Hi Steven,
Is the headset discoverable? The manual for the bluetooth headset should show you how to make it discoverable.
Does the iPhone see the headset?
This article: http://support.apple.com/kb/HT1838 will walk you through the process of pairing the iPhone with bluetooth headsets.

HT204368 Iphone won't pair with any bluetooth devices
Iphone won't pair with any bluetooth speakers, earbuds or car stereo. The devices pair with other phones computers etc. Jawbone Jambox says it's paired with Iphone but Iphone still says not paired! what's wrong? Iphone also doesn't ring or signal text or reminders just vibrates.
did that already twice. no pairings listed until I turn on bluetooth device then it just say not paired and pairing not successful. Jambox says it's paired but Iphone says not paired. Plus the ringer problem. When I choose a ring tone or sound it plays but when the I get an incoming call or text it just vibrates. And sometimes Itunes and youtube has no sound. What's going on?

MacBook Pro will not pair with any Bluetooth device
Last night my MacBook Pro stopped pairing with any bluetooth device (I've tried Apple Pro Wireless Keyboard, Wireless Mighty Mouse, Logitech MX900 mouse, Apple Aluminum Wireless Keyboard) and it simply won't pair with anything.
It sees each device but when it tries to pair it fails. With the keyboards, sometimes it'll give me the passkey to type on the keyboard but then it doesn't recognize that I've actually typed it and hit enter. Eventually I just end up with:
"The pairing attempt was unsuccessful. Make sure your keyboard is in range of this computer, turned on and "discoverable." When ready, click Continue to try again."
I've reinstalled 10.5.6 Combo update, tried to pair under a different user account, turned off/on bluetooth, and still nothing.
I did install VMWare Fusion last night and briefly shared the Bluetooth connection to that, so I'm not sure if that has anything to do with it or not.
Any suggestions?Me too. sigh
All was well until this afternoon, when I swapped out the batteries on my trusty old Microsoft IntelliMouse Explorer for Bluetooth, which has worked flawlessly for me for a few years now. After I put new batteries in, my MBP wouldn't recognize it. I went through the Bluetooth Setup Assistant, and it "saw" the mouse and properly listed it as "Microsoft Mouse". I hit "continue", of course, and the Assistant said "Pairing with your Bluetooth mouse". It got as far as "Connection established" (and I saw the mouse appear in the Bluetooth preference pane", but then I got the dreaded red words: "The pairing attempt was unsuccessful. Make sure your mouse is in range of this computer, turned on and "discoverable." Etc., etc. I made sure of all of those things and tried again, many times, with the same failure every time. I tried different batteries, rebooted the Mac, and so on. No luck.
Figuring my mouse was dead, I went out and bought a Kensington Slimblade Bluetooth Presenter Mouse, brand new, never been paired with anything. Same problem!
Also, I can no longer get my Palm Treo 650 to sync via Bluetooth. It used to be a little flaky (often complained that the port was in use by another application), but rebooting the Treo always solved that. Now, it doesn't.
One trusty old mouse suddenly stops pairing, one brand-new one refuses to pair, smartphone won't sync any more. I haven't installed any system updates or anything else since the Microsoft mouse worked. The only weird thing going on is that I'm having massive network connectivity issues with my broadband connection (probably a flaky cable modem), and a couple of days ago I bought a new Airport Extreme a/b/g/n wireless router, which is at the other end of the house from my computer and now-defunct mice.
Very bizarre. Can the Bluetooth chip or antenna in a MacBook Pro burn out or otherwise fail?
Puzzled and frustrated,

My iPhone will not pair with any devices on bluetooth, help?
I can't seem to pair my iPhone 4 with any other phones or devices.
PAISLEYSAINT
I have a THB Bury car phone cradle specific to the iPhone 4 which was working fine until I updated to iOS 5.1.1 - my phone will no longer pair with it or recognise it or any other Bluetooth device !
I carried out a full reset and relaunched the phone as if it were new - no success
I took the phone to the appple store and one of the Geniuses looked at it for me. He told me it was not designed to 'discover' other BT devices after he paired it with his MacBook but could not pair the devices in the opposite direction - i.e my phone to the MacBook. I thought this strange that the premier smart phone on the market should not have this function especially when it previously worked well.
He also carried out a full reset which is deeper than the one you can do at home - no success.
The genius would not accept that this was a fault and claimed not to be aware of any issues but offered to have it repaired for around £116 !!!
Incidentally my phone is just outside its warranty and his phone would not pair up either when I asked him to check.
THis is useless to me !! I understand Apple don't want to support BT function that allows it customers to send files between devices but come on ! My £90 BT car cradle kit is now useless and cannot be paired via my iPhone.
I now really have to consider using a smart phone that will work.
I love apple stuff and currently own no less than four different devices and was considering a new mac but not now. I feel let down by apple.
DIssapointed !:-(

