AGP 8x acts extremely unstable

I'm running Windows XP.
Everytime I set the AGP speed from 4x to 8x, my computer goes extremely unstable. I am using ATI's Catalyst drivers; I've tried a number of different versions and it doesn't seem to change anything.  My screen freezes up every five seconds and VPU recover (part of the Catalyst Driver suite) informs me that the graphics card has been restarted because it has stopped responding.  
I'd like to figure this out so that I can take back the 250 lost 3DMarks that are rightfully mine.  
Any ideas as to why I can't use AGP 8x?

Thanks, netsile!  My monitor is the P95f+.  My desktop was set at 1280x 1024 @ 85 HZ.
It's not that AGP 8x failed during benchmarks or under complex rendering strains, but on the desktop.  I still can't manage to find side-band addressing, only write-combining and FastWrite.
By the way: my card doesn't use a 'Molex' connector.
I would like to try more troubleshooting now, but I have a much bigger PC problem to deal with.  Just the other day I decided to clear the CMOS to reset the BIOS defaults before I flashed the newest BIOS (1.9 or whatever, I'm currently using the version that came right out of the box).  I was looking forward to MAT, but I never even got far enough to flash the BIOS.  Upon switching the jumper back, plugging in and resetting  the PC, I get nothing.  No video, no nothing (remember, I didn't flash the BIOS).  It is very early in the morning and I need some sleep, so I switched the jumper back to the 'clear CMOS' position and popped out the battery.  I figured that it would figure itself out overnight.  
The next morning, I switch it on to more blackness.  All of the fans come on, the drives spin up, and then my motherboard piezo-buzzer speaker thing hits me with an error code: (beep-beep-beep-beep-beep-beep-beep-beep, and then some time, and then eight more beeps, on and on forever).  My monitor comes out of Power-save mode, like it knows that the computer has been activated, but then it goes to sleep again.  My PC stays on and keeps beeping in the 8-beep pattern until I unplug it.  
I looked up the error code on the internet, and I find that the error is the result of a graphics card read/write error.  I recheck everything in the box (I built it myself, I know where everything is) and restart again... and again... and again... The INFERNAL BEEPING never ends.  The D-Bracket on my PC is telling me that booting stops right before VGA initialization (LED pattern RRRG).  I think, "Oh great, my 9600Pro is fried, not cool" and proceed to remove it from the system.  
Later that night, I take the card to my Dad's office where I have constructed him a nearly identical PC, only difference being a Gigabyte motherboard, an older Radeon All in Wonder for graphics editing, and 2-3-2-6-1T Kingston instead of my 2-2-2-6-1T.  I take out his graphics card and check to make sure that there's no integrating video.  There isn't, so I hook up and power up.  I expect nothing to happen, but I am completely perplexed when the thing boots perfectly and continues to hum happily on the older Radeon's drivers that were previously responded.  
This test leads me to believe that maybe my AGP slot is messed up, or something.  I walk (in the 5 degree F weather that doesn't belong in central Pennsylvania) up to my friend's house up the street.  He has an affinity for building PCs out of ancient parts, and I pick up an old Voodoo 3 PCI card from him for testing.  I pop that card in my machine, and I get the same INFERNAL BEEPING.  Not cool.
Several calls to my IT professional grandfather do nothing to correct the situation.  At this time, I've cleared the CMOS probably 249865 times.  I unplug everything but the hard drive, CPU, and 1 stick (instead of the regular 2) of DDR 400 Kingston and reboot with the bare minimals.  INFERNAL BEEPING ensues.  
Now I take out the hard drive.  I'm not happy.
INFERNAL BEEPING.
Now I take out the remaining memory stick.  Ah! The beeping has stopped!  And my PC is gutted, and worthless... Not cool.  
Now, I've sent MSI some emails and they may even get a call tomorrow.
I've gathered the original packing material and bookmarked the RMA page.
If I do not discover the answers I seek on one of the 3927 message boards I'm going to post this on, I'm sending the thing back.  I've had it for less than four months, and I'm not going to run out and buy a new one when it's not my fault that it doesn't work.  
My dad's Gigabyte GA-8KNXP is looking nicer all of the time...

