How do I sign a document in Adobe Reader w/electronic signature?

I'm trying to sign a tax form, its unclear from the Reader doc how to create a signature for use on the document.

Starting with the form you want to sign opened in Reader, select the Fill & Sign tool on the right side of the toolbar. This will open the right hand Fill & Sign panel. Next, open (expand) the Fill & Sign Tools pane.
There are three possible scenarios from this point...
The Place Signature tool is disabled (grayed out). This would be because the security setting on the file do not allow for changes.
The Place Signature tool is enabled, but when you select it and get the Place Signature dialog, the only option available is Use a certificate. This is because the document contains an existing signature field and the assumption is the document author requires a secure digital signature as opposed to electronic signature.
The Place Signature tool is enabled,and all of the radio buttons are enable on the Place Signature dialog.
Depending on which of the three scenarios you encounter drive what you can do next. Obviously with scenario 1 there is nothing else to do. With scenario 2 you have to have access to a valid digital ID to create a digital signature. With scenario 3 you have the option of creating either a digital signature using your digital ID, or you can create an electronic signature by placing a mark on the document. That mark can be your typed name, a signature you create with a mouse, or a scanned image of your wet-ink signature.
If you need to create a digital signature, then you need to have access to a digital ID. There are two options for procuring a digital ID; either you can buy one from a trusted Certificate Authority (CA) who will vet your identity, or, you can have Reader generated an untrusted digital ID for free. What you select to do depends on what the second party (you're the first party, the document recipient is the second party, and if a CA is involved they are the third party) requires, or more precisely, what agreement you and other party come to.

    I add files to my Adobe Reader on my iPad from Acrobat.com. They show up as recent and they disappear after a while.  Contrary to what I was told on this site earlier (items don't disappear in recents unless it is to full), my documents/files do disappear.  I was working on a document earlier and now 3 hours later ii's gone. I don't always have access to wireless enternet when I need to open the documents.  Also,when I download (or what ever it's called when you open them up on an iPad), the notes and highlights that I did on the document have disappeared. 

    I apologize for misunderstanding your comment. It's easy to misunderstand what we read here and often I find that if I just ask more questions people are frustrated so I erred on the side of providing information rather than witholding it and asking more questions. If you are frustrated I apologize, but being abusive doesn't help any of us =). Everyone in these forums who is trying to help is a volunteer. I happen to be paid by Adobe to build the Reader but I volunteer in the forums during my lunch hour or extending my day to answer questions. I'm doing my best.
    My understanding of our cache is limited, but it looks to me like our current Acrobat.com cache is 100 megabytes. We will start pushing files out of the cache from the least recently used forward until the cache size is less than 100MB. We don't currently have a way to explicitly mark Acrobat.com files to be saved locally. One way to achieve this would be to open the file from Adobe Reader into another app and then open it again in Adobe Reader which will place it in your Documents. Alternatively, you can use iTunes to add the files to your Documents from your Desktop after downloading them from Acrobat.com. I recognize that this is a little strange and will result in 2 copies of the document (one in the Acrobat.com cache and one in Documents) but I wanted to provide the option. I've emailed another engineer to verify my understanding of the cache and to see if she can provide a better workaround.

    You can sign a PDF using Adobe Reader 9 or X, but the document must be "Reader Extended" to enable this functionality, This can be done using ADEP\LiveCycle Reader Extensions (server product) or Acrobat Professional 9 (Advanced > Extend Features in Adobe Reader) or Acrobat X Professional (File > Save As > Reader extended PDF)

    I have just downloaded the newest upgrade for Adobe reader and I am not able to sign or edit documents in Adobe Reader.     Can you check (using regedit) if you have the following key:
    HKEY_LOCAL_MACHINE\SOFTWARE\Policies\Adobe\Acrobat Reader\11.0\FeatureLockDown\cServices
    and if it contains a value bEnableSignPane ?  If so, what is it set to?
