Work flow attachment

Question

Hi All,
I have one urgent requirment.
we are using the work flow.In the workflow process when ever a new vendor is going to create,we are attaching one W9 form to that pericular workflow and sending it for approvals.After all approvals according to the attachment it should come and create new vendor in the Vendor Master(XK01),if vendor is going to change then also it goes for approvals with change Document.But here the requirment is .
We need to attach w9 form to vendor master in XK01 automatically after all aproval process.
Kindly suggest me how to do this.This is very urgent,and i am new to workflow.