  • Mac is extremely unstable

    Hi,
    My PowerMac G5, which has been a highly reliable system, is suddenly extremely unstable. Apps are freezing (iTunes, iMovie, even Finder). Force Quit is frequently unable to completely exit the frozen app.
    Shut downs are also a problem, I am have resorted to forced reboot multiple times.
    I recently installed the ProKit 5.1 update. This would appear to be the root cause, but I don't know if that is the problem.
    I have run Disk Utility and repaired disk permissions. No error messages generated.
    Any thoughts on how I can get this machine back to it's usual reliable state are appreciated!

    For the shutdowns, I'm wondering if it might be overheating. And definitely check the amount of free space on the drive.
    If you don't have anything to read the temps, I'd install Temperature Monitor. It's freeware.

    To check the status of the fans + temp, the best is Hardware Monitor (Also Marcel Bresink) it costs around $10.
    _Not accurate_ for the temps, but OK for the fans is iStat Menus, which is free.

  • Digikam extremely unstable

    The last version of Digikam 1.5.0 is extremely unstable for me. It crashes very often just when browsing photos, it also displays broken thumbnails all the time. Am I the only one who experiences that? If not, is it problem with Arch Linux build, library dependences or just 1.5.0 is unstable?

    I don't know the answer. I have a problem with digikam too. It does not present any thumbnail.

    You have to tell us complete PC specs includding P.Su. brand and AMPS to 3.3, 5 and 12v. rails so we can help you better.- Right now I can say, Semprons FSB is 333, no 300, so, if you set it by hand in Bios, you have to set it to that new value.  Also you must have in mind that is better to run memory at same speed as CPU. Instability comes when memory send more data than CPU can process in a certain time. BTW Those Kingston memory are value Ram ? You also could try "upping " memory voltage in 0,1-0,2 volts to see if system is more stable. Y trata de no escribir con mayúsculas.

  • WRT54G with extremely unstable speed/ping.

    I seriously have no idea why this is happening, suddenly after the holidays my router has been acting up.
    When I ping my router (Linksys Wireless G (192.168.1.1)) it is usually between 1-2ms.
    However if I ping google or gamefaqs or something, the ping would be stable sometimes (RARELY) being at around 50ms or it would be extremely high at over 2000ms, or it would fluctuate from 50 to 500.
    I have not changed my settings on the router in anyway before the problem started. My isp is NOT the problem since if I directly connect the modem to the computer it would be a constant low ping.
    All the computers connected to the router is experiencing the same problem, I have updated the firmware for the router (no effect) and even changed the frequency of the router with the router's home page.
    I have tested this all with and without firewalls, with and without virus protection. I have tested it after scanning my whole computer for viruses and malware on safemode. I have resetted the modem by turning off the modem and holding reset on the router for 45 seconds then unplugging and replugging it.
    I have updated the firmware and resetted it after.
    This is an extremely frustrating problem as it makes it virtually impossible to play any games online, it also makes it extremely difficult to finish any of my University assignments without pulling my hair out with the load times, and I really wish someone on gamefaqs would be able to help.

    There's a lot of stuff to cover here...
    You said the router ping slows down when you ping both your router and Google at the same time. What is really important isn't how much the router ping slows, but what the ping times are to both your router and Google.
    It's nice to know you are on a cable modem. One problem might be the cable infrastructure is over-subscribed. One test is to get up in the middle of the night - something like 3 or 4 am would be good for this test - and see how your system performs. If it is OK, this suggests other users are saturating a link somewhere. Have you checked with your cable company? They might be able to help - but don't count on it.
    Shutdown all other machines on your network and perform the tests. One machine might have malware that is using all of your bandwidth. If everything tests OK with only your machine under test, bring other machines up one at a time to isolate the source of the problem.
    Are you using wireless for connectivity? If so, you are sharing bandwidth among not only yourself, but whatever neighbors are using the same wireless channel. If anyone is using 802.11b, that has the possibility to slow down all users of the wireless network. Having said that, I don't think this is your problem but it is something to keep in mind.
    Don't discount torrents as being a problem. They will suck up all of your bandwidth, and this can cause delays for other applications running on the network.

  • Macbook 2,1 extremely unstable?

