At the time that that you purchase an unlocked iPhone from Apple iit will most likely not include a SIM. This has been the case with the 4 and 4S. You will have to provide your own, depending on what carrier you choose. If you decide to swictch your ATT contract to the iPhone they should provide you with a SIM.
Plus you will be able to canccel your iPhone ATT contract at anytime since you are providing your own iPhone.

    We can't speak for Apple, but I highly doubt that there will be further price drops to the iPhone 4, not just because of the release of the unlocked iPhone 5, anyway.

    Anyone know when will the factory unlocked iPhone 5 release?

    Last year iPhone 4s was announced Oct 4, and factory unlocked sales started Nov 11 (I ordered mine then and got it right before Thanksgiving). So I'd guess in a month or two, but who knows if they'll treat it differently given the multiple models for LTE.

    Was never stated. My gut is before actually. I would think they would want current users to see the upgrade (and show off) to excite more people. Also to keep load on servers down as while the iPhone will be activated in store this time, it still has to hit back to Apple to do so.

    iOS 6 is set to release this fall. Apple has not been more specific that that.
