Followed the instructions from oracle:
To install and run:
- Ensure you have a JRE installed, if not, download here *
(For 64-bit Intel Macs, download Java for Mac OS X 10.5, Update 1,
For the latest version of MacOs, use the Software Update feature)
- Download the file above
- Extract the zip file into any folder, using folder names
- Within that folder, expand the datamodeler folder and execute datamodeler.sh
Do not understand what I need to do to get this application to launch. Requesting some assistance.

Thank you for your clear instructions, however, I am still having issues getting Oracle Datamodeler to start on my Mac. Maybe I have something basic wrong here, and someone can assist me further. As you can see from the below, my java version has been set by the Java Preferences to the Java SE 6, I have attempted to open up the permissions of the files, and changed the datamodeler.conf file to have the SetJavaHome parameter set to the Java 1.6.0 home. Still getting the same issue. Do you see any glaring issues with my java environment? Any other ideas?

  • Mac OS X Leopard Fails with Clean Access Agent

    Hey All,
I've had several students in my office saying that Leopard and Clean Access don't work together. I haven't seen a specific error, yet, but was wondering if anyone else is seeing this problem...?
I'm using the 4.1.2.0 agent for Macs.

I am at Cal Poly SLO and we have this error "Agent user operating system is not supported" Part of our system has been changed back to allowing Mac users to Authenticate using the web browser instead of the agent. That should work until cisco updates the agent

  • Launch failed with error code -2857

I've been running an app (EPSON Scan) for awhile without any significant problems. Today I check the Epson web site and they had an update for the app. So, I downloaded it, installed it, and tried to run it. Unfortunately, all I got was a couple of bounces in the Dock and nothing. I then went to the console and found the "launch failed with error code -2857" message. I've researched that as best I could and found that it is probably a problem with the application bundle. So, I opened the package, drilled down to the Mac OS folder, and double clicked on the EPSON Scan terminal command located there. Bing, a terminal window opened and the application launched and runs fine. So, the questions is, how do I resolve the CFM not finding the application correctly? I've run a pre-bind on everything but that didn't work. Seems to me like there is some config file that is not set up properly. I looked at the Info.plist file and I don't see any glaring problems. Any ideas? Epson also go a copy of this note...

First I want to say thanks for the feedback. I appreciate the support and helpful suggestions.
Second, Epson sent me an answer of sorts...they provided detailed instructions on uninstalling the application and then told me to reinstall using the disk image from their web site (in other words, don't use their web package to "update" and existing installation). I did this and things seem to be running fine now. It's unfortunate that their update instructions don't say to do this in the first place (maybe I missed it ).
So at one level this issue is solved. Unfortunately I'm too curious and wanted to know why the error happend in the first place. As a software developer myself (mainframe and midrange server based) I hate the "delete and reinstall" answer as it does nothing to really identify and solve the real problem. But I do also appreciate the idea of "just get it fixed fast" so that's what I did.
Anyway, I will now spend some time learning about Launch Services to see if I can get a more specific understanding of what happened.
    Thanks again for the help.

    Hi,
Try to put a machine disjoin the domain to see if the same issue occurs.
Sometimes the Windows 8 machines cannot use metro app once join the domain, you can try this method:
1- Open "Local Group Policy Editor"
2-Navigate to "Computer Configuration - Administrative Templates - Network Isolation"
3- Open "Internet Proxy Servers for Metro Style Apps" and set the value to your proxy server address like 172.16.0.1:8080.

  • Flash Player Install failed with error code: 30

Hi, I want to report a problem with the Adobe Flash Player 13.0.0.182 (Mac OS X, English) installer on OS X 10.9.2. Mavericks.  I'm posting this so the engineers will know what to fix in the next update version.
I have a 2013 iMac so according to Adobe blog posts it shouldn't be affected by the invalid cpu instruction issue.
Symptom is the first installer downloads the 2nd, starts to launch it, then quits it with "Error: Unable to proceed with the installation" and a green (success?) checkmark.  The GUI provides no other assistance except to "FINISH", which leads to the generic "Adobe Flash Player could not be installed." page.
The system log says: Adobe Flash Player Install Manager[22567]: Install failed with error code: 30.
    The FlashPlayerInstallManager.log says:

I have a 2013 iMac so according to Adobe blog posts it shouldn't be affected by the invalid cpu instruction issue.
Regardless... I still recommend "falling back" to 12 until they get this release right for everyone. I've read plenty of posts in the Apple forums from new iMac, Mini and MacBook users who've had nothing but trouble. Even the "gurus" there are linking to Chris Campbell's post here:
ATTENTION MAC CUSTOMERS - Flash Player 13 "Plugin Failure" Workaround
Using the offline installer completely eliminates the errors seen with the stub installer.

