Management Tool file in the Library or forcing some kind of re-link to it? some Unix guru MUST know a fix!
Obviously, the big worry for me is that I can't update my System to 10.4.5 or anything else without repairing permissions first. This IS a big deal. And others are having the same problem. Help?!?
    New 2 GHz dual g5 , Dual g4, & iBook G4 (recent) Mac OS X (10.4.4)

A system problem was involved that became, briefly, a "known issue" that was solved in the next minor update. Solved, anyway, by the advice given by the last poster. Thanks! SJ

  • HT201372 Failed to start erase of disk due to error (-9999, 0). A error occurred erasing the disk.

Is there an updated document ?
    MacBook 13" white - Early 2009 model

Oh, createinstallmedia only runs on OS 10.9!  Oh!  The 9999 is if you try to run it on an earlier OS!  Oh! Linc means to update the current drive to Mavericks, and then one will be able to run createinstallmedia, if one has first saved away a copy of the installer.
But if, like me, you are trying to create an installer to do a clean install on an empty drive, without replacing the earlier OS on your current drive, then...
You must use the slightly more complicated instructions you can find all over the place that look like
Right click "Install OS X Mavericks.app" and show package contents
Find a certain .dmg file and double click
Find a hidden .dmg within the drive that mounts from the first .dmg
Double-click or use "mount" on the hidden .dmg within the .dmg
Use Disk Utility to "restore" the second disk image onto the erased USB drive
Then replace an alias folder with a copy of a huge folder within the first disk image
I had success with the "Option 2: Use Disk Utility" section here.

  • WD Hard Disk I/O Error

    Hi Folks
I found one of the internal drives on my Mac Pro got heaps of "kernel: disk1s3: I/O error" messages in the system log, accordingly, some files could not be read or copied at all. The error seemed only happened when touching those particular files, most files and folder were fine with no errors shown when being copied.
I also found the disk's Raw Error Rate was 0x2A7 in its information report given by Disk Utility, as shown below. I think this should be a proof showing some kind of hardware failure on the disk, though per the report the disk only powered on for 309 (0x135) hours. The disk was from my WD My Book external enclosure which was not often used in the past four years.
Just in case of further impact on my data on the WD disk, I have already moved all the readable files to a newly bought 4TB disk. I then re-erased the disk, recreated the partition, and re-erased the partition again with 3 Pass Secure Erase selected. See below the screenshots.
Surprisely, no any errors found! I though I could see some I/O errors in the log when writing numbers for three times over the whole partition where some parts of it were ever failed to access. The Disk Utility did not report any errors either, and I did not heard any abnormal noise during the erasing process.
    So my questions now are:
    1. Does I/O Error mean physical hardware failure?
    2. Can 3-Pass Secure Erease completely fix the issue? Or can I still trust this hard disk and use it as Time Machine backup drive?
    Regarsds & thanks,
    B

    Hi Grant
Thank you very much for your thorough explanation from  concept and history to research and solution. Really impressive.
Regarding the S.M.A.R.T status, the drive was always shown Passed in green (in iStat) even when the I/O Errors were reported. From the disk information given  by Disk Utility, can I see how many blocks have been listed or substituted?
Per your suggestion, as no any error reported after rewriting with 1+3 passes of zero, I think I may safely use this disk as the Time Machine drive for my startup SSD (currently I am using a 1TB external enclosure). However, I will keep watching the reliability status especially in 6 months from now.
    Grant Bennet-Alder wrote:
Data are recorded on the disk surface with a semi-redundant error-correcting code. Hardware and firmware in the drive are used to accomplish correction, if possible. Small bursts of errors are often correctable, but sometimes it takes many, many re-tries to obtain correctable data. Failure to obtain correctable data after many re-tries produces an I/O Error.
I/O Error tells you the data pattern in a particular block you are trying to read is very difficult to read [most likely], or possibly was written wrong or partially over-written with other data if the power failed while writing [extremely unlikely].  This is most often caused by subtle changes to the magnetic regions that make up the physical data, causing  a few bits to flip.
The drive notes these problems, and maintains a "suspect list" of block numbers that are candidates for block substitution, and reports how many are on this list in its SMART status (accessible from Internally-mounted drives only). The drive is capable of substituting a spare block nearby, but cannot substitute another block unless and until new data are supplied, because doing so may poke a hole in the middle of a file.
Re-Writing the drive [traditionally with one pass of Zeroes] supplies that new data, and gives the drive an opportunity to check for proper data retention immediately after writing. Any blocks found not able to retain the new data have spares substituted, up to a maximum number of blocks in one pass (typically ten blocks).
This entire block substitution mechanism was developed in very early Disk Drives to allow slightly-less-than-perfect disks to have factory-detected defects mapped out, so that they did not have to be manufactured 100 percent perfect with no defects whatsoever. it was quickly extended to allow field-discovered defects to be mapped out as well.
It appears your drive has developed some defects in use in the field. This is fairly typical, and the spare block substitution mechanism has been able to return the drive to 100 percent good blocks. The number of times and/or power on hours has no impact -- you have a very small sample and anything could happen to one drive, including being stone cold dead out of the box. "Any drive can fail at any Time."
As long as the writing of new data into every block worked (NOT "Initialization Failed") your drive is good for now. Large scale studies performed by google in their servers indicate that drives that develop bad Blocks in continuous use will typically need to be removed for too many Bad Blocks within 6 months.

