SQL Servver 2012

Question

I was in server management studio doing some work and I notice, but can't remember now what, but I lost some basic functionality like not being able to right click and get my context menu and this affected all programs. I did a restart and the problem didn't
go away but then it did some hours later?
So I try to add a record to a database and get a primary key violation error. I right click on the table and get the context menu to show Edit top 200 records and their is no record showing with that key, which should be the last record entered
but when I select top 1000 I can see the record that caused the problem
Just be clear in the edit top 200 the last record or record 200 I have no primary key 3389
In the select 1000 records, record 1000 is primary key 3389
Could some explain the problem here why these tables are out of sink
I check the record count in the program that runs this data and I get the correct record count so that record is being added
The problem started when I tried to add a record in the edit 200 in Management studio

"Edit 200" means, only the first 200 records will be loaded into the data grid for editing; so the record with PK = 3389 is maybe not in that list.
You should filter the result for the grid to that one record you want to edit.