  • Time Machine backups to OS X Server intermittently failing with error 21

I have a Mac Mini running OS X Server, with two USB drives connected and selected within OS X Server as Time Machine backup destinations.  I have an iMac and a Macbook Air, both of which  are using Time Machine to backup themselves up to the Time Machine Service on the server (all are running the latest version of Mavericks).
Intermittently, the iMac backup fails.  The iMac's console log shows:
    Aug 23 13:55:34 Nigels-iMac.local com.apple.backupd[15418]: Failed to eject volume /Volumes/Backups-1 (FSVolumeRefNum: -141; status: -47; dissenting pid: 0)

    Aug 23 13:57:29 Nigels-iMac.local com.apple.backupd[15418]: Quota in effect for '/Volumes/Backups-1': Zero KB
    Aug 23 13:58:16 Nigels-iMac.local com.apple.backupd[15418]: Failed to eject volume /Volumes/Backups-1 (FSVolumeRefNum: -141; status: -47; dissenting pid: 0)
    Aug 23 13:58:16 Nigels-iMac.local com.apple.backupd[15418]: Giving up after 3 retries.
    Aug 23 13:58:16 Nigels-iMac.local com.apple.backupd[15418]: Backup failed with error 21: 21
The Mac Mini console log shows nothing apparently wrong.
Restarting the server (not the iMac) clears the problem and the iMac will then once again backup to the server for some time (e.g. a day).
There are two USB drives, one for each backup sparse bundle and although the two disks have different names in the Finder, OS X Server has named them both Backup (one is shown as 'Backup' and the other as 'Backup-1' in the Time Machine Services Backups tab).
I am wondering whether the failure occurs when coincidentally the iMac and the Macbook both try to back up at the same moment.  Any advice about how to isolate the problem, or a solution would be gratefully received!

    1. This procedure is a diagnostic test, to be carried out on the server. It changes nothing, for better or worse, and therefore will not, in itself, solve the problem. But with the aid of the test results, the solution may take a few minutes, instead of hours or days.
Don't be put off merely by the seeming complexity of these instructions. The process is much less complicated than the description. You do harder tasks with the computer all the time.
    2. If you don't already have a current backup, back up all data before doing anything else. The backup is necessary on general principle, not because of anything in the test procedure. Backup is always a must, and when you're having any kind of trouble with the computer, you may be at higher than usual risk of losing data, whether you follow these instructions or not.
There are ways to back up a computer that isn't fully functional. Ask if you need guidance.
    3. Below are instructions to run a UNIX shell script, a type of program. As I wrote above, it changes nothing. It doesn't send or receive any data on the network. All it does is to generate a human-readable report on the state of the computer. That report goes nowhere unless you choose to share it. If you prefer, you can read it yourself without disclosing the contents to me or anyone else.
You should be wondering whether you can believe me, and whether it's safe to run a program at the behest of a stranger. In general, no, it's not safe and I don't encourage it.
In this case, however, there are a couple of ways for you to decide whether the program is safe without having to trust me. First, you can read it. Unlike an application that you download and click to run, it's transparent, so anyone with the necessary skill can verify what it does.
You may not be able to understand the script yourself. But variations of the script have been posted on this website thousands of times over a period of years. The site is hosted by Apple, which does not allow it to be used to distribute harmful software. Any one of the millions of registered users could have read the script and raised the alarm if it was harmful. Then I would not be here now and you would not be reading this message.
Nevertheless, if you can't satisfy yourself that these instructions are safe, don't follow them. Ask for other options.
    4. Here's a summary of what you need to do, if you choose to proceed:
☞ Copy a line of text in this window to the Clipboard.
☞ Paste into the window of another application.
☞ Wait for the test to run. It usually takes a few minutes.
☞ Paste the results, which will have been copied automatically, back into a reply on this page.
The sequence is: copy, paste, wait, paste again. You don't need to copy a second time. Details follow.
    5. You may have started the computer in "safe" mode. Preferably, these steps should be taken in “normal” mode, under the conditions in which the problem is reproduced. If the system is now in safe mode and works well enough in normal mode to run the test, restart as usual. If you can only test in safe mode, do that.
    6. If you have more than one user, and the one affected by the problem is not an administrator, then please run the test twice: once while logged in as the affected user, and once as an administrator. The results may be different. The user that is created automatically on a new computer when you start it for the first time is an administrator. If you can't log in as an administrator, test as the affected user. Most personal Macs have only one user, and in that case this section doesn’t apply. Don't log in as root.
    7. The script is a single long line, all of which must be selected. You can accomplish this easily by triple-clicking anywhere in the line. The whole line will highlight, though you may not see all of it in the browser window, and you can then copy it. If you try to select the line by dragging across the part you can see, you won't get all of it.
Triple-click anywhere in the line of text below on this page to select it:

Copy the selected text to the Clipboard by pressing the key combination command-C.
    8. Launch the built-in Terminal application in any of the following ways:
☞ Enter the first few letters of its name into a Spotlight search. Select it in the results (it should be at the top.)
☞ In the Finder, select Go ▹ Utilities from the menu bar, or press the key combination shift-command-U. The application is in the folder that opens.
☞ Open LaunchPad. Click Utilities, then Terminal in the icon grid.
Click anywhere in the Terminal window and paste by pressing command-V. The text you pasted should vanish immediately. If it doesn't, press the return key.
    9. If you see an error message in the Terminal window such as "Syntax error" or "Event not found," enter
    exec bash
and press return. Then paste the script again.
    10. If you're logged in as an administrator, you'll be prompted for your login password. Nothing will be displayed when you type it. You will not see the usual dots in place of typed characters. Make sure caps lock is off. Type carefully and then press return. You may get a one-time warning to be careful. If you make three failed attempts to enter the password, the test will run anyway, but it will produce less information. In most cases, the difference is not important. If you don't know the password, or if you prefer not to enter it, press the key combination control-C or just press return  three times at the password prompt. Again, the script will still run.
If you're not logged in as an administrator, you won't be prompted for a password. The test will still run. It just won't do anything that requires administrator privileges.
    11. The test may take a few minutes to run, depending on how many files you have and the speed of the computer. A computer that's abnormally slow may take longer to run the test. While it's running, there will be nothing in the Terminal window and no indication of progress. Wait for the line
    [Process completed]
to appear. If you don't see it within half an hour or so, the test probably won't complete in a reasonable time. In that case, close the Terminal window and report what happened. No harm will be done.
    12. When the test is complete, quit Terminal. The results will have been copied to the Clipboard automatically. They are not shown in the Terminal window. Please don't copy anything from there. All you have to do is start a reply to this comment and then paste by pressing command-V again.
At the top of the results, there will be a line that begins with the words "Start time." If you don't see that, but instead see a mass of gibberish, you didn't wait for the "Process completed" message to appear in the Terminal window. Please wait for it and try again.
If any private information, such as your name or email address, appears in the results, anonymize it before posting. Usually that won't be necessary.
    13. When you post the results, you might see an error message on the web page: "You have included content in your post that is not permitted," or "You are not authorized to post." That's a bug in the forum software. Please post the test results on Pastebin, then post a link here to the page you created.
    14. This is a public forum, and others may give you advice based on the results of the test. They speak only for themselves, and I don't necessarily agree with them.

  • Time Machine backups suddenly failing with error 109 (console errors inc.)

I've been using Time Machine over my Airport Extreme Base Station for months without a single error, and all of the sudden it fails constantly with the error:
*The backup disk image “/Volumes/TARDIS-1/Derek’s iMac_001f5bf60eb1.sparsebundle” could not be accessed (error 109).*
I've tried selecting the drive again since it's mounted fine. It's mounted as TARDIS, not TARDIS-1, which I thought was a clue to the problem until I went to the Console to see all of the errors:

    9/15/09 Sep 15 - 8:13:41 AM com.apple.backupd[12333] Ejected Time Machine network volume.
I've seen others say that Time Machine quit working after updating to Snow Leopard, but i've been running it since launch and the problems only appeared yesterday. Ideas?

I'm not sure why you call it a "phantom" drive, because I was quite intentionally connected to that network drive. In any case, I did as you recommended, in case that was the issue (though over the months I've often been connected to the network drive). Sadly, no dice.

