Switch 2 was removed from stack unexpectedly and again joined stack

Hello,
we have 2  switches in stack, last night switch 2 was removed from stack and got rebooted and again it got back in stack.
Switch -2 entered  into the provisioned mode for certain span of time. snap shot is attached .