    Hey, I bought a Macbook 2,1 with 2.0 GHz Core 2 Duo & 1 GB RAM about a year and a half ago. I'm using Mac OS X 10.4.11 (Tiger). I've always had random errors with it (kernel panics monthly, crashing weekly), but now it has become absolutely ridiculous.
    I recently installed 2 GB of PNY RAM, which could be causing some issues, but I've been having problems for quite a while before then. The DVD drive one day somehow got warped or something; it reads discs fine but there is considerable resistance trying to insert a disc, suggesting heat damage or external stress (however, the computer had just been sitting on the charger the night before I noticed this problem, so I'm guessing heat damage).
    Over the course of it's lifetime thus far, this Macbook has probably given me 30-40 kernel panics, hundreds of instances of programs closing due to errors, and numerous GUI issues (dock turning black, taskbar not loading, desktop icons not loading on startup, options having no result, computer not shutting down, etc). I have, within two weeks, zeroed the hard disk and reinstalled the OS three times (once, the installer ran into errors and would never finish, but keep telling me to start over). In less than 24 hours since the last reinstall, I've encountered 2 kernel panics and the dock has once turned black.
    I need help. I'm a college student who absolutely needs a stable computer for work and for school, and this Apple has NOT delivered that for the past six months. I also work in MIS for a local company and have tried to find diagnostic tools for Mac OS X - but to no avail. Most of my friends have never even seen a kernel panic or know what one is, yet somehow I manage to have them randomly thrown at me every week. Safari closes due to errors all the time. Firefox does the same. Pages does the same. MS Word 2008 does the same. Garageband does the same. I want to like my Mac, but I simply can't like a computer that is extremely unpredictable and crash-prone. Help is much appreciated. Thanks.

    Why did you wait a year and a half to solve your kernel panics?
    If you got them out of the box, you should have returned the computer then while there was a return policy in effect?
    If you got them in the first year, you may have had a hardware issue that could be resolved. I'm assuming of course you haven't purchased AppleCare, otherwise you'd be calling them to ask why you are getting issues. If AppleCare is giving you an unreasonable response like erase your machine before sufficient troubleshooting is done, certainly, feel free to ask us how to solve the problem.
    Apple has had many a stable machine. It sounds to me you just got a lemon, and should have returned it or asked it to be repaired under warranty.
    Now that it is over a year old, if you don't have AppleCare, you are going to pay through the nose to get it repaired. You should always buy AppleCare within the first year.
    So here are my questions for you:
    1. Do you have AppleCare?
    2. Is your data backed up?
    3. Have you looked at the exchange/repair programs?

  • IPhoto 08 extremely unstable with external

    Good evening
    I am very frustrated with iPhoto as the application keeps crashing every 3-4 minutes. I have my pictures stored on an external drive and have created a library also on the external. When I imported all my photos over to the library, I used the advanced option of not actually copying the items to the library (1.4 gig; actual photos 'imported' are 34 gig = 16000 photos).
    Are all these advanced and external options combined making my iPhoto unstable?
    I have the latest iPhoto and MAC software updates installed.
    Any thoughts or suggestions? I want to keep my MACBook Pro somewhat uncluttered of photos so that I can edit videos!

    Welcome to the Apple Discussions.
    No, neither of these things will make iPhoto unstable.
    Post the Crash Log.
    It's at Home / Library / Logs / Crash-Reporter / iphoto.crash.log
    Double click on the file and it will open in the Console. Copy and Paste it in here.
    Just a heads up: You need to be aware of a number of potential pitfalls using a referenced Library.
    1. Import and deleting pics are more complex procedures
    2. You cannot move or rename the files on your system or iPhoto will lose track of them
    3. Most importantly, migrating to a new disk or computer can be much more complex.
    Always allowing for personal preference, I've yet to see a good reason to run iPhoto in referenced mode unless you're using two photo organisers.
    If disk space is an issue, you can run an entire iPhoto Library from an external disk:
    1. Quit iPhoto
    2. Copy the iPhoto Library as an entity from your Pictures Folder to the External Disk.
    3. Hold down the option (or alt) key while launching iPhoto. From the resulting menu select 'Choose Library' and navigate to the new location. From that point on this will be the default location of your library.
    4. Test the library and when you're sure all is well, trash the one on your internal HD to free up space.
    If you're concerned about finding the files, that can be easily done from the iPhoto Window.
    There are many, many ways to access your files in iPhoto:
    For 10.5 users: You can use any Open / Attach / Browse dialogue. On the left there's a Media heading, your pics can be accessed there. Apple-Click for selecting multiple pics.
    To upload to a site that does not have an iPhoto Export Plug-in the recommended way is to Select the Pic in the iPhoto Window and go File -> Export and export the pic to the desktop, then upload from there. After the upload you can trash the pic on the desktop. It's only a copy and your original is safe in iPhoto.
    This is also true for emailing with Web-based services. If you're using Gmail you can use THIS
    If you use Apple's Mail, Entourage, AOL or Eudora you can email from within iPhoto.
    If you use a Cocoa-based Browser such as Safari, you can drag the pics from the iPhoto Window to the Attach window in the browser.
    Or, if you want to access the files with iPhoto not running, then create a Media Browser using Automator (takes about 10 seconds) or use THIS
    Other options include:
    1. *Drag and Drop*: Drag a photo from the iPhoto Window to the desktop, there iPhoto will make a full-sized copy of the pic.
    2. *File -> Export*: Select the files in the iPhoto Window and go File -> Export. The dialogue will give you various options, including altering the format, naming the files and changing the size. Again, producing a copy.
    3. *Show File*: Right- (or Control-) Click on a pic and in the resulting dialogue choose 'Show File'. A Finder window will pop open with the file already selected.
    You can set Photoshop (or any image editor) as an external editor in iPhoto. (Preferences -> General -> Edit Photo: Choose from the Drop Down Menu.) This way, when you double click a pic to edit in iPhoto it will open automatically in Photoshop or your Image Editor, and when you save it it's sent back to iPhoto automatically.