During the step "Making backup disk available..." in the Time Machine preference pane, looking in Volumes, I have:
    lrwxr-xr-x 1 root 1 Sep 10 19:48 Macintosh HD -> /
    drwx------ 13 root 398 Mar 10 2009 TARDIS
    drwxr-xr-x 1 derek 12288 Mar 5 2009 Untitled
So it's properly connecting to TARDIS, it would seem, as root.

  • What to do about Backup failed with error: The target volume is read-only

This is a weird story. My wife has a PowerBook G4/1.33 GHz that she uses in clamshell mode with a 20" Cinema Display. One day under OS X 10.5.6, her Time Machine backups (to a networked 500 GB Time Capsule) failed with the error: Backup failed with error: The target volume is read-only. So I upgraded her to 10.5.7 and erased the sparseimage file, starting a new Time Machine backup. And it just happened again. Any idea how this sort of problem arises? My wife was doing something at her computer when the Time Machine error occurred.
Please don't advise us to upgrade to 10.5.8, because her PowerBook freezes not infrequently with any OS later than 10.5.6, and I was just about to downgrade her from 10.5.7 to 10.5.6.

Try repairing the sparse bundle, per #A5 of the Time Machine - Troubleshooting *User Tip,* also at the top of this forum.
I've tried this before, but it can't be repaired, because it can't be mounted.
Any idea how this sort of problem arises? My wife was doing something at her computer when the Time Machine error occurred.
One possibility is, the HD in the Time Capsule is beginning to fail (all HDs fail, sooner or later).
I'm backing up to my own sparseimage bundle just fine.
Please don't advise us to upgrade to 10.5.8, because her PowerBook freezes not infrequently with any OS later than 10.5.6, and I was just about to downgrade her from 10.5.7 to 10.5.6.
Those two things may be related. Whatever's going wrong may very well be what's corrupting the backups.
I'd recommend finding out what's wrong with the Powerbook. It's impossible to tell, of course, but that may be the root of the problem.
There's a problem with PowerBooks and Cinema Displays. I've posted a lot about this. Even after a motherboard replacement, my PowerBook G4/1.67 GHz had freezes, and eventually Apple gave me a MacBook Pro, but I think there's a failure involving hardware/software design.
    If you haven't yet, I'd start by dong a +*Verify Disk+* on it's internal HD (also per #A5);
    That checks out fine.
if there are no errors, or they're fixed and the problem continues, run the +Apple Hardware Test+ from the disc that came with the Mac. (One of the discs should have that, and instructions for running it, printed in tiny type on the label).
If still no help, I'd be inclined to do an +Archive & Install, preserving users & settings,+ of Leopard, then download & install the appropriate "combo" update. That's a combination (thus the clever name) of all the updates to Leopard since it was first released, so installing it should fix anything that's gone wrong since then, such as with one of the normal "point" updates. Info and download available at: http://support.apple.com/downloads/MacOS_X_10_5_8_ComboUpdate (change the last "8" to the desired number). Be sure to do a +Repair Permissions+ via Disk Utility (in your Applications/Utilities folder) afterwards.

  • Time Machine. Backup failed with error code: 21

    Hi,
I recently upgraded to Leopard 10.5.4 mainly for the time machine backup. I'm using a DNS-323 in a RAID1 configuration as the TM backup drive. Also I have the DNS-323 connected to my Airport Extreme through the ethernet to give NAS ability to my home network.
After much hunting around the forums I got the sparsebundle created and copied to the DNS-323 share volume. See http://www.flokru.org/2008/02/29/time-machine-backups-on-network-shares-in-leopa rd/ My thanks to flokru.
However, TM is still not working. The console messages have the following:
Network mountpoint /Volume/Volume_1 not owned by backupd... remounting
    Network volume mounted at /Volume/Volume11
Failed to mount disk image /Volume/Volume1_1/MacBook001b6334c307.sparebundle
    Backup failed with error: 21
It would appear this error lies in the way TM is trying to mount the volume, specifically that Volume_1, the DNS-323 volume name, is not owned by TM. TM then tries to create a new volume, Volume11 and stick the sparebundle on it.
Is there a way of making TM "own" the volume, Volume_1?
Has anybody else seen this error or better yet, got a fix for it?

