User completes approval task but system is not marking it completed

My SP designer workflow has intermittent issues completing the task after the users does their part. We've taken one of the web servers out due to the thought that the sessions were getting split up (IIS logs showed this happening) between the
two servers, but the issue resurfaced today. This user completed one task no problem, but when he completed the next task shortly after the system didn't finish up the second task so it 'stalls' (that's what I call it). The stalled task
doesn't get completed so the workflow doesn't move on. I'm out of ideas.
I've searched but can't find any reference to this behavior with approval tasks. It is pretty straightforward OOTB approval processing but has intermittent issues.

Thank you for your reply. Here are the responses to your questions:
No, there are no errors in the workflow when the task is not completed. I've turned on versioning in the task list and versioning shows the user modifying the task but the 3rd step of the system marking the task complete isn't occurring.
No, the issue occurs intermittently which has made it almost impossible to figure out the cause.
Creating a new approval workflow with the exact steps is really labor intensive unless we copy and modify it. This has not been done.
We've been looking at the ULS logs and the Workflow trace logging and what we recently discovered is an Access Denied error which has been narrowed down to a possible expired User-token and look-up to another domain by the initiator's ID. All of our
users are in domains other than the domain the SharePoint servers are in but there are two way trusts established. We have since moved the approval processing step into an Impersonation step in the workflow which runs under the system account.
We are now watching and waiting to see if this will help.
Another thing to note is all our users are granted access to SharePoint through AD groups which are then added to SharePoint groups.
We are working with Microsoft on this and hopefully we can get it resolved. When we do I will post the outcome, in the meantime I welcome any other suggestions.

The only reason why Acrobat installation fails and throw this error message is when there are some services running in the background causing the installer not to complete the installation. My take is try the following steps and suggest if you were able to install the products or not :
1) Navigate to control panel and confirm that there is no line Item that talks about Acrobat. If you see Acrobat XI listed there, please remove them.
2) Perform a selective Start up using the steps mentioned in this link.
Please suggest if the above mentioned steps were able to resolve the query for you or not.

Setting dynamic user in Approval task

While setting the user at runtime for approval task, I have created a Task Assignment adapter and associate the adapter in the assignment tab for the approval task. I have used reassignApprovalTasksToUser method of tcRequestOperationsIntf class. I have passed the task instance id as well as the user key. From the system console I am able to find that the adapter is invoked.
I have suppress the standard approval process using tcComplete task adapter.
When the requestor make the request and goto approval details page the system is showing the request is pending to the re-assigned user however when I login into the console using re-assigned user credential no request is pending in his work busket. What I observed, the request is pending in xelsysadm work busket.
Would appreciate if someone let me know the step which I am missing.

Yes, I suppressed the "Approve" Task in standard approval process.
Can you please let me know how to mapped the key type in the task assignment adapter.
I have created two variables in the adapter and mapped those two variables from the assignment tab of the approval task.
I have seen there are two more variables in the Adapters, "Adapter return value for Key type" and "Adapter return value for Key" and these two adapter variables are mapped as "Set at runtime".
How do I map these two variable?
No I do not find any exception with regard to execution of the task assignment adapter in the log.

Diamond - 4870 Installed ...... but system will not boot - no display..

Finally got my special 6-pin PCI std. connector to a MAC PRO 6-pin smaller power connector.
The ATI-X1900 works like a champ, when I remove it an install the Diamond ATI-4870 (MAC VERSION) in and connect the two power blocks on the motherboard, the system beeps like it is booting, but I do not get any output from the ATI-4870 card. The 4870's fan is running and it sounds like the system is booting, but it never displays anything.
Any Ideas ??

Update to iOS 7.04 on ipad2. The set up screen appears but it will not let me continue the set up. It will not let me get to the next screen after choose wifi network. It is if I am stuck on the choose wifi network page. Cannot cancel or continue. Cannot switch it off. Holding home button brings set up is not complete message. But it won't let me continue. That sounds like a failed logic board or display module on the logic board...either way you need to take it to an Apple store genius bar and have the techs test it to see exactly what has failed. The testing is free so give the techs a chance at diagnosing the problem.