  • Disk i/o error on sleep wake ONLY

I'm having a currious and worrisome problem. For the past few weeks I've been inindated by a problem where after waking from a lengthy sleep, my late 2011 MBP will hang for about 5 minutes with a spinning beachball. I checked the console error log and notice a number of "kernel[0]: disk0s2: I/O error" lines. Of course, my first thought was that this was a physical hard drive failure. However, I've found that after this innitial wait, the computer will run for hours, even perform huge HDD seek and write tasks and produce absolutely no other disk errors, running perfectly smoothly. If this truly was a hard drive failour, wouldn't I be experiencing disk i/o errors fairly consistantly?
Another curious thing, before every batch of disk errors, I've noticed that I get a:
    "ATSFindFolder timeout. (domain:-32763, dirType:'font', createFolder:false, ATSServer pid:162)"
This may be quincidence and simply the specific task performed during sleep wakeup in line before the operation that causes the disk i/o error. However, it might be related.
    System info:
    OS 10.9.2, 16GB RAM, early 2011 MacBook Pro (8,1). 1TB 7200rpm HGST HDD.
I recently installed the new 1TB HGST drive, and used a clone program to migrate all my files over from the old drive. But around the same time I upgraded to Mavericks, and installed a host of new software (the reason for the drive upgrade). My disk i/o problems did not coincide directly with any of these updates, though. S.M.A.R.T checks out as drive verrified. All disk permissions have been fixed multiple times, PRAM zapped.

Thank you so much for your help ruwin  I also kept getting the "Disk I/O error". It only started happening tonight. I had read other things to do - tick "show hidden files, folders, and drives" which I already had ticked. I uninstalled and reinstalled skype. This also did not work. Luckily I found your comment, and it worked! I tried deleting the DbTemp folder, but when I logged into skype it still came up with the error. So I uninstalled my version again, and installed the older version like you said. So thanks heaps, I now have it working again on my laptop. I guess it was just the update, as I had the 6.9 something version on it. I knew it wasn't a problem with my actual "Skype", as it still worked on my iphone. So I figured it had to be a problem with the laptop version. You just saved me a visit to the computer shop  Thanks again!

  • Disk I/O error randomly

After I  delete the Db temp folder and my user folder, I can log into skype, but it crashes 5-10 minutes later.  If I restart my computer, I'm able to log in and use Skype for a long time.  The error usually comes within a day.  I've also tried reinstalling Skype, but the same still happens. I can post my computer specs if you need them.

  • TM on Drobo returns Error "Flushing index to disk returned an error: 0"

Hi, I'm using a sparse bundle on a Drobo (Drobo with Drobo Share / Drobo Dashboard) to back-up my Mac via TM together with TimeMachineEditor V2.5.3(26) to modify the schedule to only once a day. Since 4 months this just worked fine.

Thank you, Pondini, for insisting to go back to the other thread and try everything there. In fact, what I got wrong at first try was that I tried to look into the Sparsebundle (inspect contents) where I couldn't find the .Spotlight-V100 folder. After that I opened the Backup Volume of my iMac with Finder (having switched on hidden files before via TinkerTool) and - voilá - found the .Spotlight folder and canceled it. After that a first backup ran OK in rather short time. Thereafter the next backup took almost 2 days constantly saying: "Indexing Backup", but at the end it terminated successfully. So, everything seems to be alright now. And my old back-ups are still there :-))
    Yay!
However, to be honest, I still do not understand this behaviour of TM. For me it looks like a bug. If the .Spotlight-V100 folder causes the problem, and you can correct that by simply canceling the folder, why can't TM simply "disregard" the folder, or cancel it automatically ??
This is quite unusual. Corrupted files can make computers do all sorts of odd things. Clearly there was something very wrong in that index; +*Repair Disk+* can find many things, but not all. It will find problems in the various directories and catalogs that keep track of where files are. It may not find things that are wrong inside a file.

