Search db from one page

Question

On my php page, I have a simple form with one textbox. Once
submitted the search results page will load with recordset
information based on the URL Parameter of the search textbox.
It works great if I filter the recordset by one value, say
username...
However, I would like to be able to search not just by one
field on my search page.
I would like another form on the search page:
Form 1: Search by username (this is what I had done aleady)
Form 2: Search by last name
How can I add another form that will use the same results
pages? Do I need to make another results page just for that form?!
I plan on building off of the results page with update record
behaviors, to name one.
What if I even decide to add another search form on the
search page?
What's the best way to go about this?

I now have one form with a few textboxes on them, each with
their own textfield name: username, lastname, user_ID...
On my search page I created a recordset with 3 colname
variables, their run-time values are $_GET['username'],
$_GET['lastname'], $_GET['user_ID']
My sql is: SELECT *
FROM members
WHERE username = colname or last = colname2 or user_ID =
colname3
This will allow me to search from the seach page assuming I
only have one textbox with information typed into it. It does work,
and I like that idea.
However, how can I modify this to search this way, and to
also search from any other textfields that are entered. So, search
by lastname (if typed in search box) and by username (if typed into
the search box)?
So only use the information that is typed into the search box
as part of the query.
