XSERVE RAID Disk Failed to Recover After Reset...

Greetings,
I've got an PATA XSERVE RAID with 14x250GB Dries setup as RAID 5.
Last week event monitor reported that Disk 14 Reported An Error Command 0x35 ERROR 0x10 STATUS 0x51 LBA 0X3978B80.
Not having a spare Drive available, I ordered one and Reconditioned the Raid Using the RAID Admin Tool. 3 days later it reported all as well. This morning my new ADM arrived and before I could replace Drive 14, it started generating basically the same Error.
So I swapped in the New Drive, and it began to rebuild, now here's the kicker...
After a few minutes it started giving me this ERROR:
DISK 14 FAILED TO RECOVER AFTER RESET. RETRYING
then...
DATA WAS LOST DURING A REBUILD.
It continued to repeat these messages until it Finally reported:
DISK 14 OFFLINE
So, I thought it must be a bad Drive, So I figured let me kill 2 birds with 1 stone and swap out a drive I had working fine in an XSERVE. So I ejected the new drive from the XSERVE RAID and Replaced it directly with the Drive from the XSERVE (Also 250GB) And...
It did the Same exact thing, same errors, etc.
So what's going on?
RAID ADMIN reports all Components OK
Any help is appreciated.

OK, So replying to my own issues here...
So I've replaced the Lower Controller and updated the firmware on that Controller.
The I ejected disk 14, waited a minute or 2, then re-inserted the Disk, then made it available for use.
It attempted to add to the existing RAID and the errors started all over again (See 1st post)
So apparently it wasn't the controller.
So I figured it must be the RAID Set itself, so to be sure, I swapped Drive 8 with Drive 14.
Deleted the ARRAY and Re-Created it, it's building now and Now Drive 8 is offline, So it would appear to be the drive, but I already replaced it with a known working drive, a new one from Apple, and then one directly from my XServe (See 1st post)
So I'm just baffled at this point.

  • XServe RAID disks not showing up in Disk Utility

    I have a new (to me) xServe RAID box and I'm stuck, the stays lights are green the fiber link light is green on the RAID and not lit on the PCI card but shows up as green in the fiber link section of the settings tool but the RAID array doesn't show up in Disk Utility so I can't format the drives.
    I set up the disks as Enhanced JBOD using RAIDadmin and I've deleted and created it a couple of times, I'm just not sure what else I should try, I've read the manual but. Can't find any suggestions of what's going wrong.

    In the system Profiler I'm not sure what you're asking for, I'm running 10.5.8 standard not server.
    the fibre channel shows link established 2Gps and cable type copper but nothing else, the disks & RAID controller don't show up anywhere I've looked.
    RAID Admin shows the JBOD disks as online and lists the LUNs etc.
    System looks fine, only error is missing second power supply.

  • Hard disk bad sectors gone after reset to factory defaults.

    Hi,
    I have HP DV6 6c19tx. a couple of days ago i noticed extreme slow startup time in my laptop; like 20 minutes. Once logged in it took another 20-30 minutes to let me click anything. I openned the task manager but no process was taking extra ordnary memory or processor. I openned HP Support assistant to troubleshoot and it showd me errors in SMART Check. which means my HDD is dying but to bad sectors. I ran the Seagate tool for HDD analysis and it also showed errors in SDST. which confirmed me that my HDD is about to die. I took the laptop to the retaler as my laptop is still in warranty. from the BIOS diagnostics he tried to run the HDD tests and it failed showing some error numer 303 or 305. He asked me to take ackup of all the data so that he can order a replacement for HDD.
    I took the laptop back to home to take the backup. after copying all the data, i wanted to remove all personal contents from the laptop so i thought y not to reset it to factory defauls. I reset it t fatory defaults and unexpectedly the system started working perfectly fine. I ran all the test that previously showed error, they all were passed; no HDD error.
    Now am confused what happened here, some one please suggest me if i still need to give it to the retailer to claim the warranty. was is some kind of virus that got removed after reseting the sestem or what. 