Video Not Marking as Complete Until Exit Button is Clicked?

I recieved the following feedback from one of our clients. They are using an AICC based LMS.
"We noticed that in order for the video to mark as completed, we need to have all of the chapters checked off, quiz must be taken, and the user has to press the small Exit button."
Do users actually have to press the exit button before a video will send "completed" to an LMS? Is there a work around?

What Captivate version are you using?
This is happening based upon the Quiz Preferences - Reporting Settings that you have configured.
Yes, you can customize the settings to mark completeing on basis of user access, slide view, Quiz attempts and many more criteria.
Please choose and customize based upon your requirement--
Instead of making the user hit the close button, you can yourself set Captivate to Close the course once it reaches the end.
You can do that fron Start and End Settings under Preferences.

I believe the virus is associated with firefox, I run a complete scan using McAfee, anti virus, but it shows up nothing. Firefox constantly shows no responding signs, then it will be normal again. Or I have to go into task manager and end firefox.
There is a fake transaction from my bank account, is there anyway I an remove the trojan?

try with a few malware/virus scan programs. You need to use all programs because each detects different malware. Make sure to update each program to get the latest version before doing a scan.

I downloaded 4.0 when prompted, but it did not open upon completion and now shortcut does not work and cannot find file to run 4.0 in the firefox folder. Tried restarts and reinstalling 3.6 which now also won't work for me. Ideas?

The shortcut now is broken and efforts to fix it fail because the file cannot be found. Checked the firefox folder and do not see a file to run firefox. Tried restarting twice with no luck. OK. Got it figured out. Disabled my virus scanning software and the download worked perfectly. Should have thought of that before now.

If the old ID ("email address") is your ID, and if your new ID was created by editing the details of this old ID (rather than being an entirely new ID), go to https://appleid.apple.com, click Manage my Apple ID and sign in with your current iCloud ID. Click edit next to the primary email account, change it back to your old email address and save the change. Then edit the name of the account to change it back to your old email address. Then save any photo stream photos that you wish to keep to your camera roll. Neighbors Have High Speed Internet but Systems Says Not Available

I have been trying for the past week to get High Speed Internet with Verizon. Two times I received an email saying… Although your line qualified for High Speed Internet service during the initial phases of the order process, unfortunately, we are unable to provide you with Internet access on your line ending in 0602 .
I live in a small subdivision on the outskirts of Colonial Beach, Virginia with approximately 500 homes.
I have two neighbors, one on each side of me i one would be the closest to the substation) and another neighbor about one block past my house.  The second neighbor drives past my house every day to get to their house.
I spoke with a local Verizon employee who told me the computer was turning down the High Speed Internet because of distance and that I needed to have someone at the local office, over-ride the automatic turn down so that it would be installed. After the second computer automated turn down, I spoke with Eric – he said to give the next tech this information – Acct Number XXXXXX Order Number XXXXX at the DSL Technical support so we could get this resolved. The DSL Technical support guy was NOT interested in obtaining the account number or Order number Eric gave and said either the Router was filled or I was too far from the Loop. I explained the neighbors and it still didnot seem to matter. He was foreign and we did seem to have a significant language barrier. He then transferred me back to Sales.
Over and over I requested to speak with the actual Local Office in Warsaw, VA so I could be given a reasonable explanation.   I spoke with a woman who said the two neighbors were closer to the loop than me. When I asked how this was possible if I am in the center of the two she said maybe it was because of a curve in the road? She also said she was the local office but was not able to tell me where that office was located so obviously it was not the actual "local" office.
How can I get someone to physically come out and determine why I cannot get High Speed Internet?  Who can I contact to make a formal complaint.

I have put mac os update from 10.9 to 10.9.5 from mac app store. Update wants restart and I do that. First it shows installing updates on monitor and then after waiting for more than 30 minutes, it did not restart. On monitor it shows no signal now. I have checked all the connections. Mac mini LED light is showing blue color and i think its signalled the ON mode. Also mouse lights are on. Please provide help, as I need to solve it asap.
