Creating Yes/No boxes

I am using a MacBook. When I create a Yes/No fill box for a pdf i am creating, the answer the user provides is automatically filled in all of the other Yes/No boxes created. This happens event though i have not copied and pasted these boxes and have built each one. Anyone have any thoughts on how to keep this from happening?

Yes, it has to be saved as a dynamic PDF.
Additionally, there are some name mismatches in your form so the script was failing. My script defined page 1 as 'page1' while your form defined page 1 as 'Page1'. I renamed your form page 1 to 'page1' and the script worked fine.
I would recommend enabling the debugger in Acrobat for form developement. See Edit > Preferences and enable the following:
I added an additional button with some script on the button 'click' event.
The purpose is to intervene in the submission process, in your case submitting by email. With a single submit button the email dialogue fires regardless of failed form validation. By introducing a second button you can do form validation before enabling submission. The attached form has a visible button called 'submitBtn' and your original 'Submit By Email' button which is declared as 'invisible'. My 'submitBtn' is labelled 'Submit By Email'. When the button is clicked is does form validation. If validation succeeds, I call form1.page1.Button1.execEvent("click"); to submit to the email address attached to 'Button1'.

Creating Dynamic select boxes

Can we create dynamic select boxes which will change when the user selection changes. How many levels we can go about.
For example
we have one combo box named states--> depending upon the states selected the corresponding districts should be shown in the other combo box, and when the user selects a district it should show the corresponding villages or towns and further..
Can we do it without ajax concept. if yes how?
If we have to use ajax one level is easy how to go about two or three levels.

it is possible when you implement Model and by reference of that
model you can change values of that combobox such as.
model1.removeAllItems();
model1.addItemAt(location); etc

Contact box? Your question is not clear perhaps if there was one and only one standard contact box on planet Earth there would probably be more the one correct answer to you question. Please describe what your desired in the PDF file. If you want something that can be filled in like a contact form you may need to use a different Adobe product like Acrobat. Or something to create an HTML Page with a contact form.

How do I create a check-box matrix question using Acrobat XI Pro?

I would like to insert a question that is a 10 x 10 matrix that allows respondents to select multiple boxes in each row. For example, the "add item > rating scale" feature formats a matrix the way I would like, although it only seems to allow for radio buttons. With this feature, respondents can only select one item per row. Is there any way to use this feature with check-boxes rather than radio buttons? Or a different feature to create a matrix that allows for multiple selections per row?

I created a 30 page Form in Microsoft Word with 20 to 30 check boxes on each page using the Zapf Dingbats Font.
When I used Acrobat XI Pro to automatically convert the form it fails to create usable check boxes.
It worked properly about 3 times on test pages but it will not do it again.
Manually inserting check boxes one by one on 30 pages is mind numbing to say the least.
Why doesn't the software work properly anymore?

As I said mail servers do have management interfaces. You need to find one and then determine what the management interface is.

It has a createDepartments.jspx page that allows users to move the input text fields around. You can use code similar to the one used here to move panelBoxes

Issue with creating a text box from a loop

I was hoping someone could take a look at this code and maybe
help me find out why this isnt working. What I am trying to do is:
(NOTE: I have used a few differents scripts and combined them, so
the code may not be the greatest).
Pull Data from a PHP file (The PHP file gets it's data from
database)
Then run a loop that brings in First Name, Last Name, Jersey
Number, and Position
Then before it loops and gets the next player it creates an
array that creates 3 text boxes with Full Name, Jersey Number and
position
then it loops through the process again untill all of the
records have been retreived.
I'm not sure if its because I have a do while loop inside of
a for loop inside of a function and an Array. I'm new to Arrays and
loops. the issue is it wont display the text boxes.
NOTE: When I trace this, the data is being retrieved.

instance, receive. and the loadvars class has no createTextField()
method, so you're not going to create any textfields if that code
is in the onLoad() body.
to remedy, reference a timeline instead of receive.

Using Automator to create a text box of certain size

I'm very new to using automator... in fact I haven't used it at all, but I am wondering if I can use it to create a text box of the same size each type to type in within preview (a PDF document). Basically I want to click or push something and have a text box be generated that is the same size each time.

- Get Specified Finder Items, and point it towards the folder that will contain the movie.
- Get Folder Contents - to get what's inside the folder.
- Filter Finder Items - set to filter by 'Name Extension' 'Is equal to' 'mov' and make sure all your video files have the right extension...
- Open Finder Items - in QT Player - note that this might cause problems if there are multiple files in the folder...
Then from the QT Player actions:
- Play Movies.
2. Save As... Application, then after saving, drag the app into the login items for the user account.

What program would allow me to create a mini box of video on screen?

I'm having trouble figuring out what program would allow me to use an effect that would create a small box of video on screen while the rest of the screen remains blank. It'd be great if there was actually a term for this, but if there is, I don't know it. I once used this effect in a silly beginner program called "studio" that allowed me to create one or more small boxes of video footage, smaller than fullscreen. I don't think this effect exists on imovie, but I have final cut pro, if anyone knows what the effect is called and how I could use it.
Please note: I'm not looking to create those four square boxes of the same image on screen (as can be found in imovie effects), I'm looking to choose the number of boxes I want and what goes in each one (i.e. different footage inside each box).
Sorry if this post is unclear, it's hard to describe when I don't know the name for it, but I hope you can help!

To scale down the video as videodude suggested, load the clip into the Viewer window, click on the Motion tab, and use the Scale control to resize the clip and the Center control to set it's position.
Or set the Canvas window in Image+Wireframe mode and use the mouse to resize and position the clip.

You can try it for your requirement by creating a dialog page of your own, say XXDialogPage (with OK and Cancel button)

The sap list box allows to select a value from the list but we cannot enter our own value in the list box .The value list that will be displayed consists of two

In screen painter to create a input/output field into list box we use
'L" as a value for dropdown attribute for the i/o field.
In screen painter to determine the type of method that will be used to fill the value
list we use the attribute value list.
If it is blank the value list will be filled by the first column of the input help assigned to the screen field.This input help can be defined in the ABAP Dictionary, on screen using SELECT,VALUES screen statements or in event POV (PROCESS ON VALUE-REQUEST ) and the input help that will be passed to the field should consists of 2 columns ,the key column is filled automatically by the system.SAP recommends value list field should be blank.
