Your Apple ID has been disabled - what to do?

I get the message "Your Apple ID has been disabled" trying to update apps on my iPad, iPhone or on iTunes on my Win7 PC.
Support requests submitted to the web site get CLOSED quickly with no helpful information or email to me about it being resolved.  Someone or something at Apple's end decided the case is resolved.  And, it isn't resolved.
The account disabling was a result of a charge-back of $99.99.  I disputed the charge to my bank and the bank refunded the amount to my credit card account.
I'm at the mercy of Apple Inc and nobody over there is apparently listening or interested.

I don't associate a credit card to my Apple account
I don't associate a PayPal account to my Apple account
Instead, I buy Apple Store credit (gift card) from Walmart.com.  After Walmart.com emails the gift card code, I enter the credit into my Apple account.  I buy small dollar amount gift cards.  These extra steps are inconvenient but has the following benefits:
limits "impulse" buying from the App Store, iTunes, etc.
removes a third party (bank or credit card or PayPal) from future disputes
The small dollar amount helps limit my potential loss.
