When she first got the ipod touch, we did not need to supply credit card information for her to set up her account. Only now that I entered it for one purchase, I can't remove it. Now my daughter has purchased over $300 in downloads the last two weeks.

You definitely can.
To remove your payment information open iTunes and go into your account. An easy way to access your account is from quick links on the store home page. After you login there will be a payment type listed with an edit button next to it. Click edit then select none under card type (next to visa, mastercard, etc).
Afterwards at the bottom of the page select done, then your card will no longer be on file. It will prompt you for a card next time you try to download something that isn't free.

When the regular customer service reps fail, try one of these:Send an email to the team at "[email protected]". Include:

    Service address
    Best contact phone number and best time to call
    A description of the problem

If you are doing it on your iOS device then try tapping on your id in Settings > iTunes & App Store and selecting 'View Apple ID' on the popup - that should also give you a payments link on your account's page.

You'll want to get in touch with ITunes support for this. Normally when you start an account with the CC you can't remove it yourself. But by contacting iTunes they can get that taken off for you.

Edit: Sorry I didn't reply back sooner. You must go on to iTunes using a computer not an iPhone or iPod. If you had an form of gift card (other than the iTunes gift card itself) you can't change your information unless you actually put a legit credit card number in. I had this problem before where I was 15 and put a Visa gift card as a credit card on iTunes and then it wouldn't let me edit it. I bought $30 worth of music using the card and never got charged for it. Anyway, this message would pop up every time I wanted to download something saying I needed to update. Eventually I couldn't do anything on that account anymore so I made a new one. Making the new one was really hard in the beginning because I they have it set up so you have to enter a card number in the new account sign up area.
I got to make my new account without a credit card because what you have to do is
-go on to iTunes
-go to the store
-look to the right and find the "quick links" area.
-scroll down past all the top songs, albums, movies ect until you get to "free apps" (make sure its the free ones so it can actually work)
-without being logged in to any account hit download for a free app
-iTunes will ask you if you want to make a new account or sign in, choose new account
-once in new account you will be asked what email and password you want along with other security stuff.
-when you get to billing there will be the place for credit card information and then all the way at the end of the list of credit card brands will be the option labeled "none". Click it and fill out the rest of the form and get your new account without credit card information

I have a PAYG subscription and have added a second credit card for payment. I would now like to remove the first credit card but there is no option to do this. How can I remove a credit credit card?

The card cannot be removed until it is associated as a default payment method to the MSDN Visual Studio Premium Subscription ID:
xxxxxxxxxxxxxxxxxxxxx. So first you need to add another company credit card to the Azure Account and Subscription by following the steps below:
Steps to add a new credit card
Login in to https://account.windowsazure.com
using the account owner Live ID.
Click on “Account” tab.
Now click on the “Subscriptions” tab.
Select the subscription that you want to edit.
Click on ‘Change Payment Method’
Click on '+' sign next to Credit Card to add a new card.
Enter the Credit Card Number, Name on Card, Expiration Date and CVV number
Click on Next to complete the process.      
Once the new card is added then you need to remove your card from the Azure Account
Login to commerce.microsoft.com
using your account administrator credentials.
Select Payment Options.
Select the credit card which you want to remove and click on
remove.

Open iTunes and select iTune Store on the left pane.
On the navigation bar, click on “Sign in”.
After logging in click on the arrow right beside your email address, and select “Account”.
Click on the “Edit Payment Information Button”.
You can now set the Credit Card option to “None”.
Click “Done” and the Payment Information box should show “No credit card on file”.

Tap your Apple ID. (If you aren't signed in, enter your Apple ID and password and tap Sign In.)
Tap View Apple ID.
Enter your Apple ID password.
In the Edit section, tap Payment Information.
Update the information that you want to change.
Note: You can find the payment methods that the iTunes Store accepts in the Payment Type section. If you don't want a payment method on your account, select None in the Payment Type section.
