Windows 8.1 upgrade fails on Inspiron 660
Attempting Windows 8.1 upgrade results in:
"We couldn't update the system reserved partition"
with event ID 20 from WindowsUpdateCilent for error 0xC1900104 when installing "English ESD bundle parent".
Googling this I find this is a problem for some systems with a system reserved partition that's 100 MB.
But, this machine with original Dell Win 8 install has nothing called that and no partition with that name or that small. It has:
2 GB Recovery Partition
500MB EFI System partition
(the balance) OS partition
7.32GB Recovery partition
The EFI system partition is protected - I can't add a drive letter to it to see what's going on there.
I read and insured that the requirements of http://www.dell.com/support/Article/us/en/04/643934/EN were met. KB2871389 was listed as "failed" under update history, so I presume that means "not installed". Antivirus was disabled prior to starting the download.

We can try to clear update temp files to check the issue:
1. Click the Start Button and type in "Notepad" (without the quotation marks) in the open box and press Enter.
2. Copy the following commands and paste them into the opened Notepad window:
net stop wuauserv
net stop Cryptsvc
ren %windir%\SoftwareDistribution sdold.old
ren %windir%\system32\catroot2 crt2old.old
net start wuauserv
net start Cryptsvc
3. After pasting the above commands, please close the Notepad window. Then choose "Save" when you are prompted to save the file. Type "temp.bat" (without the quotation marks) as the file name and choose Desktop from the left panel as the location.
4. Right click the temp.bat file on Desktop and choose Run as administrator.
5. A DOS-like window will begin processing.
If the issue still persists, please run System file checker to repair system: Open command prompt by run as admin and type following command:
SFC /SCANNOW
Any error message, please post back.

Windows 10 upgrade fail - black screen
Hi - installed windows 10 via upgrade - failed part way through with the error message "something happened". Downloaded the install ISO for usb drive - did a clean install from that (including format HD) - win10 installed ok, then 10 min later screen went black. I left the pc for several hours - nothing happened so I did a hard reset. Now when I turn on it powers up, passes bios message, then hangs on black screen. If I hard reset again the windows diagnostic runs but finds no solution - I can boot in to Safe Mode but when restarting again goes back to black screen. I've tried plugging in an external monitor to the hdmi -no luck. Have tried clean reinstall several times- no luck. Any suggestions?
If I don't install the graphics drivers, Windows Update automatically downloads and installs drivers which then cause the black screen. The only solution I have so far is to set my wifi connection as "metered" so it doesn't download and install updates automatically. I can install other updates and drivers manually, but this isn't ideal.

On Windows 7 PC, got the "upgrade iTunes" notice this morning, said sure, but the upgrade failed, now I can't access iTunes "invalid permissions"
I wasn't paying attention so I don't know the precise error I got on the upgrade failure.
All other iTunes upgrades have been fine...no brainers. Done several on this W7 PC.
If I re-download and install a fresh iTunes I assume I will lose my library...true? Can I find and save off my library so I don't have to "discover" all my music which is scattered all over the PC and a 1T attached USB drive.
Or is there some way to get the iTunes upgrade to successfully restart and install correctly?

Check for .dll files
Go to C:\Program Files (x86)\iTunes and C:\Program Files\iTunes and look for .dll files.
If you find QTMovie.DLL, or any other .dll files, move them to the desktop.
Reboot your computer.
Note: Depending on your operating system, you may only have one of the listed paths.
Uninstall and reinstall iTunes
Uninstall iTunes and all of its related components.
Reboot your computer. If you can't uninstall a piece of Apple software, try using the Microsoft Program Install and Uninstall Utility.
Re-download and reinstall iTunes 11.1.4.

Windows 10 Upgrade Failed with error code 80004005
OK, I have downloaded Windows 10 Upgrade and it all goes well until the computer restarts. Once it boots up again, the upgrade does not continue and I just go back to Windows 8.1. I then see error code # 80004005 for the upgrade in the Windows Update history. I do not know what is going on. I read on HP's Notebook upgrade page I found the following:For computers with available storage of 128 GB or less, the upgrade might require you to provide a USB hard drive (32 GB or larger). HP does not recommend using an SD card.If your computer has less than 128 GB of free hard drive space you will be prompted to connect a USB hard drive to continue with the installation. Use a blank USB hard drive 32 GB or larger. When the installation is complete, Windows 10 Upgrade will copy the windows.old file to the USB hard drive. Keep this file on the USB hard drive. It will be required to Go Back to the previous operating system if necessary.My computer definately fits in that criteria since it is equiped with a 128GB SSD. When I start the upgrade, I do not get prompted for an external hard drive. I have also tried the Media Creation Tool and ISO/USB and get the "Windows 10 Installation has Failed" error.
The system reserved partition serves a purpose. If it is deleted, then the OS will not boot back up. In which case you are toast.ON THIS PAGE, it tells what the partition is used for.-The Boot Manager and Boot Configuration Data : When your computer boots, the Windows Boot Manager starts up and reads the boot data from the Boot Configuration Data (BCD) Store. IPAM Upgrade Failed - Windows Server 2012 R2
Hi All,
I'm experiencing some problems with the upgrade procedure of an IPAM service hosted on a Windows Server 2012 R2 instance which originally was deployed on a non-R2 version. I've recently done in-place upgrade of the W2K12 OS to W2K12R2 which went absolutely
fine, however when I'm trying to upgrade the IPAM service itself (from v6.2.0.1 to v6.3.0.1), I'm receiving an error message right after the database schema update procedure.
Received error message:
Failed to convert Schema. Please see the details.
The given key was not present in the dictionary.
Please restart the Wizard to update the IPAM server.
Tried to restart the wizard several times, but it ends up with the same result.
Also I noticed that the event log shows some authentication problems with the WID DB service:

HP Envy dv7 - Windows 8.1 upgrade consistently fails
I have been trying for several weeks to upgrade my Envy dv7 laptop, and after spending several hours it eventually fails, popping up a message about failure 0xC1900101 - 0x40017. I am then forced to wait an extended time for this upgrade process to restore my previous version. As a result of this exercise I lose my laptop for 6-8 hours. Apart from the above message no useful information about exactly what failed is presented, which is infuriating because if I knew what was clobbering the upgrade I might be able to fix it - this laptop is rich with development tools and upgrades which might not be installed on a typical laptop, so I presume one of those might be causing problems. However all service packs and updates have been installed and unless Microsoft tell me what is causing the failure then I have zero chance of upgrading. The upgrade process goes thru a reboot and then a lengthy period with a 'Setting up' message gradually climbing to 100%. The failure occurs late in the process of 'Saving PC configuration'. From the appearance it looks as though the upgrade goes 99% of the way before failing.
I tried, and once again suffered an epic fail. I just wasted another 5 hours. The upgrade process went all the way through, then a failure was detected on boot up. It tried again, same result, so without giving me any choice it automatically installed my previous operating system. Then on boot up it shoved a big message in my face telling me how sorry they were that the upgrade failed. This is absolutely infuriating. I consider this upgrade process to be a monumental failure by Microsoft. Not because of the failure itself because I can accept things do fail - but of the complete lack of information provided about what caused the failure. Microsoft are not giving me any chance of being able to resolve this issue by understanding what is causing these failures. All I get is unhelpful messages saying FAILED. Why is it so difficult for the upgrade process to record a log file as it goes thru the process? What p's me off is that my Windows 8 continually pesters me to upgrade to Windows 8.1. When I try, it fails- treating me like the village idiot by withholding information about what is causing the failure. I've no idea why Microsoft would want to withhold such information. So far I have tried 3 times to do this upgrade, and probably spent the best part of 24 hours not being able to use my laptop whilst Microsoft's hopeless operating system upgrade fails. I now have a very expensive HP laptop which will shortly become a brick due to my not being able to upgrade the operating system - in due course Microsoft will withdraw support for Windows 8 and I won't be able to do any upgrades. Very unhappy user here.

SCSM 2010 to 2012 Management server upgrade failed
I upgrading my scsm 2010 to 2012 both Management and DW server.
So i have set up the secondary lab environment and do this.
The DW server has been upgraded successfully to 2012.
When i try to upgrade SCSM Management server, at the finalizing stage, i get this error as _RegisterExtensionsDeferred

We ran into this as well. One of two options:
1.) Just run a repair on the SQL Server installation
2.) Repair the WMI settings on the SQL Server as descibed here:

I see by your post that the USB isn't being recognized.
I can help you with this issue.
Are you using the same USB cable? (known good USB cable)
You want to use the normal USB slot 2.0 cable and not the latest USB 3.0.
USB 3.0 are supposed to be backwards compatible.
Try another USB port on the Laptop, some of the USB ports are more powered then others.
I have provided a document to see if the printer is partially recognized in the device manager, which is causing the printer not to be recognized again. (the uninstall from the CD should have removed the entry and the printer from the registry)
Leave the printer turned on and the USB cable connected directly to the computer and the printer's power cord directly into a wall outlet.
How do I get into Windows Device Manager?
Here is a document for Identifying problems in Windows Device Manager.
Check to see if there are any hardware driver conflicts, you might see a ! mark or a red x beside a device with a conflict or unknown device, look under universal serial bus controllers, look for USB printing support or mass storage device, delete the entry by right clicking and select uninstall, OK.
Let me know how it was listed. (screen shots welcome)
If this is a Laptop then check the power management settings.
Access the device manager. Windows 8-How do I access the Device Manager.
Select universal serial bus controllers from the list, for each USB root hub, right click and left click on properties, for power management remove the check mark for "Allow the computer to turn off this device to save power". It allows the computer to limit or turn off power for this device.
Disconnect the USB, run the full uninstall and reinstall the printer software again.

