How do I create an electronic form

Is there a way, yet, to create an electronic form in Pages?

No. Other than to export as a.pdf and convert that to a form with Acrobat Pro.
Word's form ability relies on people having a comaptible version Word, which not everyone does and is bad practice in my view.
The Acrobat method at least lets you get the free Acrobat Reader to fill it in and it also works well with databases to collect the results filled in.

    Yes, I want to enable the client's relations to be able to click on a link
    in the pdf to automatically submit the pdf to his e-mail address.
    Now a purple bar shows up when a 'relation' opens the pdf form.
    In this bar is a button included for sending the form, but they have to
    confirm the adding of their reaction to the 'reactions-file'. This refers to
    the location where I authenticallly saved the file (on my hard disk).
    I can adjust the reply e-mail adress by temporarily adjusting my id in
    Acrobat preferences, but I don't know the future location of the reaction
    file.
    It's a difficult question, but it's based on the question: Is it possible to
    make a pdf form for somebody else; so they can send en receive the reactions
    on their moment and their wanted location?

    In Acrobat 11 you'd select: Tools > Forms > Edit
    to go into form editing mode. You'll see the tools that are available, thought it may prompt you to autodetect the fields. I usually don't do this, preferring to create them myself.

    You might need to follow these steps to convert word into fillable PDF form:
    Open the scanned word document
    Click File> Print> Adobe PDF
    A PDF file would be created that would automatically open in Acrobat XI pro
    At this point, you would have a basic PDF file which still needs to get converted into fillable form
    Choose "Tools> Text Recognition> In this File" to run OCR to make your scanned doc searchable.
    Now, go to "Tools> Forms> Create"
    Acrobat will ask some questions. On the first screen, make sure 'Use an existing file' is selected and then click Next.
    On the next screen, 'Use the current document' must be selected.
    You might need to edit few fields manually. Then, just click Preview button on the top and check how you PDF form appears.
    Try this out and let me know if you face any problem.

    I have a search.asp page which when submitted transfers to
    the results.asp page. In the search I would like to also create an

    Oh, I see. So is it Dreamweaver that wont let you create the

    Ask your host if they have a form-to-email script you can use.  Many commercial hosts have scripts on their servers which you can activate through your C- or Admin-panel.
    Failing that, there are some commercial products you can use.

    Godaddy hosts my website and i can not get any type of contact form to work! When I publish to my .mac account it works fine, using the HTML snippet, however when I publish the exact same file to the Godaddy server it doesn't even show up.     One reason why it would not show is if not all your files have been uploaded. How are you uploading?
    If you are using Cyberduck or Transmit, you can actually check that the relevant files have been uploaded to the server. If uploading that page with the form on it, then ensure that you upload both the html page, plus the files folder that goes with it.
    Are you using Windows or Linux hosting? Linux hosting is preferable for iWeb.
    If you use GoDaddy they have facilities for you to use FormMail, so you can upload this directly to your hosting space and then create your own form using html code in an html snippet and then upload. You have more options if you do it this way and you have more control. If you use a third party such as Wufoo, everything is stored on their servers and if their servers go down for any reason you have not workable form.

    There are three tables involved (PROJECT, STAFF, TASK)
    PROJECT(PROJECT_ID, PROJECT_NAME)

    There is a many to many relationship between PROJECT and STAFF. The intersection table is TASK.
    Issue
    I would like to create a tabular form for the TASK table. I would like to be able to click on a button (Add Row) to add a row to the existing tabular form. The row will have a LOV for the PROJECT_ID (showing PROJECT_NAME) and one for the STAFF_ID (showing STAFF_NAME) and a free form text field for the hours.
    I've tried a bunch of different ways but am unable to acheive this functionality. I must be doing something wrong. This sort of thing is trivial.
    Any guidance/suggestions are greatly appreciated.

    Adding a new surrogate key column to an existing table is not that big a deal.
    alter table t add (pk int primary key);
    create a before-insert row-level trigger to populate the key;Other than that, you are out of luck w.r.t the wizards. Yes, you can define a 2nd PK column in the wizard, but then the 2 PK columns becomes read-only (but you want to update them).
    You can write your own tabular form using htmldb_item APIs and your own after submit processes to process the updates. See the manual Tabular Form Howto in the documentation.

    I was asked by a client to create a form where a new patient can fill out a form electronically and send to a desired email. Also, the possibility of incorporating this into their website.
    After searching it seems creating a form in InDesign then bringing it into Acrobat Pro is the way to go. This seems to have a real "tech" feel from what I have seen. I am much more on the art side of things.
    Is this the best route to go? I want the design to function well but also look professioanl and pleasing. I have the CS4 Suite incase this alters someones answer.

    You have the right idea, and here are some pointers...
    The layout of the form can be accomplished in Indesign. Areas that you want text fields, include a rule in the form, this will be helpful in Acrobat. Check boxes can be drawn out in ID, but the form is cleaner when the the check box is created in Acrobat.
    Once the form layout is completed, export PDF, open in Acrobat and Start the Form Wizard. The Form Wizard does a decent job of recognizing where intended form fields will go, and create, label, and order them. Keep in mind that the process is not perfect, and editing will probaly be necessary.
    You will need to save a working PDF, and when you are finalized, you will need to save a version that has extended features enabled. Extended Features will allow recipient to save PDF with their data input.
    Recipients will need Reader 9 or higher to save input within PDF.
    Another question you may want to ask: Does your client plan to export data from form, and input into another system? This can be accomplished, however there are a lot of factors involved with how it would be handled.
