TS1292 Brand new itunes card,when enter code says already redeemed.

When I enter a brand new card on iTunes store,I get a message that the card has already been redeemed. How can that be? What do I do about it?

I've had this problem as well. Apple Support are telling me that I need to provide copies of receipts from the store it was purchased in, but this was a gift (as in "Gift Card"), so I don't know where it was purchased and can't get the receipt.  I do know that the code cover was completely unscratched when I used it.  I'm really annoyed about this - feel very let down by Apple.  People need to be aware that these cards are not secure.   If you buy them then you need to keep the receipts and ideally video record yourself scratching the cover and redeeming.