    I know it's been a while since you posted your question, but I've recently had an issue with some SMART software claiming that my hard drive was going to fail.  After doing some research, I discovered that there's a virus that will do this to your computer.  It eventually causes you to lose access to all of your files and you'll be unable to use your hard drive.  
    My question on this is why in the world would this program exist?  There's nothing to be gained from creating a virus that just causes your hard drive to poop out.  I'd be willing to bet that it's something the manufacturer has created that forces either a new computer purchase or a new hard drive purchase.  It also could possibly be just some dirty butt psycho dreaming of ways to cause problems for people instead of getting a real life.
    In any case, it's a virus.  Your hard drive is probably fine now.  Re-initializing the hard drive most likely wiped it out.  

  • Xserve raid card failing? 3 x 1Tb drives

    Hi all
    I've had an xserve running for the last month or two now, it has been subject to frequent crashes...
    I have applied the latest 10.5.5 update and haven't had a crash for some time now.
    This morning I happened to have a look at the server monitor, and there was an error...
    I'm a little confused... especially when the xserve is also connected to an APC UPS unit.
    Should I be worried about this... is something lurking around the corner?

    This is covered in the RAID Utility User's Guide on page 12.
    Every 3 months the RAID card will discharge and recharge the battery. During this process, you will see the message.

  • New HP HDD load recovery disks fails to boot after install process on pressing continue

    I have Pavilion d7-4165dx notebook. I live out US, though made purchase from US (BestBuy).
    - HDD failed after renewing trial period of virus software.
    - All diagnostics would not recognize HDD. 
    - HP support indicated still in warranty, but I am out of country.  HP indicated most likely issue was HDD cable.
    - Purchase cables and had someone hand carry out of US.
    - Installed, still same response.
    - Ordered new HDD from HP.  Backordered.  Finally on inquiring was directed to a US 3rd part rep.  He recommended buying directly from outlet, but a higher speed.  Did that and then HP replacement HDD arrives.  Hand carry both out of US.
    - Install faster (slightly smaller) Seagate 500G 7200.  Started recovery with recovery disks (5 total that received from BestBuy, but not OEM looking), but is squawked as new HDD is smaller than original WD 640G 5400.   Put WD into #1 bay and Seagate into #2.
    - Start recovery disk process.  Go through entire recovery from factory process and on last disk, it says to not touch while PC boots on and off, etc.  Then says press Continue to continue.
    - When press continue, simply goes to black screen with "No boot device..."
    - Run other options, same message.
    - Ran diagnostics.  HDD all OK.
    - Called HP support.  He indicates I need to buy the HP recovery disks.  The ones I have (5) are not the right one, that there should only be 3!  They are as I received from BestBuy, but look like they were made there when BestBuy sets up the PC. 
    Any suggestions anyone?

    Sorry, I don't have an answer, but rather a similar issue.  I replaced a 320Gb - 5400 rpm drive that failed with a 750Gb - 7200 rpm drive and even though I have the factory restore disks, it won't recognize the hard drive and I too get the "No Boot device" message.  Unfortunately, the BIOS is not the best and it won't allow me to see if it actually recognizes the new hdd, which I suspect it doesn't.  Either that or I have a bad MB.
    Sytem information - HP G71-340US
    Processor - 2.20 GHz Intel Core2 Duo Processor T6600
    BIOS - InsydeH20 3.5
    Recovery manager *appears* to be installing W7 just fine, but it's a no go when it's time to actually boot to it.  Is this a mb issue or did I just buy the wrong hd?

  • Disk failing to mount after failed repair

    My g5 1.8dp is running 10.5.6. SInce I upgraded to Leopard from 10.4.11 it's been sluggish, quite unresponsive and programs are giving me the beachball a lot more often and for longer. I decided to run disk utility.
    It told me I needed to repair the disk after verification highlighted some errors with "nodes. I booted from my install DVD and ran disk utility from the disc. Repair failed and now I can't remount my startup disk.
    Any advice greatly received.

    While an exact copy of the error message you got from Disk Utility would have been helpful, your computer apparently has directory issues which Disk Utility cannot repair. Under conditions like these using a utility like Tech Tool Pro or Disk Warrior may be able to do the repairs DU can't.
    There is a possibility that the HDD itself may be failing. If the HDD is three years old or more, I would consider replacing it. In any case, I suggest that you take immediate steps to secure your data. If you do not have a current backup of your data, that should be your first priority. If you do not have or do not wish to purchase one of the utilities linked, you could consider backup and reinstall, possibly using Archive and Install or Erase and install.

