Install iTunes 11?
Put songs into a Playlist Folder?
Is the Playlist a Smart Playlist?
Number two, putting songs into a Playlist Folder is guaranteed to cause songs to be "lost" from the list, eventually.
So - if you need more help than the possible causes listed here, you need to tell us a bit more about the issue. One assumes for instance (since you've posted in the iTunes for Windows forum), that this is ocurring in iTunes. What devices are you Syncing with your iTunes Library, and what are the Playlists like on them?

For Example my album "AC DC Live" has track #3 Back in Black as the first song listed. How can this be fixed?

Have you tried resetting your phone (hold down the Home and Power buttons a the same time and continue to hold until the Silver Apple appears). Once the Home screen comes back up, see if your Music is OK.
If it is not, then, have you sync'd your phone to an iTunes library on a computer? If so, then hook it up to the computer, uncheck the "Sync Music" option so that no music is sync'd and then sync the phone. This should delete everything on the phone. Then go back and re-check the "Sync Music" option, and select any other criteria you want on that screen, and then sync again. This should put back all of your music.

If you have content eligible for icloud then you can stream it directly. Of course any local content requires your computer on and running iTunes to be able to see your library.

Why are some of the songs I created in garageband and then pushed to itunes "ineligible for icloud"?

I create songs in garageband on my iMac, then "share" them to itunes. I purchased Itunes Match so that I could hear them on my iphone 5 (7.1.2), but some of the songs will not show up on my phone. When I checked them on the computer, it says, "This item is ineligible for icloud". Why not?

I am trying to import cd to my itunes playlist. Some of the songs do not download properly. On the playlist is indicates "unavailable". Why is that? Can anyone help me.

After syncing my IPhone to ITunes that I had just downloaded on the new pc I just bought, I noticed that there are several songs that are now "grayed out" on my phone and won't play. Why are some of my songs showing up under # instead of first letter on my iPod?

Kinda hard to explain so I'll add a picture.
Anyhow, some songs on my iPod are showing up under the # category instead of being under the letter that the song title starts with. Anyone know why? I thought it was ones that I uploaded from physical CDs that I bought, but it's also happening with some songs that I buy from iTunes/Amazon/etc. too!
Its really frustrating because if I want to listen to a certain song, I either have to look through the # list if it isn't categorized right, or I have to click artist and search through that way.
Example: 'Dirty Country' should be under the 'D' category, not '#'

Depending upon what country that you are in (music can't be re-downloaded in all countries) then try deleting the incomplete tracks from your iTunes library and redownload them via the Purchased link under Quick Links on the right-hand side of the iTunes store home page on your computer's iTunes : re-downloading.
If you aren't in a country where you can re-download music or if they re-download in the same state then try the 'report a problem' link from your purchase history : log into your account on your computer's iTunes via Store > View My Account and you should then see a Purchase History section with a 'see all' link to the right of it ; click on that and you should see a list of your purchases ; find that song and use the 'Report a Problem' link and fill in details about the problem (iTunes support should reply within, I think, 24 to 48 hours).
Some people have had a problem with the 'report a problem' link (it's been taking people to this site on a browser instead of showing a form in iTunes) - if it does that to you then try contacting iTunes support via this page : http://www.apple.com/support/itunes/contact/- click on Contact iTunes Store Support on the right-hand side of the page.

Why are some of my songs cut off, or don't play at all?

So recently a few of the songs that I've purchased have been screwed up. Some cut off midplay like the whole song wasn'tdownloaded or something. And other songs just won't play at all! It's super frustrating to buy something and have it screwed up.

Why are some of my songs in light grey and have a circle beside them?

in my itunes a lot of my tunes appear in light grey and have a perforated? circle appearing in the column on the far left side what does that mean

Are you sure you aren't looking at the tracks in your iPhone/iPod?
That greyed out and perforated circle thing happens when your iPhone is plugged in, you are looking at the music in it and some songs are physically in your phone (ready to play) and some are in the cloud (you can download to your phone whenever you want).

I am syncing playlists and a few of my songs are not syncing when the rest of them are. I have checked that they have been downloaded (and purchased!) and have a check mark in front of them. They are in the playlist lineup...

Do the songs play in iTunes?
Do you have any Restrictions (Settings>General>Restrictions) set that would prevent syncing those songs? (iTunes purchases only)
