I did have spotify premium on my phone, it stopped working so I deleted it from my iphone 5, when I tried reinstalling the app the 'open' tab is no longer available instead there is a picture of a cloud with an arrow coming down from it. Can anyone suggest how I might reinstall the spotify app onto my iphone again?

The 'open' button only show after you've installed it on your phone, the cloud symbol imdicates that the app is only in the cloud - if you tap on the cloud icon it should download onto your phone.

2 - the iLife DVD that you purchased
or
3 - the app store - log in under the Apple id used to purchase the software or to set up the computer - look under purchases - you might have to unhide or accept the Application first

How do I download an item with a cloud with a down arrow next to it?

I purchased an app, but all I get is a cloud with a down arrow. When i click on the cloud I'm told I've purchased the item and it will download now. Then the silly cloud appears again......

You do not need to delete it as it is not there. It is only in iCloud.
You can go to settings and turn off the Show All music selection, if you do not want to see the songs that are not on your device, but available via iCloud.

What does the little cloud with a down arrow next to a song mean

I upgraded my phone from iOS6 to iOS7 and had to remove all my music to get enough space which is fine because I wanted to reload it anyways. When I opened the music app, thinking there would be no music there, I was surprised to fine hundreds of songs each with a little cloud with a down arrow next to them. What does the little cloud with the down arrow mean? If I click on it am I purchasing something? Are they songs I can listen to for free? I don't use and have never signed up for an iCloud account so they can't be songs I purchased because I haven't purchased any songs.

Means they are stored in the cloud and you need to download them.

I just bought a song on my iPhone 5C and synced it to the mac. Now when I try to play the song on my phone it has a cloud with a red down arrow and when I tap it, it doesn't down load. I don't know what to do?

If it has a cloud icon it means its no longer on your device. Tapping on the cloud will effectively reinstall the App from scratch to your device.
There is no way to remove it from the cloud because its not yours to remove from there. Its the general App repository, you are just given access to it to download content you've already purchased.

It sounds like you are unable to connect to FaceTime on your Mac. I would use these troubleshooting steps from the article named:
