    How do you mean? What does happen is that Apple puts a $1 hold on  your account for free apps. That $1 is then refunded so the free app is still free.
    Another thing, many/most free apps are rife with in app purchases. Is it possible that's what's happening?

    How much is the 'charge' ? You aren't charged for downloading free apps (though some then allow you to make in-app purchases in them).
    Did you add or change your credit card details on your iTunes account ? If you did then each time that you do so a small temporary store holding charge ($1 or approximate local equivalent) may be applied to check that the card details are correct and valid and that it's registered to exactly the same name and address as on your iTunes account - it should disappear off your account within a few days or so.

    Maybe you went back to the store and repurhcased it. To downlaod past purhcases for free, you need to use your purhcase history.

    That is determined by what type of disk that it is. It may be a disk that only allows you to install Windows on your Dell computer and would not allow Windows to be installed on a Boot Camp partitition or as a virtual machine in an app such as VirtualBox on your Mac. If that is the case, you would need to purchase a retail Windows install disk.

    You most likely do not have a valid form of payment entered in your Apple ID. The valid form of payment is required to download free apps or purchase apps. If the app is free you will not be charged. You can enter a credit card as a valid form of payment. If you don't want to use a credit card or don't have one, you can purchase an iTunes gift card and use that as your valid form of payment.

Not all games make it clear that coins, gems and such cost real money. She may also want to turn off the ability to install apps, and set the password to be required immediately.     As to a refund, that's not automatic since the terms of sale for the iTunes Store state that all sales are final. You can contact the iTunes Store as AnaMusic suggested, explain the reason for your request, and ask, though. They're usually pretty lenient in the case of inadvertent purchases.

    Report to Apple and they should be able to provide the credit.  In iTunes under the Store menu, select View Account.  There you should find a selection for Purchase History.  Find the item and select the arrow tab next to it where you will find a selection to Report a Problem.  From my experience Apple will get back to you within 24-hours or less.

    Sounds like you have multiple accounts that got upgraded. Contact that payments team through the online contact form and they will be able to locate the other account. If you receive an automated reply directing you back to the community, just reply to it and you should receive a  response from a representative within 24-48 hours.
