Remove + sign after adding character to list

I add a character to the script, say, "Mum". I then add that character to character lists by reason that there's a PLUS sign inviting me to do so. We then decide to give 'mum' an actual name e.g. 'Sandra'. Accordingly, we amend all those occurrences of 'mum' and replace with 'sandra'. All the time track changes is on. Problem is that when I replace the first 'Sandra' and click plus sign to add to the character list...no problem. However, on next occurrence, I have to enter another Sandra character to the list to get rid of plus sign i.e. double up. I then delete the extra Sandra out of the list, but plus sign then reappears beside name saying we don't have a character in the list with name 'Sandra'.
Is there a work around?

Character's name that appears in the script (screen name) can be different from the character's first and last name.
My guess is that the character 'Sandra' in your character list has screen name as 'Mum'
If that is the case then you can delete this entry from charcter list and add a new one whose 'screen name' is 'Sandra'
