Table SWWWIHEAD cannot be written URGENT

All my workflows are failing with this error. It says that the workflow engine is not able to create entries in this table because of duplicate entries error.
When I go to the workflow log, I see work items numbers such as 356, 357 etc where as the maximum work item number that is there in the table till yesterday was 1257999. It looks till for the workflows which are in process or the new workflows that are starting are not picking up the next number which should be 1258000 but instead using lower numbers that are available in the table such as 357 etc and then failing with duplicate entry message when trying to insert records in SWWWIHEAD.
Can anyone help me with this? This is in production environment and kind of URGENT. I will award sufficient award points for the person who points us in right direction.

Thanks a lot for your response. But the 'Maintain Prefix Numbers' under 'Maintain Definition Enviroment' is never maintained is SWU3. It has a red cross against it. When I check the entried, there is nothing for workflow work items. It has some prefix numbers for various SAP systems and client.
I do not think the work item id is coming from here. Can you please check and let me know? For some reason there is no work items that are getting generated in the system. Also I see lot of dumps in ST22 with insert to SWW_CONT failed message. These inserts are failing due to duplicate entry message. I am not sure why the workflow are trying to create entries again in this table when it is already there.

From last few days, In SM12 there are lock entries coming for workitems of table SWWWIHEAD even though workitem is completed successfully. Time of lock entry shows it is time when approver started taking action on it.
Please let me know is there any impact of this on system and how it is happening and what we should do with these lock entries.

Actually we faced an issue related to this like the approver took action on it. after that the item got locked. the basis is telling there could be the RFC session from SAP Portal inbox which is not closed somehow. may be session is exited without properly logging out. in this case the item was still in his inbox and when the approver tries to open it, the error message says workitem is locked bye the same user id. The basis is telling the lock will be removed after the default stipulated time (in my case, its 8 Hrs ! ).

The attempt to burn a disc failed. The disk is locked and cannot be written

I am having a problem burning CDs. iTunes begins to burn CD than cancels with the following message "The attempt to burn a disc failed. The disk is locked and cannot be written." After some research I found the following article "Burning disc in iTunes for Windows using a Sony CD-RW/DVD CRX830E model drive fails with 4280 error." I followed directions as noted, this worked for the burning of one CD, then failed message returned. I have tried different CDs and lowering record speed to no avail. Burner worked up until updating to latest iTunes. Here is the diagnostic report

Ipod write protection error - disk is locked and cannot be written to

Has anyone experienced this 'disk is locked and cannot be written to' when trying to update their ipod mini? I've tried everything, including all 4 R's, but still get the same message. And now that i've restored my ipod to its factory settings, I can no longer see my ipod in iTunes 7. So frustrating! Any thoughts are appreciated...i've just about given up. thanks.

Hello.
I keep getting this "The iPod '...' cannot be synced. The disk is locked and cannot be written to." error when I try to sync my 11 GB of music from my very old, updated Windows XP Pro SP3 machine to an old iPod Touch 1G (iOS v3.1.3). It has a brand new (lat/new)est iTunes installation too. I tried rebooting the the machine, disconnecting USB cables, etc. None of them helped. I did not have this problem in the end of 2013. Is my iPod Touch 1G dying?

I did a reset since I had to restore v3.1.3 firmware and reconfigure from a Mac OS X 10.5.8 MBP before I ran into problems. Isn't that enough? Or do I have to do it again? Also, these were brand new installations and setups for iTunes and iPod Touch 1G. So, I am doing everything from scratch.
I also found out that iTunes wants full (not read) access to my shared medias in my very old, updated Windows XP Pro SP3 machine. When I gave them full access, then I got farther, but it still failed later on (almost half way since 5 out of 11 GB according to math with disk spaces on both iTunes and iOS v3.1.3?) I didn't use Mac OS X 10.5.8's iTunes (only kept it for whited00r firmware part) for syncing since all my media is on the other machine.

InDesign CS6 "Failed to export the JPEG file. File cannot be written. Disk may be locked or full."

I get this error "Failed to export the JPEG file. File cannot be written. Disk may be locked or full." when sending small (3"x3") and large (40"x60") files to a printer hot folder. Sometime if I close the program and restart it will let me export. What could be causing this to happen?
I am on Windows 7 with 4GB RAM PC.

I guess the printer is grabbing the file before it is entirely written…
Workaround: Do not save to a hot folder. Move the files after exporting to the hot folder.

Just upgraded to Mavericks. Now I cannot access scans from my hp scanner. After it complete the scans and I try to save as tiff, jpeg or pdf I get a either a grey blur or a message that says cannot save because the file cannot be written to.

I did not have any problems before the upgrade. I would greatly appreciate any light shed on this.

Go to the HP site and look for drivers for your scanner. It apparently needs an updated driver to work with Mavericks.

The "Exception" field cannot be written

When I try to enter a website as an exception (so the password I use for it will not be saved anymore), the field cannot be written. The only action I can do is to click on the "Close" button !

The problem is that this info bar appears only when you visit a website for the first time.
I have already tried to ask the password manager to forget the password I wanted to be forgotten, but it does not work, it still appears by default in its field when I go to the site.

I have made a change to ZTABLE ( essentially added another field which is not key field ).
i have checked that all are active.
technical settings are complete too.
now when i adjust in se14 it shows me no error.
but when i transport to Quality i get the eroor
E-nametab for table ZPP_MATERIAL cannot be generated.
hence it does not refelcet in another server...
what shal i do?

check the transport request is transported properly are not if yes transport again.
and there is a possibulity of copying the dataelement in other request.
check with all the requests by checking these two you will definatly solve your problem.

When trying to create a Access sequence using the condition table 499, (Movement Type) the following error message is occuring:
"Switch: Condition table B499 cannot be used".
When we check the switch(DIMP_GENERAL_APP) relevant for this, the status is "OFF"
Our question is By activating the switch are there any implications?

If the media is in the library then yes it is that simple. Connect the device, load it up with your media. The problem is when you try to connect a full device to a new empty library on a new computer when iTunes will offer to replace the content on the device with that in the local library, i.e. nothing! (For anyone in that position see Recover your iTunes library from your iPod or iOS device).
You should be able to add your devices at https://supportprofile.apple.com/ though I'm not sure what happens if the second hand iPod was previously registered.