Unable to start after Windows 7 Pro upgrade, am I crazy?
I'm now on my fourth try and I'm about to lose my patience.
I purchased a T400 6475-B14 with Vista, 32 bit. I purchased a copy of Windows 7 Professional upgrade. I am installing the 64 bit disc. Currently the machine has 2GB of ram, but I ordered 4GB (2X2GB) of RAM from Lenovo and it should be here in a few days. Additionally, since the machine is still under warranty Lenovo is sending me the restore discs (I think that's what they are called) and they should be here in a few days as well.
On my three previous attempts I upgrade the OS and everything seems to be fine. However when I go to the Lenovo site it can't detect my system. That seems a bit odd, but I can deal with it. I put in all of the machine info and download the Toolbox. Once I did that tells me to download the System Updater, but for some reason it forwards me to the link for Vista/XP. I then manually find the System Updater, Win 7 64 bit, download and install it and it finds and installs the PowerManager, Connections, Recovery and Repair, etc.
I don't seem to have any problems until I shut down the machine. When I go to restart, the machine hangs at the 4 color Microsoft logo. After a few minutes (5-10) it gives me the option to try and restore to a previous point and fix the machine. However each time it tells me that the system is unable to repair itself. I then reboot and hit the F11 button at the THINKPAD screen and restore to a previous version.
Last night, after the two previous restorations failed to give me a stable machine I completely reinstalled the OS. Everything seemed to be working fine again. However this time I rebooted after each update, to see where the problem might be.
I had downloaded all of the System updater info except for the R & R and C++. I installed C++, but it didn't require a restart. I think installed the R & R, installed it and then machine rebooted.
Then the same thing happened again!!!!!!!!
I went into the diagnostics and it came the the following:
Root Cause Found:
A recent driver installation or upgrade may be preventing the system from starting.
Repair Action: System Restore
Result: Failed. Error code = 0x1f

Result: Failed. Error code = 0x490
Time taken = 945178ms
I'm going to re-install Windows 7 again, but this is starting to really drive me crazy (granted it's a short road, but still!).
Can anyone help me understand what I'm doing wrong? I'm not trying to do anything crazy and I'm following the instructions to the letter but I still can't seem to get my system to work!!!!!!

You are doing (trying to do) exactly what I did when I punted Vista (good riddance!) and updated to Win 7 Pro 64 on my T400. I don't see anything obvious that you are doing wrong. Here's what I did:
Installed Win 7 upgrade as a clean install. Then ran Windows update. Then installed ThinkVantage System Update for Win 7, and let it do its thing, No problems.
Is that the updater you installed?

SCOM 2012 R2 agent upgrade fails crippling agents
Running into a large amount of SCOM agents that are failing the upgrade from 2012 SP1 to R2 and would appreciate any feedback from my SCOM community colleagues. Warning this issue is not for the faint of heart.
Plan:
I am in the process of upgrading 1900 manually installed SCOM 2012 SP1 agents to R2. I am using SCCM to deliver the upgrade using the standard sanctioned upgrade parameters.

Problem:
I have run into a problem where on a larger group of systems, 165 servers, where the upgrade fails and leaves the agent in a crippled state. At this point the agent cannot be removed cleanly via add remove, nor can a straight install of the agent
be done. What is required is a manual removal of registry keys and then a clean install of the agent can be performed to remediate. This is no problem and I am able to do this. (Note: all other methods of agent removal did not work ie:
Cleanmom.exe utility or add remove)
Task:
Seeing that we need to do agent upgrades in the future it would be great to know why this happened so we can plan for this in the future.
Notes:
Seems like a random sample of servers (2003/2008) with different applications running on them so that doesn't help in narrowing things down.
Looking at the MSI log, I see a common issue among systems that had this problem.

Ok so the obvious is that the installer is looking for original source installation files and not able to find them. What is surprising to me however is that the product references a 32 bit scom agent guid
387306D9-78CE-4E0E-B952-28A50CC8B3EE, however this is a 64 bit machine. Our build process dictates that a 64 bit machine only receive a 64 bit SCOM agent.
Doing a search on this product guid I realized I skipped some other references at the top of the MSI log that might offer some more explanation...

Well that is interesting, seems to me that in the first line the MSI installer reads the OM_OM12_SP1_AGENT_FOUND property and identifies this with a 64 bit agent guid (8B21425D-02F3-4B80-88CE-8F79B320D330), but then appends
the 32 bit guid at the end (387306D9-78CE-4E0E-B952-28A50CC8B3EE). This is the point of my confusion and my suspicion as the cause of the problem I am having.
Concluding thoughts:
Why does the installer seems to first recognizes a 64 bit agent, but then later changes it's property to include a 32 bit agent guid. Could a 32 bit agent have got on this 64 bit server based on the details of this log? Would this
cause my agents to fail the upgrade? I think so, but looking to bounce this over to another fellow SCOM colleague who may have wrestled with this before.
