Why is ringtone in limbo in iTunes and missing from iPhone after last iOS 6 update?

I have a ringtone that I purchased that has now disappeared from my iPhone 5 after the last iOS 6 update. If I look in iTunes (which is an abomination in itself and not intuitive in any way), in the On This iPhone tab of the iPhone page, with Tones selected on the left hand side menu,
I see the ringtone "Stewie Mom Mom Mom" grayed out with a weird circle to the left of it . I have a second ringtone that is also visible there, with no weird circle.
If I go to the Tones tab of the iPhone page, I see that Sync Tones is checked and it shows 1 tone synched...the one without the weird circle shown in Tones.jpg is actually on my iPhone, so that is the 1 tone. No matter what I do to try to manually sync the tones to my iPhone, Stewie Mom Mom Mom is never copied to the iPhone.
Can somebody please tell me how to get this orphan ringtone back on my iPhone? What does the weird circle mean? I've gone to the iTunes store on my phone and also in the iTunes app on my computer and in both places when I view the ringtone in the store, it's acting like I never purchased it. It's not like $1.29 is so much that I wouldn't pay for the ringtone again, but honestly I don't trust that the ringtone won't be orphaned again and again, and I'm not just going to keep buying it over and over. HELP!

This happened to me to.
It took me a while to figure out a solution, but this may or may not work for you.
Long story short, previous versions of iTunes used the Ringtones folder to store all tones, but now iTunes uses the Tones folder.
(I had copied my library of music from one computer to another, where the old computer used an older version of iTunes. I assume most users aren't affected by this, because they normally just upgrade iTunes on their primary computer, and the upgrade process probably takes care of renaming the Ringtones folder to Tones)
For example, my media folder is set to be:
C:\Users\vincetg2\Music
so all my old tones files were in:
C:\Users\vincetg2\Music\Ringtones
but iTunes was expecting them to be in:
C:\Users\vincetg2\Music\Tones
To solve this, I moved all the files in the 'old' Ringtones folder to the 'new' Tones folder (you should create it if it's not already there):
then dragged the files into iTunes' Tones section:
and then synced (erase and sync) the tones over to my iPhone:

This means that:
I can not verify the iCloud username because it is a non-excisting email with no alternative e-mail adress
I can not delete the iCloud account because I do not have a password
I can not use forgot password because the iCloud-ID is a non excisting e-mail adress
I can not turn off "find iPhone" because the iCloud username is not verified
My Appel-ID is different from the icloud-ID with only one letter after the @ prefix, and that is the reason for this whole mess. I was supposed to type my Appel-ID as my iCloud-ID, but mistyped and approved the "new" and wrong iCloud-ID. During the last IOS update I activated the "find iPhone" functionality as a part of the installation procedure. The result is listed in the list above.
Any suggestions on how to solve this?

Without knowing all of the information we can only guess .... And my guess would be that you did not have the apps selected to sync from iTunes on your computer. If the apps are in iTunes on you computer, select them to sync and sync again.
Connect the iPad and sync again - if you auto sync. Then after the iPad syncs the first time - leave it connected to the computer and select the iPad from the left sidebar under the devices heading. Click on the Apps Tab in iTunes. Select each app (click on the boxes next to the names), then click on the Apply button in the lower right corner of iTunes. Make sure that the Sync Apps heading is checked as well.
If the apps are not in your iTunes library - if you had never transferred the purchases - you will have to download them again via the purchased tab in the App Store on your iPad. Make sure that you use the same ID to redownload the apps that you used to buy them with.

Airplay dropouts - only on some tracks in iTunes and not from iPhone

I experience heavy Airplay dropouts on certain tracks in my collection. And when I play the exact same track from my iPhone4 there is no problem. So I expect it to be an issue with iTunes and not a network bandwidth issue.
What I noticed is that it occur more often on tracks with a high bitrate than on tracks with low bitrate - but not always. Almost all my tracks is 320 kbps mp3.
However I've also experienced the problem on AAC tracks purchased from iTunes Store.
And finally some podcasts play completely without any dropout, and others drop out after a few seconds.
The above scenarios are easy reproduceable, and I think it might be an iTunes time specific problem when decoding the file and filling the airplay buffer.
I don't think it's a general computer resource problem, as the max CPU usage is approximately 2-3% and 0-1% most of the time. There is also plenty of memory available.
Are anybody else experiencing the same problem? Or maybe has a solution for the problem?

Dell E6510
CPU Intel core I7
Windows 7 64 bit.
iTunes 10.2.0.34

At this point, considering that the primary issue is with getting the Windows update resolved, we need to get that taken care of first. To do so you may need to consult Microsoft, or whoever supports the installation of Windows on your computer, for more troubleshooting to resolve it.

