Need Urgent help in fixing the defect by Oracle EDQ

Question

Hi All,
I was trying to fix a defect in real-time individual matching using Oracle EDQ. I tried a lot but still unable to fix it.
The data in Customer Hub is as follows-
Firstname    Lastname                   Dob                                Type
David             Nelson                      12/08/201                       Customer
Dave              Nelson                      03/02/2014                     Affiliate memeber
David             Neilson                     06/11/1976                     Ordinary memeber
Dave              Neilson                     07/09/1985                     Prospect
If I give input driving record as-
Dave  Nelson then
Candidte record are found from CH as below--
Firstname    Last name          DOB                        type
David        Nelson            12/08/201                Customer
Dave          Nelson            03/02/2014              Affiliate memeber
Please look above table , first name is different but last name matches exactly.
If we give input  driving record as--
Dave  Neilson then
Candidte record are found from CH as below--
Firstname    Lastname            DOB                        type
David        Neilson            06/11/1976                Ordinary memeber
Dave          Neilson            07/09/1985                Prospect
We can see above that first name does not matches excactly where last name matches exaclty.
Our requirement is--
If we give input  driving record as--
Dave  Nelson then
all data from CH , having first name(David /Dave) and last name(Neilson/Nelson) should come as potential duplicate record.
I tried on  settting different compasrision configuration. But hopelessly,it is not working as we expect.
Can anyone among Oracle EDQ expert please help me in this dificult time. please give me some idea that which comparision configuration will provide us both duplicated recolds(Nelson/Neilson).

Thank you very much for quick response.
I was applying the  rule (Standardized Full name, full address) under rule group Standardize full name. 
The full data  in CH is ---
Firstname    Lastname           Dob                        Type                                StreetAddress         StreetAddress2        City     State        Country     postalcode

Dave              Nelson               03/02/2014              Affiliate memeber                3011                       George St              tamluk      PQR        Indonesia        1234

Looking on above data, it is clear that these are potential duplicates. Now we need to find it out by using EDQ match logic. As you said earlier we need to use cluster level 3(Exhaustive) . which one or all we need to  choose  among  ( A5F/OSP/FLM) to find out expected duplicates.
In our project, We had already used A5F cluster.  We were using four cluster FLP, EF9, A5F and PLN  in our project .Do We need to use FLM also.  Please tell us the  rulename and comparision configuration to find out all duplicates irrespective of Nelson/Neilson.
Our requirement is that if we enter driving record as  :
Firstname    Lastname                 StreetAddress         StreetAddress2           City         State           Country          postalcode
David             Neilson                           3011                       George St                tamluk      PQR            Indonesia                1234
All the above data( i.e. 4 records) should be found as duplicates. As I found that whether I gave input Dave or David , the ouput name is coming both  Dave and David . So the requirement is same for last name as well. whether we enter Nelson or Neilson , Duplicates should come in the combination of Nelson and Neilson .
Please help me .
