Php contact form has acquired... something from the server?

Question

www.wrfinn.com/contact.php password/login fred
At the top is an email address - that I didn't put there. A
blank screen
shows the email address (if accessed directly as in the link)
followed by
the page load with the address on top.
It is nowhere in the site - that I can find.
Is it on the server? What on earth?

Thanks Osgood and seb - that did it. I have absolutely no
idea where it came
from. If it reappears I'll know what to do plus get on to the
host. I'd even
looked at the source but ignorance got in the way of working
it out.
