SQLDev refusing to emit most DBMS_OUTPUT info... what is up?

I have been having serious problems getting DBMS_OUTPUT out of SQL Developer. I go to the dbms_output tab and enable it, set polling quick, have plenty of buffer....
I have done DBMS_OUTPUT.ENABLE (20000) in my code...
Certain PL/SQL code just seems not to emit output. If I run the same block in SQL*Plus with serveroutput on, it is fine.

Windows XP version 2002 SP 2 (I believe this is pretty up-to-date patch)

I haven't had this sort of problem with the DBMS Output.
Note that enabling DBMS Output on the tab automatically does a DBMS_OUTPUT.ENABLE(<buffer size>).
Have you applied any of the post-prod patches? Go to Help -> About -> Extensions and check the version of Oracle SQL Developer - I see 10.1.3.1557, which means I have Patch 2 installed (1527 = Patch 1 and 1467 = Prod).
What version of DB are you connected to?
What to you mean by "certain PL/SQL code"?
Does it work if you do a simple, enable dbms output and then run:
begin
dbms_output.put_line('Hello World');
end;
One thing that I have noticed as a problem with the DBMS Output enable button, is that if you set it on for one worksheet and then open up another worksheet, the button looks enabled on the second worksheet but DBMS Output is not. To switch it on in this case, click the enable button twice to switch it off and then on.
Another thing that can be confusing is that if you have multiple worksheets open for the same connection and have explicitly enabled DBMS Output on both of them, then only one of them will get the DBMS Output.