    James,
It would appear this error lies in the way TM is trying to mount the volume, specifically that Volume_1, the DNS-323 volume name, is not owned by TM. TM then tries to create a new volume, Volume11 and stick the sparebundle on it.
Is there a way of making TM "own" the volume, Volume_1?
Actually, it is not TMs failure to mount the volume that is causing your failure to backup. I have received the same Console message using a Time Capsule. However, Time Machines' attempt to 'remount' the volume always succeeds and the backup goes through. That appears not to be your experience, but it is NOT because the volume is not being mounted. It is.
    *Mount Point Conflict (TMDisk-1)*
NOTE: the following distinguishes “network drive” (the physical hard disk) from “disk image” (the sparsebundle file TM uses to backup to).
First of all, I am convinced that the second mount point that is created (in your case Volume11) is due to the fact that you already had the network drive mounted on your desktop at the time of the attempted backup. Was that the case?
When you manually mount a network drive the system creates a mount point in the /Volumes folder (Let’s call it TMDisk ). While that exists any attempt by Time Machine to mount the same disk will created a second mount point ( TMDisk-1 )
Nevertheless, as I have observed, this doesn't seem to prevent backups from taking place successfully, at least on the Time Capsule. And according to your Console logs it appears that your volume is being ‘remounted’ successfully.
*”The Backup Disk Image Could Not Be Mounted”*
The real issue is Time Machines' failure to mount the disk image (sparsebundle) contained on your network drive.
+Failed to mount disk image /Volume/Volume1_1/MacBook001b6334c307.sparebundle+
    +Backup failed with error: 21+
More often than not this is due to having the Time Machine backup disk image (sparsebundle) mounted on your desktop during a backup attempt.
Eject the backup disk image (sparsebundle) by either clicking the little Eject icon to the right of the disk image in the Finders’ Sidebar, or Ctrl-Click the drive icon on the desktop and select “Eject” from the contextual menu. Now try backing up again or launching Time Machine to view previous backups.
For more information, see an article on this here:
Alternatively, a backup disk image can fail to mount if there is a problem with your computer name.
    *Proper Computer Name* #
Make sure your computer has a proper name. Go to System Preferences --> Sharing. Time Machine needs to differentiate your computer from others on your network (i.e. "Bills MacBook" or "Office iMac"). If the "Computer Name" field is blank, create a name. Realize that if this step is necessary, you will likely have to start the Time Machine backup process over again and do another full initial backup.
According to THIS article [http://support.apple.com/kb/TS1760], Time Machine may experience problems if your computer name includes certain characters. Make sure the computer name only includes ASCII characters from the following set.
    (0123456789abcdefghijklmnopqrstuvwxyzABCDEFGHIJKLMNOPQRSTUVWXYZ)
Once a change in the computer name occurs, you should stop and restart Sharing on the affected computer. Uncheck and then recheck Sharing in the Services list on the left.
Let me know if any of this helps.
    Cheers!

Check the formatting and partitioning for your Time Machine disk. Launch Disk Utility from the dock or from the Applications > Utilities folder. Click on the upper icon for your Time Machine disk in the left window and the partition type will show on the bottom of the Disk Utility window. It should be GUID for Intel Macs and Apple Partition Map for PPC Macs.
Click on the indented icon below the upper Time Machine disk icon. That will show the formatting. It should be Mac OS Extended (Journaled). If the formatting and/or partitioning is not correct, reformat following the directions in this article.

  • Time Machine Backup Failed - Copy stage failed with error:11

My TM backups started to fail on a Mac Book Pro running 10.5.8 a few weeks ago with the 'Copy stage failed with error:11' message. It seemed to happen after coming out of hibernation from clamshelling the laptop.
This happened a few times before when I had initially set TM up so to 'fix' it I just blew away the TM backups and started fresh. I would rather not do that this time as I have accumulated 3 months worth of backups.
My TM is on a samba share, I've searched a bit and followed a guide:
    http://blog.robberthamburg.nl/mac-osx-leopard-repair-a-time-machine-error-11-pro blem
to try and fix the problem by deleting a .inProgress backup but this has not helped. unfortunately this has did not fix it. I had also read that there are sometimes problems with TM and Spotlight indexing so I have turned spotlight off completely. Still no luck.

These can usually be stopped by excluding the backups from any anti-virus scanning, and Spotlight indexing, at least on supported volumes. Unless you're sure you have Spotlight off entirely, try excluding your backups via System Preferences > Spotlight > Privacy.