  • An error has occured attempting to mount a disk image "5" error #5

for quite some time now, i have not been able to load programs. it seems to have started after i did a firmware update that was meant for a OS 10.5 system and mine is OS 10.4.11. ever since then, every time i would click on "Software Update", nothing would happen, if i were to upgrade to itunes to 8.0 whatever, nothing happens. if i were to try to mount with stuffit expander i would get the following error: an error has occured attempting to mount a disk image "5", error #5. it is getting very frustrating.
    any help will be appreciated.

Hi Watsmaster, and a warm welcome to the forums!
If you haven't done any Disk or OS maintenance, then I'm thinking corrupt HDD or possibly some Hardware problem.
Could be many things, we should start with this...
    "Try Disk Utility
    1. Insert the Mac OS X Install disc that came with your computer, then restart the computer while holding the C key.
    2. When your computer finishes starting up from the disc, choose Disk Utility from the Installer menu. (In Mac OS X 10.4 or later, you must select your language first.)
*Important: Do not click Continue in the first screen of the Installer. If you do, you must restart from the disc again to access Disk Utility.*
    3. Click the First Aid tab.
    4. Click the disclosure triangle to the left of the hard drive icon to display the names of your hard disk volumes and partitions.
    5. Select your Mac OS X volume.
    6. Click Repair. Disk Utility checks and repairs the disk."
    http://docs.info.apple.com/article.html?artnum=106214
Then try a Safe Boot, (holding Shift key down at bootup), run Disk Utility in Applications>Utilities, then highlight your drive, click on Repair Permissions, reboot when it completes.

that does not sound good to me,   the message is that the disk is stuffing up,  in that skype can not read information from it,  at leat that is how I read it,   I would suggest you use a program that comes if a free cut down version http://www.hdsentinel.com/  it was so good I paid for it,  not expensive and does not expire,  so the key lasts as long as the company keeps updating, not like MS windows where you have to pay and pay,  or ithis one may be good but I have not tried it but it is free http://download.cnet.com/HDD-Health/3000-2086_4-10804806.html
another thing to also do is is check the hard disk sectors with something I would recomend http://www.partition-tool.com/ which has a free version with all the tools you want.

  • Error when starting Firefox 3.6. "Javascript Application error. Invalid username/password combination. If you continue to get this error, try entering your email address as your username." Any suggestions on what to look for to resolve this?

When starting Mozilla Firefox 3.6, I get an error window which opens up additionally overtop my browser windows stating:
    "Javascript Application"
    followed with :
"Invalid username/password combination. If you continue to get this error, try entering your email address as your username."
Obviously it is happening with one of my email addresses/usernames & password combinations of which I know the username for (which I choose not to post here) and I'm not sure where to begin looking for a resolution to this problem.

I was able to resolve this by disabling (under Tools - Add-ons) Yahoo! Mail Notifier

  • Contacts on client machine suffer from disk I/O errors

Contacts are running slow if not at all on my client machines.  I check the logs and it is having trouble with accessing the files and or writing the files.  all other programs seem to work fine.  Mail hangs but I think it is because of the contacts issue.  It seems to have trouble reading the contacts list and dumps the list and attempts to rebuild from iCloud and the server.  there are many .unknown files in the library.  The logs state that there is a disk I/O error while accessing /Network/myserver/Volumes/MyBook/Users/username/Library/Address Book (this is from memory but it is close).  The contacts work perfectly (as expected) if the user is logged into the server.
I am running 10.8 server and clients with shared home folders.  the user folders are on an external drive.  the drive does have permissions turned on.  I have wiped out the contacts directory in the  Library folder and it proceeds to rebuild but the problem is still there.

At first I thought it may be related to upgraded 10.8 clients and server i.e. went throught the normal install option without re-installing OSX.
So I completely rebuild (i.e. erased and then installed Mountain Lion from USB stick) the server and the client machines...but I still get the same issue, so clearly this is NOT the solution.
I have also observed (upon reading others' issues with Contacts/mail) apple forums - so I followed the the same action (trashed the Address Book folder in ~Library/Application Support/ ) after unchecking everything in iCloud in system prefs/restarting etc.) but this doesn't so solve the problem either
I also observed teh following behaviours:
Login into iCloud for the first time - appears to be ok
Run Contacts - appears to be ok
Wait the addresses to sync from iCloud - appears to be ok
    Quit
Run Contacts - still appears to be ok
(i.e. the errors above are not displayed in log)
Run Mail (which takes a long time)
Contacts sudden looses all address and effectively both Mail and Contacts freeeze. It is at this point I start seeing the above errors
I get a further odd error in Contacts (in a triange with '!' warning)
Basically Contacts and mail don't seem to want to work together - they appear ok in isolation - obviously this isn't acceptable.
I've noted that if I create a local logon on any one of the client macs, the issue with contacts/mail does not occur and runs perfectly smoothly. So in my eyes it appears to be a problem in Mountain Lion Server/Client Combo Only (I did not have this issue under Lion Server/Client Combo)