  • Extremely unstable wireless connection

    Since Saturday my connection just stops working every 15-30 minutes. Disconnecting and reconnecting solves the problem, but I'm sure there's a better option than netcfg -r'ing all the time.
    There was a kernel update on Friday (3.1.9 -> 3.2.1), so I suspect that could be the problem. Any ideas?
    EDIT: I noticed there was a new kernel update available, the connection seems to work for now... It worked for over 2 hours yesterday before acting up, though, so I'm not sure if the problem's actually solved yet.

    Well, there's only so much you can do on your own. Still, there are some things for you to consider.
    First, we still don't know if it's a bug that only affects you. It may be more widespread, but I still wasn't able to find anyone reporting similar trouble as you have. It may be caused by the fact that your wifi card isn't that common - it should be supported by the kernel, but the fewer people use a device, the longer it may take until a bug gets recognized and fixed. It still may be caused by something in your system, but I'm out of ideas on how to figure that one out. And since it's a kernel we're talking about, I'm of the opinion that the kernel developers should be made aware of the issue. Here are some guidelines on how to report kernel bugs, but it would be appropriate if someone more accustomed to this process could step in now and give more information on how to proceed. I never had to do something like that myself, so what I'm saying is just what I could find out by googling and directly on kernel.org.
    And second, reporting a bug won't fix your immediate problem. Even if you report it, we don't know if it ever gets any serious attention, and even if it does, when it's going to get fixed. Staying on outdated kernel can hardly be called a solution either. So, these are the only options I think you have, besides reporting a bug, which you should do either way:
    - keep using older kernel and watch changelogs and your bug report mailing list for any development
    - be ok with having to reconnect every now and then while using fully updated system, and again keep watching the situation. You may consider using tools like networkmanager or wicd, which should be able to reconnect you once you lose connection
    - since you're using a desktop PC (which I assume you don't move around), consider using ethernet connection for now if that's an option
    - get a new wifi card. Simplest solution available, if working wifi is your only option and you can spare the cash, they aren't expensive nowadays anyway. Obviously not a very elegant solution, but it will work for sure
    Sorry I couldn't help any more. Again, someone please step in if you have any more ideas.

  • Apps extremely unstable

    My iPad has always been a bit unstable, but it has really gotten worse the last couple of weeks. The first couple of months, only Safari and iCab Mobile crashed, although they did it 1-2 times a day during a ~2-4 hour usage.
    Now, my browsers crash ~5 times a day, and all other apps also crash on a regular basis (I assume at least one crash every 2 hours). Yesterday I wanted to show a friend the Smurf Village app, and each time I clicked on the "Show featured village", which usually works, it repeatedly crashed (Tried 5 times, even shutting down the iPad). No changes at all to the app or iPad, it just randomly decided to start crashing.
    In ~/Library/Logs/CrashReporter/MobileDevices/, I have currently 579 crash logs from my iPad, of which 292 of those are LowMemory crash logs.
    Is this really to be expected, or should I take it to Apple?