For some reason, the Camera & Scanner Wizard has started to detect the phone now. Maybe it will stop again in the future, but for now it's OK.
I do have iTunes installed, and the iPhone's still not appearing in Windows Explorer. I might reinstall if the problem recurs.
Yep, it's an official Apple cable, but it's a bit old. I got it with my iPod a few years back. Maybe i should start using that as the charger cable, and the new cable i got with my iPhone for syncing?

After updating my phone to the iOS 6.0 I can't find the tones I downloaded (to the phone), even though iTunes shows them in the "Tones" page and they even appear on the "On this iPhone" page in iTunes...but they are not in the tones list on the phone. Where are they?

i just ran the diagnostic in itunes and it says that itunes doesn't find any usb ports. they're clearly there and working. thought i'd add the info just to see if anyone had a clue as to what was going on.

Ok, so after my old dinosaur computer finally finished updating and restoring my iphone4 with the new software, everything seems ok with the exception of about 20 ringtones that didn't get restored over. So I go to replug in my phone and I get an error message saying "This iphone cannot be used because the Apple Mobile Device service has not been started."

Envision99 is nearly correct.
First thing to try is to re-install iTunes (download it HERE and run the installer) - but, there's no need to uninstall it first. If that does not correct the problem, you may need to remove and reinstall AMDS, as described in this linked kbase article.

Playlists, not checked on iTunes, showing up on iPhone after installing IOS 7.2

My playists, which are not checked, are being synced to my iPhone. I can't figure out how to delete from phone as I do not want them on my iPhone. One playlist has been synced up to my phone but one has not. How do I get rid of the playlist on my phone? How do I get them to not be synced up to my iPhone? The playlist has no checks in the boxes to be synced up. I can't delete manually from my phone.

I wasn't suggesting iTunes Match as a solution, but as a symptom. If you're not a subscriber then make sure you have not turned this on by mistake (Settings/Music).
If that's not it, is your iPhone set to auto-sync to your iTunes library, or do you manually manage music by dragging and dropping songs to your iPhone from iTunes?
If you autosync, are you syncing your entire library? If you don't want to do that, change your sync settings on the Music tab while your iPhone is connected to iTunes. On that tab you can auto-sync to specific playlists, artists, genres, or albums, any combination. That way you don't get things installed that you don't want.
If you manually manage music, connect the iPhone to iTunes, select the iPhone name in the left column, expand its contents by clicking the small triangle to the left of the iPhone name, click on the Music category to expand it, then delete the playlist(s) and/or songs that you don't want on the iPhone.

In Safari, (I use Firefox only) cookies must be enabled. I never knew there was a connection between iTunes and Safari until now.

I dropped my iphone 4s in water yesterday. Got out my old 3g to use temporarily. I synced it to itunes and restored from backup. However, none of my pictures or text messages transferred to the 3g. Did I do something wrong? Why won't those transfer? I had just synced my 4s over the weekend. Everything I have read says to just hit restore and it should have restored to what was on my 4s. Can someone please tell me if I've lost those things forever?

If you got a replacement iPhone 4s or iPhone 5 you could restore the backup of the damaged 4s device to either device.

I backed up my iMac using Time Machine using an external drive, after purchasing a MacBook Pro. I now want to move my iTunes and iPhoto from this iMac time machine backup to my new MacBook, but I can't get past the "you can't open the application because it is in a time machine backup". I attempted to migration assistant and it said it moved all of my applications over, but the songs and photos from my old iTunes and iPhoto didn't transfer. There are separate sections for iphoto and itunes.
It has to be so complicated because Apple made TM to do too much and be too many things to too many people.. a horse designed by a committee. It has become flakey in later editions.
If you still have the iMac you can move the files directly from the source.. you don't need to use TM backups at all.

I am having the same problem, i edited everything multiple times, sometime my album artwork will show up and then disappear after time. I have tried everything, even wiping my ipads music library and changing each file to AAC format, the artwork remains in my itunes when i pull up a song but as soon as i transfer the music to my ipad, everything loads successfully...EXCEPT the album artwork. Really annoying!! I really wish i could find an answer before I load all of my music back on and just give up on the search for answers,

Well then it is somewhere on one of your Home pages or in a Folder:
Do a spotlight search to see if it accidently got moved into a Folder: Swipe down in the center of the screen to bring up the Spotlight search and type in iTunes. If it is in a folder, the folder name will show up to the right.
If it is not in a folder, but shows up on the Spotlight search, then you need to look at each Home screen to see if you can find where it got moved to.
Finally, you can go to Settings>General>Reset>Reset Home Screen Layout, which will reorganize all of your icons so that the original Apple core apps are on the first Home screen as they were originally. Of course, if you have a lot of app icons that you have set up on specific Home screens you would have to move them all back to where you wanted them. But that should put the iTunes icon back on the first Home screen.