  • IPod Classic Failing To Recover From Reset

    Hi folks
    Anyone else seen this? My new 160Gb classic (purchased new last month) behaves a bit differently to my old one (old 160gb from '07). It doesn't 'freeze' very often, but when it does, resetting does not solve the problem- it doesn't reboot, stays on the Apple screen, and you're left listening to its poor HD trying to get there, failing, and going back to the start (which, if it happens at work, means 8 hours of it until either you can get it home to restore, or its battery runs out. Can't be good for it!). With the old model, I had complete confidence that a reset would always bring it back.
    Just wondering if this is a known symptom of the most recent model, or if it's exclusive to mine and I should try to get it replaced. The HD is definitely noisier on this one, I can hear it trying to read quite regularly, and it doesn't fill me with confidence.

    If my 160GB Classic doesn't reset when I try it, I simply try it again, even if it appears to be stuck on the Apple logo. I give it a couple of minutes before trying the reset procedure again. (Apple recommend you try it at least three times before deciding that it won't reset.)
    I have had to do three resets occasionally. Next time it requires a reset, try it up to three tiems (or more if you're determined!), what happens?

  • Please help: fail to recover after system crash

    Hi all,
    I tried to start our directory server after system crash due to hardware failure. Error log shows:
    [17/Apr/2010:01:35:15 +0800] - WARNING<20488> - Backend Database - conn=-1 op=-1 msgId=-1 - Detected Disorderly Shutdown last time Directory Server was running, recovering database.
    However, the recover process seems never ends (I waited overnight...). While CPU usage of ns-slapd grows continuously, the server has no response (e.g. ldapsearch waits forever). I make copies of db before starting ns-slapd and thus I tried several attempts:
    1. start as it is.
    2. remove db/log* and start
    3. remove db/log* and db/__db* and start
    4. remove db/* except db/NetscapeRoot and start
    All attempts have the same result - recovery goes on continuously.
    Then I tried to remove db/* and run restoreconfig with an old configuration backup. Then ns-slapd can be started. However, when I do ldapsearch testing:
    ldapsearch -h localhost -v -D"cn=directory manager" -b "" -s base objectclass= namingContexts
    ldap_init( localhost, 389 )
    Bind Password:
    ldap_simple_bind_s: Invalid credentials
    I'm sure the password is correct, and also tried modifying nsslapd-rootpw in config/dse.ldif and corresponding userPassword in configuration file created by saveconfig before restoreconfig.
    Again, all attempts have the same result - invalid credentials. I can see BIND cn=directory manager attempts in logs/access, but also see following line in logs/errors:
    [17/Apr/2010:21:59:08 +0800] - INFORMATION - conn=-1 op=-1 msgId=-1 - get_entry: entry cn=directory manager not found (error 32)
    I'd like to know:
    * if there any other method to make recovery process complete successfully?
    * if there is anything missed/wrong in the restoreconfig process?
    I tried several days but can't make it. Would anyone please help?

    I hope you have a export of LDAP data backup in LDIF format, if so, please stop the instance and remove all the files from your db directory which includes .db3 and log.* and __db* and then do a ldif2db.
    Once import is complete, start the slapd process.

  • Low disk space never recovered after cleanup

    Hi all,
    We have a CPI 1.3 up and running but with this issue.
    Cisco Prime Network Control System
    Version : 1.3.0.20
    /dev/mapper/smosvg-optvol 200G  162G   28G  86% /opt
    I followed the procedure on cleaning up databases and files but It didn´t work. After doing that we started to get databases error on some navigation features.
    Steps I followed
    1.- Remove HA
    2.- ncs stop
    3.- ncs cleanup
    4.- ncs start --> after that we started with other database´s errors.
    once this I followed with a ncs reinitdb
    1.- ncs stop
    2.- ncs reinitdb
    3.- ncs start
    So, we are still having the same issue. 86% disk used, how we supose to cleanup the disk?
    ade # pwd
    /opt
    ade # du -sh oracle/
    128G    oracle/
    Any idea?

  • K8N Diamond Plus IDE Disk Does not recover after standby or hibernate

    Ok this is wierd after i suspend or hibernate windows does not see my IDE disk anymore. I am running bios 1.10. Note ide cd and dvd drives dont seem to be a problem.

    Have had any problems with the drive at startup not being detected?
    Check the system log for the type error you are getting for the failure?
    As all the other drives in your system SATA, the Western Digital older system, has a lot of hours on it?

      When submitting a problem, include a complete list of your system components; include part numbers, all Power Supply Voltages, and their output ratings.  It is almost impossible to estimate what your problem is without knowing something about it. 

  • Expanding xServe RAID Array - updated info sought.

    I don't want to have to reformat the RAID and I don't want to lose data because...
    1) The organization will suffer extended downtime.
    2) I don't have ready access to 3TB of empty storage to back up the entire contents of the RAID.

    My (familiar) situation.
    I inherited administration responsibilities for an xServe RAID using the latest firmware (1.5.1).
    The xServe is an intel xeon dual 2 GHz, running Mac OS X Server 10.6.8
    The RAID had six (sigh) drives in the left controller. None in the right.
    When I became admin, one of these six was dead. So I went on eBay and got three refurb ADM's.
    --one to replace the dead drive.
    --another to expand the RAID 5 to a full seven drives. (from 2.5T to 2.73T)
    --a third to put on the shelf as a spare.
    Now then.
    The replacement drive rebuilt beautifully. Happy 6 drive RAID.
    I put in the 7th drive and expanded. It took several days.
    I'm at the point of "merging" slices that gives everyone pause about losing data.
    Much discussion here about whether this can result in added capacity without destroying data relates to earlier versions of Mac OS X Server and G5 not Intel xServes. So that's a point that I think I could use some clarity on. Can Intel Macs can expand the file system with this slice-merging function without data loss whereas Power PC Macs cannot?
    Next, my RAID as I inherited it was one slice. That is to say, one volume. One partition. In RAID Admin under "Arrays and Drives" all the little drives display a [1]. They did when the RAID was degraded to 5 drives. They did when I added the 6th drive replacement. They do now, having used the Expansion function under "Advanced" to add the 7th drive.
    So, next question. WAS IT EVER SLICED?
    That seems to be a deal-breaker for merging slices without data loss.
    For this, I looked in RAID Admin to my Events tab which dates all the way back to 2008.
    I see exactly zero mentions of slicing anywhere. Come with me on a journey, won't you?

  • AFP and Xserve RAID very SLOOOOW

    Recently we have been having problems with AFP pegging the Xserve RAID activity lights. Xserve RAID Disk access is very slow.
    Machine Specs:
    Dual 2.3 GHZ G5 Xserve
    1GB RAM (recently downgraded because of memory problems after a power outage)
    1x80GB Xserve drive module
    3TB Xserve RAID 5
    Xserve RAID has been upgraded to 1.5 FW and has both ethernet connections connected.
    The whole story is we had a power outage last week. The servers all came back up fine. Throughout the day we noticed that the file server was acting strange. We checked server monitor and it said that RAM slot 3&4 had problems. We opened up System Profiler and it reported that modules 2&3 were only 256MB! (These are 512MB modules). We brought the machine down and pulled the modules. The machine is now at 1GB of memory with more coming. The next day the machine was incredibly slow and the RAIDs access lights were pegged. Also something was pulling all free memory into inactive memory. After trial and error I've found that if I turn off AFP the RAID is no longer pegged and the memory goes back to normal. I thought AFP was the problem and did a complete rebuild of the server last night. This morning the same thing is happening. Also the logs show nothing is wrong.
    All of our Macintosh users are Mobile accounts over AFP. We have around 60 Macintosh users. All of our PC users connect using SMB. We have around 20 PC users. All of our network users work off of the file shares.

    Check so that the Xserve RAID diskcache settings are not reset/off.
    I have had to take apart the RAID controllers and putting them back togheter again to alleviate slowness/"malfunction" making a couple of RAIDs operating like normal again.
    I use Helios Lantest http://www.helios.de/ and XRG to measure/stress disks locally and over the network to see if they are working correctly.

