If you have a payment source associated with your Apple ID, it needs to be a vaild form of payment.  To troubleshoot issues with your payment type being declined, please follow the steps below.
In most cases, this issue is caused by a formatting error in the billing address that you have provided. You can correct the issue by verifying that the information you have provided is correct, or by changing the format of your address to match the format that your credit card company has on file.
You can verify and/or modify your billing address by following these instructions:
Open iTunes.  If you are signed in to iTunes, skip to step 4.
Note: Verify that you are signed in to the correct iTunes account. If you are unsure which account you're signed into, choose Store > Sign Out and proceed to step 2.
Choose Store > Sign In.
Enter your account name and password, then click the Sign In button.
Choose Store > View My Account.
Click the Edit Payment Information button.
In the Payment Method section, choose None.
Click the Done button at the bottom of the page.
Click the Edit Payment Information button again.
Enter your billing address exactly as it appears on your credit card statement.
Select your credit card type in the Payment Method section.
Enter your credit card information, your card's security code, and the expiration date.
Click the Done button at the bottom of the page.
If the issue persists, contact your credit card company and verify that they and any company they use to process credit card authorizations have the correct information on file.

    As the article below states you do need to have payment information on file if you have already created an Apple ID and are using the iTunes Store or App Store for the first time.  If you want to remove your payment information afterwards, you can follow the steps in the article below to do so.

    To download apps, even free apps, you usually need the payment information associated with your Apple ID. You most likely have your credit card name, billing address, card number and expiration date already on file or you would be asked for these too. Apparently though your security code is not on file with your Apple ID. To avoid being asked each time to download an app go to your Apple ID and edit your payment information to include the security code.

    If the amount of the purchase is close to $21.07, then you need to take into account taxes may be added.
    If you go beyond the credit from a gift card the balance will be billed to your credit card account.

    My younger brother had the same problem. You have to go into settings, app store, details and then change your credit card details to none. If there is no none option, you will have to use a different apple id.

    Thanks diesel....tried that as well, but failed to mention it in my OP. No luck. Also rebooted, unplugged, reset, etc....no luck as well. It has to be something on the ATV though since it rented just fine on my phone. I have 13 $1 pending charges on the 2nd credit card I tried, so it was looking, just not verifying.....strange. Have to think it wouldn't be my bank though, since again, renting on my phone went fine.

    You can create an iTune and App Store account without credit card details
    1. Sign out of current Apple ID if you are sign-in to one (important)
    2. Go to App Store and select a free app
    3. Tap INSTALL APP
    4. Create New Apple ID
    5. Confirm Your Country
    6. Agree with Terms and Conditions
    7. Fill in your Apple ID and Password (you must create a new Apple ID; don't use your old Apple ID)
    8. Create and answer your secret question
    9. Select NONE for Payment Method
    10. Fill in Billing Address
    11. Submit application for new Apple ID
    12. Wait for verification email
    13. When email arrive, verify your account
    14. Start downloading your free apps

    If you update your billing information, the iTunes Store will place an authorization hold on your payment card account, usually in the amount of 1 USD, to verify your account information. It will be credited back to your account later.

    Why does it do that? It used to do that for free apps to but once I put my credit card on it, it stop asking for free apps. But now it still wants a card even though I still have 20 dollars left from a birthday gift card. I heard somewhere that this was because of a new feature to stop kids from downloading apps that their parents don't want them to or buying a bunch of apps and racking up a huge bill, is this part of it?

    Is the card expired? You cannot have an expired card in your account.

    She hasn't changed countries nor regions, she's not using the either store for the first time, nor does she owe anything.  She doesn't have a gift card.  She's just a person without a credit card.  She did have one, and now she doesn't.

    It is not being deducted from your credit?  Are you sure?
    Just because it is asking for your cc info, does not mean that it will be charged.
    This is how itunes verifies your account and that you are a resident of the country of the itunes store that you are trying to use.

    iCloud backs up your iOS device (not your Mac) automatically. You can remove the backup:

    and it will not back up again. This will not affect a backup made using iTunes. The notice you see refers to calendars, contacts, iWork documents and so on, which live on the server and are read from there by your Mac, and so will obviously disappear if you turn iCloud off altogether (which you don't need to do) or turn off Documents (which you don't need to do either).
