Using a Feedback Form FLA in Captivate

Question

To anyone looking for a challenge,
I'm a longtime captivate user going back to the RoboDemo
days. I've been excited to see its evolution over the last couple
of years and recently began a project that required feedback from
end users. I've coded a Flash FLA that is a form. The form pulls in
the title of the main captivate file, the slide number, and has a
field for a reviewers name and comments. I'm trying to add the
custom animation to a captivate project so that my users can press
the form button, have it slide out, fill out the fields and press
the submit button to send the info to an ASP page that will put the
data into a database. HAS ANYONE TRIED ANYTHING LIKE THIS BEFORE?
I've been able to get some things to work. I'm still having issues
with getting the custom fla animation to send the data to the asp
page. Any help or alternitive ideas would be greatly appreciated.

I have created an area for someone to type some words in, which you can then save (see attached)
They said this is not an option when opening in reader 10. This is the comment we received.
“there appears to be a small editable text line after the “installation ID” tag, however this seems to be blocked by the non-editable blue line that the text is supposed to sit on.  It would require an editable text space that sits on top of the line rather than before it in order for it to work effectively.”
I’ve never had this problem producing pdfs for acrobat 9.
