Start here (change country if necessary) and navigate to 'Password and Security', reset your security questions using the link provided, you will receive an email to your rescue address, use the link in the email and reset your security questions.
If that doesn't help, you don't receive a reset email or you don't have a rescue address, you should contact AppleCare who will initially try to assist you with a reset email or if unsuccessful will pass you to the security team to reset your security questions for you.
If you are in a region that doesn't have international telephone support try contacting Apple through iTunes Store Support.

On your browser, whether Safari, Google Chrome or otherwise, navigate tohttps://appleid.apple.com.
Once here click Manage your Apple ID button and sign into your iCloud account. Here, on the left side select Password and Security and this will list options on the right side. You should be able to see an option to reset your security questions.

If you cannot see or reset your security question, or even obtaining an error, then you would have to contact Apple directly: http://support.apple.com/kb/HE57. To be honest it was a secondary account to use the US store and since I was just downloading free apps I didn't pay attention to security questions, I never thought I would make a purchase until I got a gift card but it's asking me to answer them and I have no idea, I don't have a rescue email to reset them either (and to add one you must answer the questions.. doesn't make any sense for me) What do I do?

If you have a rescue email address (which is not the same thing as an alternate email address) set up on your account then you can try going to https://appleid.apple.com/ and click 'Manage your Apple ID' on the right-hand side of that page and log into your account. Then click on 'Password and Security' on the left-hand side of that page and on the right-hand side you might see an option to send security question reset info to your rescue email address.

If you don't have a rescue email address set up then go to Express Lane and select 'iTunes' from the list of 'products' in the middle of the screen.
Then select 'iTunes Store', and on the next screen select 'Account Management'
Next choose 'iTunes Store Account Questions' or 'iTunes Store account security' (it appears to vary by country) and fill in that you'd like your security questions/answers reset.
You should get an email reply within about 24 hours (and check your Spam folder as well as your Inbox).
