This came from another thread, but worked for me. Problem is with newest iTunes 4.62. Unistall iTunes, as well as its helper programs (Apple Application Support, Apple Mobile Divice Support, and Apple Software Update). Reboot, and reinstall iTunes. It will reinstall all the other ones and you should be able to connect your nano. Good luck.

1. Update iTunes to the latest version. Plug in your iPod. If iTunes still can't recognize it, then in iTunes in the top left corner click help> run diagnostics. On the box that comes up, check the last two things. Click next and it should identify your iPod.
2. Click on your windows start menu. Type in "services". Click on it and when it pops up, on the bottom of it click on "standard". Now Scroll down to find "Apple Mobile Device" Right click it when you see it and click on "Start". When it has started, close iTunes and replug in your iPod and it should show up.
3. Check the USB cable
4. Verify that Apple Mobile Device Support is installed
5. Restart the Apple Mobile Device Service and verify that the Apple Mobile Device USB Driver is installed.
6. If you just want to add some photos, songs and movies from computer to your devices, you can use an iTunes alternative to do the job.
7. Check for third-party software conflicts.

I found I had the same problem so I uninstalled iTunes, Apple Mobile Device Support, and Apple Application Support and then redownloaded the latest version of iTunes which reinstalled everything I uninstalled and I was able to sync my 1st Gen iPad and my 3rd Generation Nano with iTunes 11.1

I seem to recall that this may be cause by overloading the USB bus (lack of power to go around). Shut down your computer. Reset the iPod shuffle using this procedure.

Disconnect ALL peripheral USB devices, including keyboard and mouse. Power up the computer. If you need your mouse, connect it and launch iTunes. Connect the iPod shuffle to a direct USB port and try the sync again. See if there is any difference.

If you're able to connect to your email server and are able send email to other email addresses and if the failures are specific to one email recipient address, then please contact the intended recipient of the failing email, and confirm their address is valid. If you are unable to send any email to any other email addresses and this is specific to your email address, then try the web mail client interface (if one is available) to verify your login user and password, and check with your email ISP for assistance. I just encountered the same problem on my 2 month old MBPro. I found a work around by changing USB ports. As for the actual cause or the reason why that worked, I dont know. The USB port I was using works fine with my phone or other USB devices.

I also had issues opening AVCHD files on case-sensitive filesystems.
You can try these steps to solve the issue:
Right click on the AVCHD file and select 'Show Package Contents'
Right click on the BDMV file and select 'Show Package Contents'
Rename index.bdm to INDEX.BDM (it seems that all files in the BDMV folder should be in UPPERCASE for Quicktime to open the AVCHD file).
Go back twice and try again to double-click on the AVCHD file (Quicktime should open a window showing the multiple clips, as expected).