    I'm having the exact same issue with my iPad (verified my crash logs for iPad only), and I suspect I may be having it on my iPod Touch (haven't looked at crash log yet) as well.  I called tech support a few weeks ago and they recommended that I do a factory reset in case I have a corrupt app(restore and manually reload, do not reload from backup because the backup may also be corrupt). 
    I did this for both my iPad and iPod Touch and the issue went away, but only temporarily.  After about 2 weeks of peaceful, reliable operation, both devices went back to the repeated crashes.
    I'm going to call Apple back for support on this.  If it is a corrupt app, it is corrupt again and I need to figure out how not to keep repeating this madness.  Both devices are pretty much worthless this way.  You can't keep an app running for more than a minute or two and battery life seems to be cut in half.  I'm actually worried I am going to shorten the life of the batteries on both devices.

    my system, on a scale of 1-10(10 high) of how unstable my computer is with my cpu at 133FSB..... *34*

    Make sure the HSF and CPU are clean. (read the sticky on how to clean them)
    Once they are clean put a VERY small amount of grease to the CPU core (the small metal bit in the middle of the CPU). You only need enough to smear over the core. About the size of a rice grain. Place the HSF on the CPU and clip it in place.
    Now for a little trick I use to help. VERY gently twist the HSF a fraction one way then back again. This helps disperse the grease and give the best metal to metal contact possible.
    Remember. Smear of grease and GENTLE  twist.

  • What's with Safari 6? Extremely unstable, scrolling lags, typing lags...

    I upgraded to Safari 6 just before (using Software update, I'm using 10.7) and... it's completely unusable.
    I tried to post this question using Safari but it crashed 4 times, so I'm in Chrome now. (No crash report dialog appeared to report to Apple.)
    And it's slow: There are huge lags in typing. And long scrolling lags.
    It's basically unusable. That's not an exaggeration - it's seriously completely unusable. Just the crashing alone makes it unusable.
    Wasn't this tested?
    I usually use Safari as my main browser and Chrome for testing, but I'm going to have to use Chrome as my main browser until this is fixed.
    Anybody know what's going on?

    To help ascertain part of the problem - open Activity Monitor (it's in your Applications folder in another folder called 'Utilities')
    Open that, in the Processes dropdown, select 'All Processes'
    In the list of 'processes' check and see how much CPU % that the Safari and Safari Web Content processes are using.
    For me, they have been consistently using 60-110% of my CPU% even when I have only a couple pages open - even with Flash disabled, Extensions turned off, etc.
    This is the problem at its core.
    I've also noticed that my IM app Adium is taking an abnormal amount of CPU % now also, I suspect because it also uses WebKit to display a message window.
    Which makes me think there is a large bug in Safari 6's implementation of WebKit - which can affect any application that uses a webview to display content.
    Not good.
    Let me know if the high CPU% issue is true for you guys also.

    This bug check indicates that the Intel CPU generated a trap and the kernel failed to catch this trap.

    The 1st parameter of the bug check is 0x00000008, or Double Fault, indicates that an exception occurs during a call to the handler for a prior exception. Typically, the two exceptions are handled serially. However, there are several exceptions that
    cannot be handled serially, and in this situation the processor signals a double fault. There are two common causes of a double fault:
    A kernel stack overflow. This overflow occurs when a guard page is hit, and the kernel tries to push a trap frame. Because there is no stack left, a stack overflow results, causing the double fault.
    A hardware problem.

    We seem to be at DISPATCH_LEVEL at the time of the crash, so there's nothing wrong there. I suspect a hardware problem despite the problem stemming from acquiring a spinlock.

    Which should I download?
    You can either download the pre-compiled ISO that you would burn to a CD and then boot from the CD, or you can download the auto-installer for the USB key. What this will do is format your USB drive, make it a bootable device, and then install the necessary
    files. Both do the same job, it's just up to you which you choose, or which you have available (whether it's CD or USB).
    Do note that some older generation motherboards do not support USB-based booting, therefore your only option is CD (or Floppy if you really wanted to).
    How Memtest works:
    Memtest86 writes a series of test patterns to most memory addresses, reads back the data written, and compares it for errors.
    The default pass does 9 different tests, varying in access patterns and test data. A tenth test, bit fade, is selectable from the menu. It writes all memory with zeroes, then sleeps for 90 minutes before checking to see if bits have changed (perhaps because
    of refresh problems). This is repeated with all ones for a total time of 3 hours per pass.
    Many chipsets can report RAM speeds and timings via SPD (Serial Presence Detect) or EPP (Enhanced Performance Profiles), and some even support changing the expected memory speed. If the expected memory speed is overclocked, Memtest86 can test that memory performance
    is error-free with these faster settings.
    Some hardware is able to report the "PAT status" (PAT: enabled or PAT: disabled). This is a reference to Intel Performance acceleration technology; there may be BIOS settings which affect this aspect of memory timing.
    This information, if available to the program, can be displayed via a menu option.