Does the Lumia 822 work with ANY bluetooth headset...
I've got four different Samsung bluetooth headsets, and they all work great with other phones. I canNOT get them to work with my nokia though. They connect fine and seem to work, but once I'm on the phone for say, 45 minutes or so, they just stop working. The call is still connected, but its like the connection to the headset is lost. It THINKS its still connected, but I can't hear the other party and they can't hear me.
The phone needs to be restarted in order to get it working again. I sent the phone back to verizon, but the replacement is doing the exact same thing.hi mate,
the Portico update that has been announced to have rolled out to devices around the world as of last week, has addressed a lot of Bluetooth issues. you may wait for this, and contact your carrier Verizon, to see if they can give you a firmer date for when the update will be released to the public. -
My iPhone will not pair with Yada Bluetooth speakerphone
My iPhone 5 (iOS 8.1.1) will not pair with a Yada Bluetooth car speakerphone. (Android phone has no problem.) Is there a fix?
ThanksHello grassyknap,
For most issues with pairing your iPhone with a Bluetooth device like your car, then take a look at the article below to sort you out. Make sure to reset network settings to clear it out from your frequent pairings to reestablish the connection.

Bluetooth freezing up when trying to pair with a bluetooth headset
I recently got a Plantronics Voyager 520 and whenever I try to connect it with my MBP it like freezes, not the whole computer though. Whenever I move the mouse into the top right corner (where the time and stuff is) it does the spinning thingy to show that its working. Also, system preferences stops working as well.
I have a brand new macbook pro model that I purchased in June. I was hoping to be able to use my headset with it but I have no idea why it isn't working.
Oh, and I paired the headset with computer fine. But whenever I try to connect them after that it doesn't work.
Any help is greatly appreciated.
ThanksIf the error persists consider capturing a user mode dump of the crash to see if perhaps another, possibly third party, module may actually be causing the crash.
You can configure Windows to create user-mode dumps. Create a System Restore Point first.
If you copy and paste the following in Notepad and save as a .reg file (save with the .reg extension and give it any name such as explorerdump.reg) then right-click the .reg file and select "Merge" to add to the registry, a .dmp file at the time
explorer.exe crashes should be created in the C:\CrashDumps folder:
Windows Registry Editor Version 5.00
[HKEY_LOCAL_MACHINE\SOFTWARE\Microsoft\Windows\Windows Error Reporting\LocalDumps\explorer.exe]
"DumpFolder"=hex(2):43,00,3a,00,5c,00,43,00,72,00,61,00,73,00,68,00,44,00,75,\
00,6d,00,70,00,73,00,00,00
After Explorer (explorer.exe) crashes, can you then make the .dmp file available (provide link) via a public folder on Windows Live SkyDrive or similar site.
Here's a couple of links on using OneDrive (SkyDrive):

Can't pair Logitech FreePulse Bluetooth headset
It refuses point blank to work.
I can put the headset into "discoverable" mode with the red/blue flashing.
I can set up the Bluetooth device and pair it with the 0000 passkey.
It WILL NOT connect.
I CAN'T choose it from the Sound devices in system preferences.
How do I fix this?A2DP support can be added to Tiger, however, being that it is a "hack" you should backup your data at least twice before attempting. You should do this anyway before installing Leopard. To backup at least twice, follow these directions*:
http://www.macmaps.com/backup.html
Once you have the backup complete, see this hack:
http://blog.david.connolly.name/2007/06/stereo-bluetooth-profile-a2dp-on-mac-os. html
Note, with hacks, you may encounter difficulty upgrading your operating system.
Verify the hack works first with upgraded operating system before performing the the hack.
Also before upgrading your operating system, verify all your software and hardware is known to work with it first.

Pairing with a Bluetooth Headset
I have a Curve 8530 ... the BB can't find my fully-charged Plantronics 520 headset when I go through the Bluetooth Setup page. Is there something I'm missing or not doing?
Solved!
Go to Solution. No ... I didn't at the time ... the Verizon Wireless guy helped me out ... the headset I was trying to pair had a funky push-button pairing pattern ... once we got to pairing mode, no problems.
Sorry for the trouble! -

I can record voice memos fine using the built-in iPhone 4 mic. And my Bluetooth headset (Jawbone Era) works fine when I leave messages on voice mail systems etc. when calling on the iPhone 4, so it appears my headset mic is fine. I can also use voice activated dialing, although it fails miserably interpreting numbers. However, I cannot record voice memos with my Bluetooth mic. I just get barely audible static. Any suggestions? Thanks.
Hello, did you ever get an answer to your question? I just picked up a Jawbone Era and using on an iPhone 4s running 5.0.1. Seems to work fine on regular calls, but not on the built in Voice memos application. It worked fine on my older Jawbone Icon, but haven't tested on the 4s or iOS 5.
Thanks! -
