Our organization purchased ten Acrobat Pro XI - Student Teacher Edition. The side of the box says "STOP - Proof of eligibility MUST be provided..." but it does not offer a specific link or form that needs to be completed. We are a small organization and this software was purchased for our teaching staff. Please provide a link for assistance.

Best option is to use Adobe Reader (or Acrobat, of course) on a desktop computer (ie, not on a mobile device). Any other configuration is likely to be problematic.

How do I add a functional "submit button" to a pdf form in Adobe Acrobat Pro XI ? I created the pdf form in Adobe Forms Central. It's for an online Diet Questionnaire. After people complete the form I'd like them to click "SUBMIT" and the completed form will be emailed to me.

This can be a bit confusing because Acrobat 11 comes with the desktop app that allows you to create simple PDF forms without having a FormsCentral account. Some people find this helpful, but you need to understand that when you generate the PDF form, it is Reader-enabled by Acrobat. In order to edit the form further in Acrobat, you have to create a non-enabled copy of the form. You do this in Acrobat by opening the form and selecting: File > Save a Copy
and opening the copy. It is not opened automatically.
You can now add a button and set it up to submit by email, either using a "Submit a form" action or the submitForm JavaScript method. You can set it up to include just the form data or the entire PDF, and will want to use a mailto type URL. Submitting the form to the FormsCentral server has a number of important advantages over email (much more reliable, more secure, etc.), so you might want to consider it.
If the form needs to be saved with Reader versions prior to 11, then you will need to Reader-enable the document. In Acrobat 11 you do this by selecting: File > Save As Other > Reader Extended PDF > Enable More Tools

Not sure what you mean by that: is that URL in an image, or is it actual text?
If it is in an image, then you need to extract the image, edit it with something like Photoshop, then add it back to the PDF.
If the URL is actual text, I suggest that you remove the entire URL, then add the corrected link.

I have Windows 8.1, and when I right click on the Windows flag (bottom left), the menu gives me a "Search" option. When I enter "%temp%", it goes to " "my username"/ appdata/temp" which is a list of file 1718 file folders. Many are empty but I did a small sample. Is that the right place to delete? I tried Windows- Disk Clean, but that did not help.

I've created a form using Adobe Acrobat Pro, now I want to distribute the form but it keeps asking me to subscribe for the monthly service. Is this not included in the Pro version?

Formcentral service is not included in the perpetual license of Acrobat XI. It must either be purchased separately or it is included in the subscription version of Acrobat.

I have got a ADSL Broadband connection to the Internet from my ISP, [UK Online] that has a Cable to my DrayTek, Vigor2500v Router.
My router is not wireless. I have a Ethernet cable connected from the router to my G4 iBook, 800MHz. My iBook is AirPort Extreme ready and is without the AirPort Extreme Card. What is the different between AirPort Express and AirPort Extreme?

I have my current Router wired by a Ethernet cable to my G4 Desktop, also have a VoIP connection from my router. So, I want to keep this router and add the “AirPort Express/Extreme base station” to have the wireless connection to a iBook only.
If I understand you correctly, I can disabled the DHCP server on the “AirPort Express/Extreme base station” only, Because I need the DHCP for the connection to my G4 Desktop. Is this what you mean?
