Turn your phone off, then:

If you still dont see the recovery message repeat these steps again. iTunes will give you the option to restore from a backup or set up as new.

Plug your iPod into iTunes. Click on your iPod in the left panel. After you click on your iPod, scroll down and there should be a restore button. Click that and go from there. Let me know if it works. (it might take awhile for it to restore)

If you haven't backed up /imported the photos, it's most likely you can't access that data. Passwords/code are made for security, remember them in the future.
You could try importing photos using
iOS: Importing personal photos and videos from iOS devices to your computer
You can use
iOS: Forgotten passcode or device disabled after entering wrong passcode
to restore the device, but everything that hasn't been backed up(purchases excluded) will be lost.
After restoring, you can redownload your purchases using this:
Downloading past purchases from the App Store, iBookstore, and iTunes Store

You can use one account with iTunes on your Mac, and different account(s) with the Mac App Store (for buying/downloads apps or OS updates for your Mac), iCloud (System Preferences > iCloud) and Messages (Messages > Preferences, or command-comma)

I'm not sure if that would work, but just give it a shot.
Don't only hold the home button and the wake/sleep button down till it resets,
try holding it down for about 30-40 seconds.
This time it should not just reset. You just have to make sure it's long enough. You will see ;-)
But then, you will have to put everything back on it again.
Better than having no access, I guess ;-)

Easy-these are just aliases. Do a cloverleaf+F/command+F (Find) and they should appear. Either create an alias and drag it to the dock-or there's an "add to Dock" command in the Finder. so click on whichever app you'd like to add to the Dock, then add to Dock and it should appear there. If something isn't in the Dock that doesn't mean it's gone for good. It's just an alias