  • Had to replace Xserve RAID controller and now I get Xsan Admin: ERROR: duplicate LUN label RAID1-Left1

    I need some help, I had one of my five Xserve RAID boxes fail. The upper controller card needed to be replaced and so we replaced it. But now only one of my volumes is loading and I am seeing the following error in my Console.
    7/18/12 9:23:18.550 AM Xsan Admin: ERROR: duplicate LUN label RAID1-Left1
    This raid box holds three LUNs. the left is split into two with one being the metadata / Journal for the main volume and the 2nd being the same for my secondary volume. The right side of this box is the second volume while the remaining 4 RAID boxes are my main volume. Here is the rub, the main volume is using the left1 LUN and the secondary is left2. The left2 is the one that is not working and when I go into the LUN info the WWN number does not match the actual id on the raid box.

    Did you try turning it off and on again? I mean, the full SAN shutdown/startup: http://support.apple.com/kb/HT4027
    You're probably seeing "duplicate LUN labels" because the MDC has seen two LUNs with the same label but different serial numbers since it has been online. The serial number (which I think is based off the WWN) would have changed when the RAID controller changed. A reboot should fix that specific error message.
    If the volume still doesn't start after rebooting, then you would want to check the cvlog for that volume.

  • Xserve RAID: Server Monitor says degraded, disk utility says mirror FAILED.

    Xserve Quad Intel 4x2GH
    4096 GB RAM
    OS X Server 10.4.11
    Xserve RAID 7TB 14x 500 GB ADM via dual fiber channel, Mirrored, 3.5 TB array.
    Flashing yellow indicator light on Xserve this morning. Investigated and found that RAID Admin says everything is OK, all 14 drives normal.
    RAID Admin log says :"4000 1 Wed Apr 08 09:37:59 EDT 2009 Disk 2 Failed To Respond. Retrying. RETRY COUNT:0 LBA:0x1C700"
    Under the RAID tab, Disk utility says: "Mirrored RAID Set: Degraded" and "Disc2s3: FAILED"
    All LED's are solid green across the front of the ADMs.
    Since all my drives are reporting OK, could this be a problem with the Fiber Channel interfaces?
    Should I just backup (again), shut down and rebuild the mirror using Disk Utility?

    I am trying to migrate user data from a RAID - 1 slice from my old PowerMac G4 running OS X 10.4.11 to my new MacBook Pro running OS X 10.5.4 by reading a mirror RAID slice from my old Mac that is mounted in an external drive enclosure and connected by FireWire.
    The problem is that the new Mac cannot read the old Mac disk, it never shows up on the desktop, or comes up as available when trying to use Migration Assistant. Then, if I go into Disk Utility, the external RAID slice shows up briefly with a locked yellow padlock beside it, then I suddenly get the window shade of death, and have to hold down the power button to reset/restart my MacBook.
    Why can my new computer not read my old computers disk, and how can I change permissions, or make the old drive readable by the new mac.
    Regrettably I am away from the original Desktop, and cannot load the old drive back into it, connect the two machines and migrate that way. My only option for reaching my necessary data is to retrieve it off the drive I have with me.

  • Hard Drive dead? - Failed to Issue COM RESET successfully after 3 attempts.

    Hello, i think my 13" 2.53Ghz Unibody (mid2009) MBP's hard drive has had it's last spin. I was using it normally in bootcamp, when windows7 froze, so I rebooted holding the power button....windows wouldnt start any more...ok, so i tried to boot into OSX...stuck at the loading screen with the apple.
    Went into verbose mode and it stops at "Failed to issue COM RESET successfully after 3 attempts. Failing..." Google says this has to do with the hard drive.
    Did a extensive hardware test (boot + D), all OK, booted from install disk and tried to run disk utility, but it crashed when trying to locate the drives (didn't even list the drive on the right pane). Single User mode wont load either, will give the same COM RESET message. I am literally out of options here.
    Is there something i can do to either fix this or verify 100% that my hard disk is truly dead?
    I have a backup in timemachine so the data is ok, but the timing couldnt be worse, i have no time to fix this now and needed a working laptop, highly frustrating.

    Welcome to the Apple Forums!
    Your HDD is probably dead, since the error doesn't appear to indicate something wrong with the motherboard. Just buy a new 2.5" HDD. I don't know about prices in Brazil, but here in the US, a 1TB drive runs for around US$100. Also, check on eBay for people who upgraded their drive and are selling their Apple drive. Some will ship to Brazil, and you can score those for $50-100.
    I hope you backed up your hard drive. If not, you'll have to do the whole reinstallation process.