Both of these appear to indicate problems on your backup drive, perhaps directory problems. For a supported one, a +*Repair Disk+* (not permissions) will often fix it. If there's a way to run that on your backups, it's worth a try.
It's also possible there's a problem on your internal HD, so it's worth doing a +*Verify Disk+* (not permissions) on it, too, via Disk Utility. If that shows errors, you'll have to boot from your Install disc and use it's copy of DU to repair them.

    any ideas why its failing?

    Hi,
I've seen the same error on a customer where we had Windows Intune Client (SCCM) installed previously. Did you try it on a newly deployed computer? If you had Intune or another CCM client on it previously make sure you clean it up. Here is a well formulated
    instruction:
    http://douwevanderuit.wordpress.com/2014/01/30/removing-windows-intune-client/
Usually when deploying to many clients we use a startup script, there is a great one that Jason Sandys made in the link below.
    http://blog.configmgrftw.com/configmgr-client-startup-script/
There should be no problems deploying SCCM 2012 R2 clients on Windows 8.1 unless there is something wrong with either the setup or the client computer.
All the best, Jesper Hassing - MCTS SCCM 2012 - MCSA 2012 Server - MCP

I think the installer finds some remenance of the old SMS_MP. When looking i found some remains in IIS . I tried to remove the remains but still have the same error when installing the client. I uninstalled IIS completly. Can someone tell me where the client
installer looks for the MP version? Would it help if i installed an SCCM MP on that server and the uninstalled it (Upgrade the MP)?

Their must be something remaining that ccmclean can't find or can' access so i ran ccmclean /mp. This did some stuff and told me the system was clean again. I reran the client setup.
    Guess what
    It worked !

  • FAILED with error NT_STATUS_NO_SUCH_USER

    Setup:
OS X Server 10.3.9. PDC. ALL clients are a combination of XP and W2K. No macs (sadly).
    Situation:
Over the last 3 years I've been plagued with this problem. Symptoms include new authentications are not granted access, but existing connections suffer no degradation of service. The simple solution has been to simply go into Server Admin and "Restart Service" on the Windows service. Done and done. However, this can happen anywhere from once a week to (up to) 3 times a day. Even on weekends when (likely) no one is in the building, though some users may not log out of their workstations - let alone shut down.
    Troubleshooting:
    1) I set the smbd logs to 1 (debug) and at the point of degrading, I only see this:
***** Begin: I changed nothing; I left out nothing ***********************

    2) Recently, I tried to change the default allowed connections per KB Article: http://docs.info.apple.com/article.html?artnum=107899 but that actually only seemed to exacerbate the problem. Today I removed the directories that were added in Netinfo Manager from the KB.
    3) I also looked at the nmbd.log and frequently see this, which is equally as troubling:

    Question:
How do I make it stop? Ok, to be fair, I've searched for the last few years for a solution and I'm not sure why samba is continuing to 'break.' Is there someone here that can enlighten me or poke me in the right direction to why this could be happening? I know little to nothing about samba. I DO know networking, and this could be the issue. The server was handed to me by the previous admin and so to rebuild (the easy solution) isn't so easy because the previous admin didn't document anything he did to build it, like NAT forwarding and such, which, if I don't put back into it, will cause MANY headaches. So, eventually I WILL rebuild from scratch (the time may be nigh), and I think it may fix a misconfiguration, but I fear it will be difficult. This is our production server with no test environment (other than using SuperDuper! to image the drive and play with it over a weekend or something).
    Plea:
    For help.
Edit: How do I suppress the wiki markup? It's butchering the posted log excerpts. <nowiki></nowiki> doesn't seem to work.

Turns out a printer driver installed on an XP (even W2K(?)) was (apparently?) flooding the OS X SMB server to the point of collapse. Uninstalling the "HP Tools" part of the driver cleared it up. The printer is an HP LJ1300. I had downloaded the full driver from HP.com. I don't know if any/all these conditions need to be matched, but: the printer was on the network using an HP print server JetDirect EX Plus, and the computer(s) in question were connecting directly to it (not via a print server). It's been too long ago, but there were always several errors in the System Log (Win XP Event Viewer) that correlated with the errors on the OS X server.
Proud to say that since that day (10+ months ago) I've not seen it happen again. whew.