  • Azure Web Apps extremely unstable following switch from VMs

    Last week we made the switch from Azure VMs to Azure Web Apps (web sites). The main reason for doing so was to take advantage of continuous deployment, auto-scaling and deployment slots.
    We have 4 web apps running on S2 instances (currently scaling up to 2 instances). We then have 4 other virtual machines that host our databases (RavenDB) and another web app that cannot be hosted on Web Apps. We're using VNETs to connect everything together.
    Previously the entire system was hosted on just 2 virtual machines.
    A week on we're finding
    a) Performance has degraded
    b) The Web Apps (and associated service) are increasingly unreliable
    We used to deploy frequently throughout the day without issue. Since moving to Web Apps we regularly experience 503 errors for up to a minute following a deployment. We switched to staging slots to try and circumvent this but even this hasn't been foolproof.
    We're now actually reluctant to deploy changes to our API during the day because it has been so unreliable.
    Performance has also suffered - we're using New Relic for monitoring and our throughput has halved since moving to Web Apps despite the web apps actually having more resources than they did before.
    Auto-scaling has also been very hit and miss. Our apps rarely put any load on CPU but they can use quite a lot of memory. We set up auto-scaling based on memory but rather unhelpfully the new portal changed our "less than or equal to" scaling rule
    to "equal to" meaning that the instances never scaled down - thanks Azure! I believe this is still an issue in the portal.
    We've gone from almost 6 months of 100% uptime to a week of regular issues. We deployed a new web app today and attached it to the same to virtual network used by our other apps and it caused a disruption in the other apps - our API could no longer contact
    the virtual machine hosting RavenDB.
    All-in-all this has not been a great experience and if it's a choice between reliability/performance and the bells and whistles that Azure Web Apps provide, I'll be going back to VMs.

    It's an ASP.NET Web API application and we're deploying with Web Deploy.
    When we first switched to Web Apps and deployed directly to the "live" instance we found that we were getting 503 errors. There were no errors in our application logs (Elmah) so I'm assuming this is an issue outside of the application. We do have
    250 or so sites that are serviced by the API so I would expect a slight delay whilst the application starts but certainly not a 503. In some cases we had to re-deploy (the same code) to get things back.
    We then switched to using Staging Slots to try and avoid this issue. Whilst this avoided the 503 errors most of the time, the time it takes for the application to come back up following a deployment has increased dramatically - and this is despite "hot
    swapping" from staging to production.
    Yesterday I tested this by deploying to staging and then swapping the deployment. It took over 30 seconds following the swap before the API was available. When I used to deploy to the VM it was available almost immediately.

    i updated to 4.0.1 a couple days ago. no problem doing the update, downloading the new version. but i seem to be missing certain things (reload and stop "buttons") on my new version as well as having some things not functional (back and forward "buttons"). i'm not able to create links or hyperlinks at websites that i was able to create them at before this new version. that's just what i know of that's "wonky", who knows what else isn't in my version or isn't functional?

    Firefox 4 uses a combined Stop/Reload/Go button that is positioned at the right end side of the location bar.
    During the page load process it shows as a Stop button and after the loading has finished the button is changed to a Reload button.
    If you type in the location bar then that button becomes a Go button.
    Middle-click the Reload button to duplicate the current tab to a new tab.
    Make sure that all the toolbars like the "Navigation Toolbar" and the "Bookmarks Toolbar" are visible: "View > Toolbars"
    * If items are missing then open the Customize window via "View > Toolbars > Customize" or via "Firefox > Options > Toolbar Layout"
    * If a missing item is in the toolbar palette then drag it back from the Customize window on the toolbar
    * If you do not see an item on a toolbar and in the toolbar palette then click the "Restore Default Set" button to restore the default toolbar set up.
